    Description / Table of Contents: Zusammenfassung 24 Std nach i.v. Injektion von 100 mg/kg eines Pentosanschwefelsäureesters (SP 54) sind in der Nierenrinde von Ratten schwere degenerative Veränderungen der proximalen Tubuluszellen zu beobachten. Das Tubulusepithel ist flachkubisch umgewandelt, das Tubuluslumen ist angefüllt mit ausgestoßenen Mitochondrien und anderen Zelltrümmern. Am stärksten betroffen ist die Pars contorta des proximalen Tubulus. Weniger stark veränderte Tubuluszellen sind angefüllt mit Cytosomen, die eine spezifische Feinstruktur haben oder gehäuft gegenüber Kontrollen Cytoplasmaprotrusionen in das Tubuluslumen aufweisen. Die akute Tubulusdegeneration ist ein spezifischer Effekt von SP 54. Heparin bewirkt nur eine leichte Schwellung des Tubulusepithels, die häufiger auftritt als bei Kontrolltieren.
    Notes: Summary 24 hrs after the intravenous injection of 100 mg/kg of a Pentosansulfuricacidester (SP 54) severe degenerative alterations are observed in the proximal tubular cells in rat kidney cortex. The tubular epithelium has changed to a single layer of elongated cubic cells. The tubular lumen is filled with cellular debris and extruded mitochondria. The greatest change is seen in the pars contorta of the proximal tubule. Less degenerated tubular cells have a great number of cytosomes with a specific substructure or have cytoplasmatic protrusions into the tubular lumen. This acute tubular degeneration is a specific event after the administration of SP 54 and is not seen after an equimolar dosis of Heparin. Heparin induces only a slight swelling of the tubular epithelium, more frequent seen as in the control animals.

    Notes: Summary Deposition of complement factors, immunoglobulins and infiltrating cells was evaluated by immunohistochemical staining in 30 temporal artery biopsy specimens from patients suffering from temporal arteritis and/or polymyalgia rheumatica and in controls. In the temporal arteritis group infiltrating cells, classic complement, alternative complement and lytic complex activation were detected. In specimens from patients suffering from only polymyalgia rheumatica there was unexpected evidence of classic complement and lytic complex activation. We conclude that immuno-histochemistry provides support for the concept of temporal arteritis and polymyalgia being based on the same pathological process.

    Notes: Abstract Urinary excretion of glomerular basement membrane (GBM)-related peptides was analysed in 72 patients with a variety of renal diseases by immunoblotting using polyclonal antibodies against either collagenase or pepsin digests of human GBM. The specificity of the antibodies was verified by elution of antibodies bound to urinary GBM-related peptides on nitrocellulose blots and demonstration of reactivity of the eluted antibodies with the respective GBM digests. Furthermore, six mice immunized with urinary GBM-related peptides all developed focal linear deposits of mouse IgG along their GBM, linear and mesangial deposits of C3 in the glomeruli and serum antibodies reactive with human GBM. Monoclonal antibodies against urinary GBM-related peptides of one of the mice reacted with different peptides of the non-collagenous and collagenous domains of type IV collagen, the major structural protein of GBM. In the majority of the 75 patients' urines tested, excretion of GBM-related peptides with molecular weights of 33, 50, 80 and 150 kilodaltons (kD) was detectable. Patients with a diminished glomerular filtration rate (GFR) demonstrated excretion of the 33 kD peptide more frequently (91%) and never of the 80 kD peptide as compared with patients with normal GFR (33 kD [42%] 80 kD [87%]). The pattern of urinary GBM-related peptides was not specific for the underlying renal disease as in Alport's syndrome.

    Description / Table of Contents: Summary The late results of 63 operative treated cerebral palsies have been examined. In correcting the equinus deformity gastrocnemius recession without denervation of the soleus muscle has been successful in mild cases. If spasticity was extreme heel-cord-lengthening was added. The results have been analysed and shown to be good in the majority of cases. Investigations on the cause of failures are added. Futheron the results of adductor-tenotomies and different operations on the upper and lower extremity are analysed. The indication for operation of cerebral palsy-patients is discussed in detail.
    Abstract: Résumé Les résultats finaux de 63 patients spastiques (àcause d'une encéphalopathie prénatale ou infantile) traités opérativement sont exposés. Dans les cas moin graves on trouvait de bons résultats par le détachement des muscles gastrocnémieux (sans dénervation du muscle soleus). S'il s'agit d'un spasticisme très fort on faisait le rallongement du tendon d'Achille en plus. Les résultats fonctionnels sont en général bons: les mauvaises succès étaient analysés. En suite il y a des recherches d'autres opérations (p. ex.: la ténotomie des muscles adductores). L'indication de l'opération chez les spastiques est discutée en détail.
    Notes: Zusammenfassung Von 125 seit 1954 operierten Patienten mit infantilen Cerebralparesen konnten 63 jetzt nachuntersucht werden. Bei diesen waren insgesamt 141 Eingriffe vorgenommen worden. Durch die Nachuntersuchung sollten Maßstäbe für eine noch genauere Indikation zur Operation ermittelt werden. Die Ergebnisse zeigen, daß die Gastrocnemiuskopfablösung nach Silfverskiöld, die wir ohne Nervenresektion durchführen, gute Ergebnisse in leichten bis mittelschweren Fällen bringt. Im Laufe des Wachstums traten in einzelnen Fällen aber Rezidive auf. Eine Rekurvation des Kniegelenkes als Operationsfolge konnte nur ganz vereinzelt beobachtet werden. In schwereren Fällen wurde zusätzlich eine dosierte Achillotenotomie durchgeführt. Damit wurden zwar in funktioneller Hinsicht befriedigende bis gute Ergebnisse erzielt, in einem Teil der Fälle konnte der Zehenballenstand aber entweder unvollkommen oder gar nicht durchgeführt werden. Bei 33 Patienten war das Ergebnis gut und sehr gut, bei 7 befriedigend, bei 5 gering und bei 9 ungebessert. Als Ursache von Rezidiven konnte eine ungenügende Kraft der Antagonisten ermittelt werden, der zeitliche Abstand zwischen Operation und Nachuntersuchung war ebenfalls von Bedeutung. Kniebeugekontrakturen mäßigen Grades besserten sich nach Gastrocnemiuskopfablösung häufig, nur in schwereren Fällen mußten Verpflanzungen oder Verlängerungen der Kniebeuger durchgeführt werden. Die Erfolge der subcutanen Adductorentenotomie waren durchschnittlich gering. Es wird daher eine offene Adductorentenotomie mit Resektion des sehnigen Ansatzes in schwereren Fällen vorgeschlagen. Zur Beseitigung einer spastischen Innenrotation der Oberschenkel sollte die Tenotomie des Ansatzes des M. add. magnus durchgeführt werden. Bei Spastik der Hand- und Fingerbeuger wurde eine Ablösung der Muskeln an der Ulna mit Verlagerung des N. ulnaris durchgeführt. Sie erwies sich aber in schwereren Fällen als ungenügend, so daß Sehnenverlängerungen vorgenommen werden mußten. Abschließend wurde die Indikation zur Operation erörtert. Die Entspannung von Muskeln durch Ablösung und Tenotomie wird der Denervation vorgezogen. Besonders entscheidend für den Erfolg der Operation ist die genaue Untersuchung des Spastizitätsgrades und des Funktionszustandes aller einzelnen Muskeln vor der Operation.

    Notes: Abstract A rapid and easy method for isolating IgM from serum specimens in order to detect specific antibodies againstTreponema pallidum and rubella virus by routine serologic procedures is described. Serum IgM was isolated by immunoaffinity chromatography using anti-human IgM antibodies covalently bound to controlled-pore glass beads in a microcolumn. The final concentration of the IgM in the samples tested amounted to at least 16 % (average 32 %) of the original concentration (corresponding to a serum dilution of 1∶〈8). IgG contamination did not exceed 0.38 % of the original serum concentration. The capacity of the column was stable for at least 50 absorption/ elution cycles. The new technique enables rapid and reliable detection of specific IgM by the rubella hemagglutination inhibition andTreponema pallidum hemagglutination tests.

    Notes: Abstract Complete absence of Factor I (C3b inactivator) was found in the serum of a 3-year-old boy with recurrent polytope bacterial infections. Analysis of the complement (C) components of the patient's serum showed that while serum levels of the earlier C components (C1q, C1r, C1s, C4, and C2) were within the normal range, levels of C3, C5, Factor B, and properdin were decreased significantly. Factor H (β1H) was also decreased and, by crossed immunoelectrophoresis, migrated to the alpha region (Hα). C-dependent biological functions such as chemotactic activity, opsonic factors, immune adherence inhibition, and intracellular killing of bacteria were defective. The patient has been treated with fresh frozen plasma (10 ml/kg body weight) every 4 weeks for 2 years. Most of the C-derived biological functions improved, and Factor H regained beta electrophoretic mobility (Hβ) following plasma infusion. During the period of treatment, the child has been free of bacterial infections. Mild anaphylactic reactions to the plasma occurrred on two occasions; the other 22 infusions were well tolerated.
