    Notes: Abstract 44 species of the genus Clostridium were examined with regard to their ability to grow on citrate. In addition to Clostridium sphenoides, a known citrate utilizer, the following species were found to utilize citrate: C. sporosphaeroides, C. symbiosum, C. rectum, C. indolis, C. subterminale and C. sporogenes. The major products formed from citrate were acetate, ethanol and carbon dioxide (not measured). Minor products were butyrate, butanol, acetone and acetoin. The enzyme citrate lyase was detectable in cell extracts of C. sporosphaeroides and C. symbiosum using an optical assay. Evidence for the presence of this enzyme in the other species was obtained in immunological experiments and in experiments with [1,5-14C]citrate.

    Description / Table of Contents: Zusammenfassung In einer retrospektiven Analyse wurden Patienten, bei welchen wegen eines Rectumcarcinoms eine anteriore Rectumresektion oder eine abdomino-perineale Rectumexstirpation vorgenommen wurde, erfaßt. Dabei wurde für jedes Operationsverfahren die Rate der Operationsletalität, der postoperativen Komplikationen, der Wiedererkrankung (loco-regionäre Rezidive, Fernmetastasen) und der Mortalität in Korrelation zu der Tumorlokalisation, dem Tumorstaging und dem Tumorgrading gesetzt. Gleichzeitig wurde unter diesen Aspekten ein Vergleich zwischen diesen beiden Operationsverfahren bei Carcinomen des mittleren Rectumdrittels (7–11 cm) vorgenommen. Es zeigte sich, daß im Gesamtkrankengut wie auch bei den Patienten mit einem Carcinom im mittleren Rectumdrittel die Rate an Wiedererkrankungen und postoperativen Komplikationen bei den anterioren Rectumresektionen niedriger war als bei den abdomino-perinealen Rectumexstirpationen, was jedoch auf die unterschiedliche Verteilung der Tumorstadien zurückgeführt werden muß.
    Notes: Summary Eighty-seven patients with an abdomino-perineal extirpation of the rectum and 75 patients with an anterior resection of the rectum could be analysed retrospectively. The mean observation period was 36 months for anterior resection and 38 months for abdomino-perineal extirpation. The rate of recurrences, mortality, and postoperative complications of each surgical method was correlated to tumor-localization, -staging, and -grading. The poor prognosis of patients with abdomino-perineal extirpation was mainly due to relatively more cases in advanced tumor stage. In rectal cancer above 7 cm anterior resection should be performed.

Bei 22 von 26 Patienten mit Rezidiv, Fernmetastasen bzw. Zweitcarcinom war der CEA-Test frühester diagnostischer Hinweis, im Mittel 5,5 Monate vor Sicherung der Diagnose. Bei 8 von 10 Second Look Operationen ausschließlich auf Grund des auffälligen CEA-Verlaufes wurden ein Rezidiv bzw. Fernmetastasen intraabdominell gefunden. Bei 16 Patienten erfolgte eine operative Sekundärtherapie, 9mal potentiell kurativ.
    Notes: Summary Between 1978 and 1980 a follow-up study was performed with 155 patients after a curative operation for colorectal cancer. In 26 cases recurrent disease or a second colorectal cancer was diagnosed. In 22 cases the CEA test was the first sign of tumor progression in the mean 5.5 months before clinical diagnosis. In 8 of 10 second-look operations, intraabdominal tumor relapse was found. Of the 16 patients operated on a second time, 9 are potentially curable.

    Notes: Abstract In a prospective controlled study we analysed the classic nail, a new intramedullary implant for the fixation of peritrochanteric fractures. By means of lateral bending of 4°, unreamed implantation is generally possible. Our collective of 85 patients was characterized by elderly mean age (82.3 years), a predominance of female patients (4.3 : 1), a minor trauma aetiology without relevant additional injuries, and a high rate of concomitant disease (92%). Only 31A-type femur fractures were treated, with a low specific complication rate of 13%, whereby operative revision was necessary in only two patients (3%). A good anatomic reconstruction and full weight-bearing with the osteosynthesis was achieved in the majority of patients. No secondary shaft fractures and only one case of cutting out was observed. The mortality (30 days: 18%, 6 months: 25%) as well as the high rate of unspecific complications (20%) were caused by the patients’ multiple morbidity. Deficits in the Merle d’Aubigne score at follow-up after 6 months can be interpreted within the same context. Nevertheless, 85% could return to their former social environment and only 15% became dependent on a nursing institution in connection with the fracture treatment.

I–IV, TNM-Klassifikation) wurden auf die Konzentration von Ca 19-9 (pos. ≧ 25 U/ml) und CEA (pos. ≧ 5 ng/ml) hin un tersucht. 55 Patienten mit gutartigen Erkrankungen dienten als Kontrollen. 27/130 Patienten hatten erhöhte CEA und nur 20/130 Patienten pathologisch erhöhte Ca 19-9 Werte. Wertet man aber die Kombination beider Marker - Ca 19-9 und/oder CEA pathologisch erhöht - als positiven Befund, so sind 38/130 Patienten tumormarker-positiv. Bei der Tumornachsorge ergaben sich bei 16/18 Patienten mit Rezidiv oder Fernmetastase eines Magen Ca. pathologische Markerbefunde (7 Patienten nur CEA pos., 4 Patienten nur Ca 19-9 pos. und 5 Patienten CEA und Ca 19-9 pos.).
    Notes: Summary The preoperative sera of 130 gastric cancer patients (stages I–IV, TNM classification) were assayed for Ca 19-9 (positive ≧ 25 U/ml) and CEA (positive ≧ 5 ng/ml); 55 patients with benign diseases served as controls. Of the 130 patients, 27 exhibited elevated CEA values, and only 20/130 had pathologically elevated CA 19-9 values. In contrast, the combination of both tests increased the sensitivity (38/130 patients positive) if a positive result means that when either or both markers are positive, the result is positive. In the follow-up situation, 16/18 patients with recurrent or metastatic gastric cancer were marker positive (7 patients only CEA pos., 4 patients only Ca 19-9 pos., and 5 patients CEA and Ca 19-9 pos.).
