Summary On the grounds of investigating 630 kidneys (590 kidney biopsies) of patients, who suffered from different forms of glomerulonephritis, an attempt is made to reclassify the glomerulonephritis regarding the stages in the course of development. Beginning with the normal structure of the glomerula and emphasizing the dominating morphological changes the following acute (potentially completely curable) fr oms of glomerulonephritis are distinguished: 1. the acute membranous glomerulonephritis, 2. the acute proliferative glomerulonephritis, 3. the acute exsudative glomerulonephritis, 4. the acute haemorrhagic glomerulonephritis, 5. the acute focal glomerulonephritis. From these types, the relatively seldom peracute glomerulonephritis is separated because of its usually primarily unfavourable course. The “subacute, extracapillary glomerulonephritis” ofVolhard andFahr belong to the group of peracute glomerulonephritis. Cases of glomerulonephritis not healing in the acute stage, are, during the course of futher development, at first classified as postacute (in the sense of latent chronic) and with the onset of kidneystrinkage processes, as chronic. Under the different forms of glomerulonephritis, the group of membranous glomerulonephritis (acute membranous glomerulonephritis, postacute and chronic perimembranous glomerulonephritis) is taken as a disease unit in contradistinction to the other forms of glomerulonephritis. In this regard the specially frequent occürence of nephrotic syndrom with the membranous glomerulonephritis ist pointed out. The seldom, lobulary glomerulonephritis ist classified as a special type of the proliferative glomerulonephritis. Discussing the focal glomerulonephritis it is noted that this disease is notrarely accompanied by nephrotic syndrom. Lastly it is emphasized that the glomerulonephritis occurs more often in males than in females. In our cases the relation of male to female was 1,94: 1.

Zusammenfassung Aufgrund der Untersuchung von 630 Nieren (590 Nierenbiopsien) von Patienten, die an verschiedenen Formen der Glomerulonephritis erkrankt waren, wird der Versuch unternommen, die Glomerulonephritiden unter Berücksichtigung ihrer Verlaufsstadien neu zu ordnen. Ausgehend von der normalen Struktur der Glomerula werden unter Herausstellung der dominierenden morphologischen Veränderungen folgende akute (potentiell völlig heilbare) Glomerulonephritisformen unterschieden: 1. die akute membranöse Glomerulonephritis, 2. die akute proliferative Glomerulonephritis, 3. die akute exsudative Glomerulonephritis, 4. die akute haemorrhagische Glomerulonephritis, 5. die akute Herdnephritis. Von diesen Formen werden die relativ seltenen perakuten Glomerulonephritiden wegen ihres gewöhnlich von vornherein ungünstigen Verlaufes abgegrenzt. Zu den perakuten Glomerulonephritiden gehört die „subakute, extracapilläre Glomerulonephritis“ vonVolhard undFahr. Nicht im akuten Stadium abgeheilte Glomerulonephritiden werden während ihres weiteren klinischen Verlaufs zunächst als postakut (im Sinne von latent chronisch) und mit dem Auftreten von Nierenschrumpfungsprozessen als chronisch bezeichnet. Unter den verschiedenen Glomerulonephritisformen wird die Gruppe der membranösen Glomerulonephritiden (akute membranöse Glomerulonephritis, postakute und chronische perimembranöse Glomerulonephritis) als Krankheitseinheit den übrigen Glomerulonephritisformen gegenüber gestellt. Es wird dabei auf das besonders häufige Vorkommen des nephrotischen Syndroms bei den membranösen Glomerulonephritiden hingewiesen. Die seltene, lobuläre Glomerulonephritis wird als Sonderform der proliferativen Glomerulonephritis bezeichnet. Bei der Besprechung der Herdnephritiden wird darauf hingewiesen, daß dieses Krankheitsbild nicht selten mit einem nephrotischen Syndrom einhergeht. Schließlich wird betont, daß die Glomerulonephritis beim Mann häufiger vorkommt als bei der Frau. Im eigenen Untersuchungsgut betrug das Verhältnis männlich zu weiblich 1,94:1.
