Zusammenfassung Entsprechend der Symptomatik kann das drohende und das manifeste Kompartmentsyndrom unterschieden werden. Die kausale Therapie des Kompartmentsyndroms ist die Dekompression der Muskellogen, die entsprechend der Lokalisation in einer speziellen Technik zu erfolgen hat (ausgedehnte Hautfascienincisionen, Excision nekrotischer Muskulatur, Stabilisierung der Fraktur, sekundärer Wundverschluß). Aus den Jahren 1978–1981 wurden 103 KS analysiert. 36 Patienten entwickelten Spätfolgen. Hinsichtlich der Folgeschäden, der Amputations- und Infektrate ließ sich eine eindeutige Abhängigkeit vom Zeitpunkt der Dekompression nachweisen.

Summary Differentiation can be made between the imminent and the manifest compartmental syndrome. The only causal therapy is the decompression of the compartment (excision of necrotic muscle, long incision of skin and fascia). Skin closure is not permitted because of postoperative swelling which can produce a ,,rebound" compartmental syndrome. From 1978 to 1981, 103 compartmental syndromes were analyzed of which 36 patients developed late sequelae, i.e. functional loss, amputation and infection.
