    Notes: Summary: Mitogen-activated protein (MAP) kinase phosphatase-1 (MKP-1) is encoded by the mitogen-inducible gene 3CH 134. MKP-1 has recently been shown to be a dual specificity (serine/threonine, tyrosine) protein phosphatase, which dephosphorylates and inactivates MAP kinases in vitro and in vivo. to seek the role of MKP-1 in growth regulation of mesangial cells, expression of MKP-1 mRNA in cultured mesangial cells and in glomeruli isolated from anti-Thy 1.1 mesangial proliferative glomerulonephritis rats was studied. the effect of inhibition of endogenous MKP-1 by use of antisense-DNA technology on the regulation of MAP kinase activity and the growth regulation of rat cultured inesangial cells was also studied. By northern blot analysis, it was demonstrated that in mesangial cells, MKP-1 mRNA expression was rapidly induced after the stimulation by serum, growth factors and vasoactive peptides. Maximal signals were found in 30-60 min in all growth factors tested. Fetal calf serum (FCS) was the most potent stimulus of MKP-1 mRNA expression, followed by platelet-derived growth factor (PDGF)-B and arginine vasopressin. to elucidate a possible involvement of MKP-1 in disease development of mesangial proliferative glomerulonephritis, MKP-1 mRNA expression was examined in rat anti-Thy 1.1 glomerulonephritis model. A marked increase in MKP-1 mRNA level in isolated glomeruli was observed at day 3 after disease induction (4.3-fold over control). In situ hybridization of MKP-1 mRNA in Thy 1.1 glomerulonephritius rats confirmed the enhanced glomerular expression of MKP-1. to study the role of MKP-1 in mesangial cell growth regulation, phosphorothioate oligodeoxynucleotide (ODN) were used to modulate MKP-1 expression. an antisense ODN targeting the translation initiation site of MKP-1 mRNA inhibited stimulated (by FCS, or PDGF-B) DNA synthesis and FCS- or PDGF-induced mitogenesis in mesangial cells. Sense ODN or mismatched ODN had no effect on the DNA synthesis or mitogenic response of mesangial cells. These results suggest that MKP-1 is an immediately early gene in rat mesangial cells and it plays a critical role in growth regulation of mesangial cells in vitro.

    Notes: Summary: Recent progress of genetic engineering allows us to create animal models expressing the new genetic phenotype and also indicates a sure future for clinical use of gene therapy. We applied HVJ-liposome method for manipulation of transforming growth factor (TGF)-β gene expression in anti-Thy-1 experimental glomerulonephritis. Either the glomerular introduction of TGF-β antisense oligodeoxynucleotides or transfection of gene for decorin, a natural inhibitor of TGF-β, into the skeletal muscle could suppress the extracellular matrix (ECM) expansion in glomerulonephritis. Thus, these results may suggest the potential of gene therapy as a novel treatment for fibrotic diseases caused by TGF-β.

    Notes: Abstract. Intravascular ultrasonography (IVUS) was performed to diagnose portal vein invasion by pancreatic carcinoma. In 55 patients with pancreatic carcinoma, the intravascular ultrasonographic catheter was introduced during operation through a branch of the superior mesenteric vein into the intrahepatic portal vein. The catheter was gradually withdrawn, and the cross-sectional images of the area under investigation were recorded. Altogether 45 patients underwent resection, and 34 underwent combined resection of the portal vein. The IVUS findings were compared with the histologic findings of the resected specimen, preoperative computed tomography (CT), and arterial portogram. The wall of the portal vein was visualized as an echogenic band with a thickness of 0.5 to 1.0 mm. A subtle portal vein invasion can be detected by observing this portal vein wall. For the diagnosis of portal vein invasion, the sensitivity, specificity, and overall accuracy of IVUS were 99.6%, 92.3%, and 94.5%, respectively. The equivalent values for portography were 79.3%, 61.5%, and 70.9%; and those for CT were 82.1%, 74.1%, and 78.1%. IVUS provides more valuable and accurate information on portal vein invasion by pancreatic carcinoma than either CT or portography.

    Notes: Abstract Postoperative delirium is a common complication which can interfere with the surgical treatment and recovery of elderly patients, and is likely to prolong their hospitalization. Unfortunately, there is as yet no completely effective pre- and/or post operative technique of patient care to reduce or prevent postoperative delirium. In this study, 36 patients aged over 70 years undergoing gastrointestinal operations were assessed to examine the relationships between the preoperative cognitive state, the postoperative sleep cycle, and the occurrence of postoperative delirium. All patients were evaluated preoperatively using the revised version of Hasegawa's dementia scale (HDS-R). We correlated those test results and assessed the sleep-wakefulness disturbance postoperatively, to obtain a clinical DMS-III diagnosis of postoperative delirium. The incidence of postoperative delirium was 17% (6/36). The patients who developed postoperative delirium demonstrated preoperative cognitive impairment, and had a short sleep period during the night and a long sleep period during the day. Postoperatively, these results suggest that HDS-R is a useful method of evaluating preoperative cognition in elderly patients. Considering that sleep deficiency is likely to predispose elderly patients to postoperative delirium, techniques to prevent sleep deprivation may be of considerable value in minimizing the incidence of postoperative delirium.

    Notes: Abstract We report herein a case of cholangiocarcinoma with large cystic lesions. Computed tomography (CT) demonstrated large cysts in segment IV and the paracaval portion of the caudate lobe, and a solid tumor in the anterior segment of the right lobe of the liver which was contiguous to the cyst in the paracaval portion of the caudate lobe. The large cysts were diagnostically misleading and a liver abscess was suspected. Thus, percutaneous transhepatic drainage of the cyst was performed. The fluid in the cyst was negative bacteriologically, but malignant cells were detected. A CT scan done 2 weeks after drainage of the cyst showed progression of the solid tumor with intrahepatic metastasis and replacement of the cystic lesions by the solid tumor. Following percutaneous transhepatic portal embolization, a right hepatic trisegmentectomy with caudate lobectomy was performed. Pathological examination confirmed cholangiocellular carcinoma. These results indicate that drainage of the cystic lesion induced the tumor progression in the liver. Therefore, the possibility of cholangiocarcinoma with a large cystic lesion should be borne in mind when considering the differential diagnosis of a cystic lesion in the liver, and appropriate surgical therapy should be carefully selected.

    Notes: Abstract  In order to examine the effects of mild to moderate obesity, moderate to heavy smoking, and moderate alcohol consumption on cardiac parasympathetic activities and systemic sympathetic activities, a cross-sectional survey was carried out in 282 healthy Japanese male white-collar workers. Their autonomic activities were assessed as amplitudes of spectral components of heart rate variability (HRV) which was measured in the annual physical examination at their work sites. Taking the effects of aging on HRV into account, the cardiac parasympathetic activity at supine rest and its response to a change in posture were reduced in mildly to moderately obese subjects with a body mass index of 21–36, whereas the sympathetic activity was not. The effects of smoking and alcohol consumption on HRV were not confirmed. The above results means that we should consider obesity as a covariate when we examine possible relationships between cardiac parasympathetic activity and other environmental factors. There is a need for further studies on the relationships among obesity, change in parasympathetic activity, and development of health problems. The dose-effect relationships between long-term smoking or alcohol consumption and chronic changes in autonomic activities also remain to be determined.

    Notes: Abstract Eighty patients with pancreatic carcinoma were treated by intraoperative radiotherapy (IORT) with or without surgical resection of the tumor, and the results were compared with those of 111 patients treated by surgery alone. For resectable patients, the radiation dose was 30 Gy and the average field sizes were 8 or 10 cm; for unresectable patients, these values were 20–30 Gy and 6 or 8 cm, respectively. No side effects of IORT were observed. In 49 resectable stage III patients, the IORT group (n=16) had a higher survival rate than the non-IORT group (n=33); i.e., 1-year survival rates of 44.6% vs 23% and 2-year survival rates of 37.2% vs 7.7% after surgery (P〈0.05). However, there was no significant difference in survival rate between the IORT group (n=28) and the non-IORT group (n=29) in 57 resectable patients in stage IV. In unresectable patients, the IORT group (n=31) (P〈0.05) had a higher survival rate than the non-IORT group (n=38) (P〈0.05). The palliative effect of IORT on abdominal or back pain was evaluated in 15 patients who had such symptoms and did not undergo tumor resection. Overall, pain decreased or disappeared in 13 of these patients (87%).

    Notes: Abstract: We report the response of two patients with advanced nonfunctioning islet cell carcinoma of the pancreas with liver metastases treated with a combination of surgi-cal resection and transarterial embolization (TAE), using Lipiodol with epirubicin. After pretreatment evaluation, the two patients were diagnosed with nonfunctioning islet cell carcinoma of the pancreas with liver metastases. Preoperatively, in both patients, TAE was performed through the hepatic arteries, using Lipiodol and sponzel plus epirubicin. Surgical resection of the primary tumor (radical distal pancreatectomy and pancreaticoduodenectomy) was performed. After surgical resection and evaluation of the malignant histopathological features of the neoplasms, chemotherapy, which included oral 5-fluorouracil (FU), and transarterial infusion therapy, using Lipiodol with epirubicin, was administered to the patients. Follow-up evaluation of the two patients by computerized tomography (CT) scan showed a reduction in the size of the metastatic hepatic masses after several chemoembolizations through the hepatic arteries. This combined treatment modality may be an effective therapeutic strategy for improved management of patients with advanced nonfunctioning islet cell carcinoma of the pancreas with liver metastases.

    Notes: Abstract A 54-year-old male was admitted to our hospital for treatment of a liver tumor which was pointed out by screening ultrasonography. Computed tomography revealed a tumor in the right lobe of the liver measuring 7 cm in diameter. Angiography revealed a hypervascular tumor. A hepatocellular carcinoma or hypervascular metastatic tumor was suspected. A right hepatic lobectomy was performed. The patient had previously undergone operations for a hemangiopericytomatous meningioma in the occipital fossa in 1972 and 1977. The histological findings of the liver tumor were identical to those for hemangiopericytomatous meningioma, so the etiology was considered to be liver metastasis from the previous meningioma. Nineteen cases of extracranial metastasis of hemangiopericytomatous meningioma have been reported in the literature, but hepatic resections of this metastasizing tumor have been very rare.

    Notes: Abstract Affinity microspheres were prepared by immobilizing human γ-globulin (HγGb) onto carboxylated poly (styrene/acrylamide) latex particles [P(St/AAm)-H; average diameter 0.33 μm], which were prepared by emulsifier-free emulsion polymerization. HγGB was covalently immobilized onto the latex particles with high efficiency by the carbodiimide method. A fusion protein (ZZB1B2) of immunoglobulin G and albumin-binding domains (ZZ and B1B2, respectively) was expressed intracellularly and extracellularly in Escherichia coli and was purified by the affinity microspheres. In poly (ethylene glycol) (PEG)/potassium phosphate aqueous two-phase system, the affinity microspheres were partitioned into the PEG-rich top phase, while cells and cell debris of E. coli were displaced into the salt-rich bottom phase. Therefore, ZZB1B2 was directly purified from cell disintegrate or culture broth by combining the affinity microspheres with the aqueous two-phase partitioning, and its purity was almost the same as that purified by conventional affinity chromatography. Therefore, by this purification method, the primary purification process and the subsequent high resolution purification process are combined, and the number of purification steps can be reduced.
