    Notes: Abstract Injection sclerotherapy is one of the most effective treatments for esophageal varices. To investigate the fate of esophageal varices embolized with a sclerosant (ethanolamine oleate), we have examined at autopsy the esophageal wall of 14 patients with esophageal varices. When the sclerosant was injected into the varices, at first thrombi formed. After two weeks, granulation tissue appeared and the thrombi were gradually replaced. The granulation tissues were organized slowly after 3 months and in the organized granulation tissue, microscopic recanalization of vessels were seen in 8 out of 9 cases. There was no recurrence of the original varices andde novo varices occurred in one patient. These observations indicate that the varices embolized with ethanolamine oleate become organized in three months after the treatment, and accordingly the esophageal varices are cured. It became also clear that recurrence of the original varices did not occur after the organization.

    Notes: Abstract We report herein the case of an 81-year-old woman found to have small intestinal carcinoma producing carbohydrate antigen (CA)19-9, in whom recurrence on the abdominal wall was strongly suspected 4 months after resection. She presented to our hospital with acute abdominal pain with severe anemia. Marked serum elevation of CA19-9 to 164.8 U/ml suggested a progression to malignancy. A fluorography using an ileus tube revealed an abnormal mucosal pattern. An exploratory laparotomy showed an incomplete annular constrictive Borrmann type 2 tumor, located approximately 190 cm from Treitz's ligament, without any signs of peritoneal or hepatic metastases. Histological examination confirmed a diagnosis of papillotubular adenocarcinoma without metastases of the regional lymph nodes. CA19-9 antigenicity was detected in the cytoplasm and on the surface of the cancer cells, using the monoclonal CA19-9 antibody, NS19-9. In this report, we demonstrate the CA19-9 productivity and distribution of the cancer tissues in relation to their prognosis.

    Notes: Abstract Adenocarcinoma of the small intestine is uncommon. Due to this paucity and the lack of specificity of symptoms, patients are usually seen late in the course of their illness, when curative therapy, mainly represented by extensive surgical resection, is unlikely. The authors report a case of primary well-differentiated tubular adenocarcinoma (T4N0M0) arising in the duodenal limb of a reconstructed Billroth I gastroduodenostomy, 9 years after a distal gastrectomy for signet-ring cell carcinoma of the stomach (T4N0M0). Evidence for excluding the possibility of a recurrence of the primary gastric cancer was based on the different histologic pattern, the long disease-free interval, and other features of the second neoplasm. Relatively early diagnosis of the neoplasm, followed by curative surgical therapy was made possible by the early onset of the obstructive symptoms and the favorable anatomical location of the tumor.

    Notes: Abstract A case of well-differentiated adenocarcinoma (Borrmann type 3) of the stomach in a 76-year-old man associated with the typical skin manifestations of acanthosis nigricans and with multiple protruding lesions showing epithelial hyperplasia of the esophagus is reported. The advanced tumor was located in the cardiac region of the stomach, and measured approximately 8cm in diameter, with partial invasion to the esophagus. The associated cutaneous lesions were characterized by hyperpigmentation and by protruding verrucous papules on the torso, head, face, neck, upper extremities, perineum, and inguinal region. Histologically, the protruding skin lesions showed keratinocytes proliferation throughout the epidermis, resulting in diffhyperkeratosis, papillomatosis, and acanthosis of the skin. Immunohistological analysis showed coexpression of transforming growth factor alpha (TGF-a) and epidermal growth factor (EGF) receptors in the tumor from the stomach. It is reasonable to conclude from this evidence that gastric carcinoma cells secrete TGF α in an autocrine for auto-stimulation. EGF receptor expression was also noted on the papillomatous hyperplasia of the cutaneous lesion. Serum level of TGF α, determined by an enzyme-linked immunosorbent assay, was high (144pg/ml; normal, 22.0 ±16pg/ml (Mean±SD)). Serum TGF α abruptly decreased to 49pg/ml on day 7 after the total gastrectomy, and then gradually increased to 77pg/ml within 28 days. Amelioration of the cutaneous lesions and the protruding lesions in the esophagus was observed after surgical resection of the gastric carcinoma. This suggests that the TGF α stimulates the proliferation of keratinocytes involved with EGF receptor. Large amounts of circulating TGF α in the blood over a long period released by the primary tumor seem to act as an endocrine-like mechanism causing epidermal and esophageal epithelial cells to proliferate. There is a possible link in the pathogenesis of the acanthosis nigricans as a cutaneous paraneoplastic syndrome, and epithelial hyperplasia of the esophagus.

    Notes: Thickness effect is studied experimentally. At free surface of the specimen, shear lipfracture pattern appears, though dimple fracture pattern is observed inside of the specimen. The areaof shear-lip fracture changes due to the change of the specimen thickness. In this study, experimentalstudy is conducted by changing specimen thicknesses. Fracture surfaces are precisely observed usingSEM, and dimple patterns on them are observed. At the free surface, very narrow no-void area isobserved. It is also found that many voids are nucleated in shear-lip fracture area. FEM simulation iscarried out using Gurson’s constitutive equation. It is found that shear-lip type fracture is simulatednear free surface area by this method. The results show similar tendency with the experimentalobservation

    Notes: Biocompatible and water-soluble fibers (sodium carboxymethyl cellulose (CMC)) werefabricated via a wet-spinning method. The CMC fibers/ polymethyl methacrylate (PMMA)mixtures and CMC fibers/Poly (L-lactide-co-glycolide-co- -caprolactone) (PLGC) mixtures wereprepared by a heat-kneading method. For CMC fibers/PMMA, after removal of the CMC fibersfrom the mixtures, the interconnected porous scaffolds with porosity from 27.79 % to 60.98 %(volume percent) were obtained. For CMC fibers/PLGC, the interconnected porous scaffolds withporosity 38.55 % and 76.83 % (volume percent) were prepared. The solid CMC fibers/PLGCmixtures had the higher ultimate tensile strength and Young, s modulus than those of porousPLGC scaffolds

    Notes: The bone-like self-organized hydroxyapatite/collagen (HAp/Col) nanocomposite spongewas prepared from HAp/Col nanocomposite fibers. We analyzed osteogenic activity of humanosteoblastic MG63 cells in the HAp/Col sponge under pressure/perfusion culture. Collagen (Col)sponge was used as a control. The MG63 cells were attached well and showed good proliferation inthe HAp/Col sponge. The total DNA content in the HAp/Col sponge was approximately 1.8 timesgreater than that in the Col sponge at 21 days. The MG63 cells showed good osteogenic geneexpression in the HAp/Col sponge by reverse transcription-polymerase chain reaction analysis.These result suggested that HAp/Col sponge can be useful for bone tissue engineering scaffoldmaterials

    Notes: In the virtual crack closure method (VCCM), the energy release rate is computed based onthe results of finite element calculation, and the stress intensity factor (SIF) is computed from theenergy release rate. In this paper, the stress intensity factor of mixed-mode surface cracks under threepoint bending is studied by using the three dimensional modified virtual crack closure method(MVCCM). The modified virtual crack closure method is required to open one element face areawhose shape is arbitrary and finite element widths are unequal across the crack front. The effect of thedistance between the location of load and crack face, crack shape and crack depth to the stressintensity factor is also discussed, along with practical results and conclusions

    Notes: Fatigue crack growth under mixed mode loading conditions is simulated using S-FEM. Byusing S-FEM technique, only local mesh should be re-meshed and it becomes easy to simulate crackgrowth. By combining with auto-meshing technique, local mesh is re-meshed automatically, andcurved crack path is modeled easily. Plural fatigue crack problem is solved by this technique. Fortwo parallel crack problem, criteria of crack coalescence are proposed. By simulating this problem byS-FEM, it is verified these criteria are conservative ones

    Notes: The Mode I and Mode II dynamic fracture toughness (DFT) values, KId and KIId for highstrength steels 40Cr and 30CrMnSiNi2A were evaluated using a combined experimental-numericalmethod. The tests were performed on three point bend (3PB) and shear specimens with Hopkinsonpressure bar. The time of crack initiation was determined by a strain gauge. With the assistance of3-D transient finite element analysis, the temporal evolution of the dynamic stress intensity factorunder different loading rates was obtained, and the DFT was determined by the fracture initiationtime. In Mode I tests, a brittle transgranular fracture was found in 40Cr, while evidence for ductilefracture was observed on the failure surface of 30CrMnSiNi2A. In Mode II tests, both tensile cracksand adiabatic shear bands were found for the two steels. The effect of ligament size on fracturetoughness was discussed. The correlations of KId and KIId with loading rate were also investigated.The micromechanisms of loading rate effect on the DFT were discussed for both modes
