    Notes: Fine grained rodingite-like rocks containing epidote, clinozoisite, garnet, chlorite, phengite and titanite occur within antigorite serpentinite boudins from the high-pressure metamorphic Maksyutovo Complex in the Southern Urals. Pseudomorphs after lawsonite, resorption of garnet by chlorite and phengite and stoichiometry suggest the reaction lawsonite + garnet + K-bearing fluid → clinozoisite + chlorite + phengite, and define a relic assemblage of lawsonite + garnet + chlorite + titanite ± epidote as well as a later post-lawsonite assemblage of clinozoisite + phengite + chlorite + titanite. The reaction lawsonite + titanite → clinozoisite + rutile + pyrophyllite + H2O delimits the maximum stability of former lawsonite + titanite to pressures 〉13 kbar. P–T conditions of 18–21 kbar/520–540 °C result, if the average chlorite, Mg-rich garnet rim and average epidote compositions are used as equilibrium compositions of the former lawsonite assemblage. These estimates indicate a similar depth of formation but lower temperatures to those recorded in nearby eclogites. The metamorphic conditions of the lawsonite assemblage are considerably higher than previously suggested and, together with published structural data, support a model in which a normal fault within the Maksyutovo complex acted as the major transport plane of eclogite exhumation.The maximum Si content of phengite and minimum Fe content in clinozoisite constrain the metamorphic conditions of the later pseudomorph assemblage to be 〉4.5 kbar and 〈440 °C. Rb–Sr isotopic dating of the pseudomorph assemblage results in a formation age of 339 ± 6 and 338 ± 5 Ma, respectively. These results support the recent exhumation models for this complex.

    Notes: Epitaxial CeO2 buffer layers and YBa2Cu3O7−δ thin films have been grown in situ on (11¯02) sapphire by electron beam evaporation. Buffer layers of only 20 nm thickness inhibit interdiffusion between YBa2Cu3O7−δ and Al2O3 as determined by depth profiling using x-ray photoelectron spectroscopy. The layers show smooth surfaces and narrow interfaces. High lattice perfection of the CeO2 layer has been shown by x-ray diffraction. Laue oscillations up to ninth order have been observed in thin CeO2 buffer layers on sapphire. We found only one epitaxial orientation with YBa2Cu3O7−δ (001) (parallel) CeO2 (001) (parallel) Al2O3 (11¯02) and YBa2Cu3O7−δ [110] (parallel) CeO2 [100] (parallel) Al2O3 [112¯0]. YBa2Cu3O7−δ films grown on these buffer layers reveal Tc=88±0.5 K, ρ(300 K)=380 μΩ cm, and jc(77 K, 0 T)=1.3×106 A/cm2.

    Notes: We report on the in situ preparation and characterization of thin YBa2Cu3O7−δ films on various substrates. The films were grown by electron-beam coevaporation of the pure metals Y, Ba, and Cu. A pressure stage is used to obtain high partial O2 pressure close to the substrate and low pressure in the main vacuum system. In situ resistance measurements were performed to investigate growth dynamics during film formation, and temperature dependence during the cool down from T≈1000 K to T≤Tc. The films were analyzed by R(T), ac susceptibility, x-ray diffraction, and critical transport current measurements.

    Notes: We report on effects of composition onto superconducting and structural properties of Y-Ba-Cu-O thin films, prepared by a carrier-gas free metalorganic chemical vapor deposition technique. Thin films with critical temperatures Tc≥90 K, transition widths ΔTc≤1 K, and critical current densities jc (T=77 K, B=0 T)≥106 A/cm2 can be prepared by this method. Here we report on a strong dependence of the Tc on the ratio of Ba/Cu. For Ba/Cu〈2 the surfaces show CuO precipitates, increasing in quantity and size with increasing concentration of Cu. An excess of Y leads to oriented inclusions of cubic Y2O3. A deficit of Y and Cu causes an expansion of the c-lattice parameter. To investigate the influence of Y2O3 and CuO precipitates on the pinning potential, resistive transitions in magnetic fields up to 12 T were measured and interpreted by a thermal activated flux-creep model. These measurements indicate the inefficiency of both, inclusions and precipitates, on the enhancement of the pinning potential.

    Notes: A new carrier gas-free metalorganic chemical vapor deposition technique was developed yielding a high deposition rate. The β-dicetonate precursor compounds were synthesized with reproducible water content which is of great importance for their vapor pressure. With the technique described above c-axis oriented YBa2Cu3O7−δ thin films were prepared with Tc(R=0)=91.8 K and jc(77 K)=1.3×106 A/cm2 on (100)SrTiO3 substrates and Tc(R=0)=91.1 K and jc(77 K)=3×105 A/cm2 on (100)MgO substrates at B=0 T. The influence of the metal composition on the formation of precipitation was studied by SEM investigations.

    Description / Table of Contents: Zusammenfassung Unterschiedliche elektrokardiographische Manifestationen einer dualen AV-Knoten-Leitungsphysiologie sind beschrieben. Ein besonders selten auftretender Subtyp stellt hier die doppelte ventrikuläre Erregung dar. Wir berichten über eine 53-jährige Patientin mit über sieben Jahren bestehenden typischen paroxysmalen regelmäßigen Tachykardien. In den vergangenen sechs Monaten kam es zu einer Änderung der vorbestehenden Symptomatologie der Herzrhythmusstörungen mit einem Wechsel zu länger anhaltenden und irregulär empfundenen Rhythmusstörungen. Medikamentöse Behandlungsversuche mit Antiarrhythmika der Klasse I waren nicht erfolgreich. Ein Therapieversuch mit Beta-Blockern verstärkte die subjektiven Beschwerden. Nach erneuter Befundung des EKG der Patientin wurde vom zuweisenden Krankenhaus die Diagnose einer dualen AV-nodalen Leitungsphysiologie mit doppelter Ventrikelerregung gestellt und die Patientin unserer Klinik zur invasiven elektrophysiologischen Untersuchung und zur Katheterablation des langsamen Leitungsweges des dualen AV-Knotens zugewiesen. Die invasive elektrophysiologische Untersuchung bestätigte die Diagnose eines dualen AV-Knotens mit doppelter Ventrikelerregung. Die Hochfrequenz-Katheterablation der langsamen Leitungsbahn erreichte die vollständige Unterbrechung der doppelten Ventrikelerregung. Das erfolgreiche Ablationsergebnis konnte durch Langzeit-EKG-Registrierung und Belastungsergometrie weiterführend bestätigt werden. Während einer dreimonatigen Nachbeobachtung blieb die Patientin vollständig asymptomatisch und es kam zu keiner Wiedererholung der dualen AV-Knoten Leitungsphysiologie im Oberflächen-EKG.
    Notes: Summary A variety of electrocardiographic manifestations of dual AV nodal physiology have been reported. The specific subtype dual ventricular response is considered as a very rare phenomenon. We present the case of a 53 year old lady who suffered from paroxysmal regular tachycardias for more than seven years. In the last 6 months the symptomatology of the cardiac arrhythmia changed to more persistent und irregular rhythm disturbances. Treatment with class Ia antiarrhythmic drugs and beta-blocking agents failed. The latter even seemed to worsen her very disturbing palpitations. After examination of the ECG recordings, the diagnosis of dual AV nodal physiology with double ventricular response was made – the lady was referred to our institution for electrophysiological testing and radiofrequency catheter ablation of the slow pathway. An invasive electrophysiological study reconfirmed the diagnosis of a dual AV nodal conduction pattern with irregular double ventricular response. The radiofrequency catheter ablation of the the slow pathway achieved a complete cessation of the double ventricular response. This satisfactory outcome was confirmed by analysis of a postinterventional 24hour holter recording and an exercise stress test. During a follow-up period of three months, the patient remained free of symptoms and there was no recurrence of dual AV nodal conduction physiology in the surface ECG.

    Description / Table of Contents: Zusammenfassung Inadäquate Therapien implantierbarer Kardioverter-Defibrillatoren infolge supraventrikulärer Tachykardien stellen weiterhin die häufigste Komplikation der ICD-Therapie dar. Die Einführung des Morphologie (MD)-Kriteriums verspricht eine Verbesserung der Spezifität in der Tachyarrhythmieerkennung bei Erhalt der Sensitivität. Ziel dieser multizentrischen Studie war die systematische Analyse von Sensitivität und Spezifität des MD-Kriteriums in kombinierter Anwendung mit den Kriterien Stabilität und Frequenzsprung (Sudden Onset) bei spontanen Episoden ventrikulärer und supraventrikulärer Tachykardien. Bei 259 Patienten wurden nach ICD-Implantation 787 gespeicherte Episoden bei 74 Patienten mit erhältlichem Speicherelektrogramm während einer Nachbeobachtungszeit von 359±214 Tagen analysiert. Bei nominaler Programmierung des MD-Kriteriums mit ≥60% ergaben sich unter Berücksichtigung aller Episoden eine Sensitivität/Spezifität der Detektion von 82,6%/77,2%. Bei getrennter Analyse von Sinustachykardien, Vorhofflimmern und Vorhofflattern betrug die Spezifität 80,6%, 69,6% und 75%. Bei Patienten mit primär adäquater MD-Detektion waren Sensitivität und Spezifität mit 95,8%/91,7% deutlich verbessert. Bei nominaler Programmierung des Sudden Onset-Kriteriums mit 〈100ms und des Stabilitätskriteriums mit 〈50ms ergaben sich für die Kombination MD- und Sudden-Onset-Kriterium bzw. MD- und Stabilitätskriterium eine Sensitivität/Spezifität von 96,2%/52,2% bzw. 94,4%/63,8%. Die Anwendung des MD-Detektionskriteriums lässt in der kombinierten Anwendung mit den herkömmlichen erweiterten Kriterien wie Sudden Onset- und Stabilitätskriterien eine deutlich verbesserte Spezifität der ICD-Therapie erwarten.
    Notes: Summary The high incidence of inappropriate therapies due to supraventricular tachycardia remains a major unsolved problem of the implantable cardioverter defibrillator. A new morphology discrimination (MD) algorithm has been introduced to improve specificity of ICD therapy without loss of sensitivity. It was the aim of this study to systematically analyze sensitivity and specificity of the MD criterion in combination with the enhanced detection criteria sudden onset and rate stability in the detection of ventricular and supraventricular tachycardia. After ICD implantation in 259 patients, 787 detected episodes in 74 patients with available stored electrograms were documented during a follow-up period of 359±214 days. With a nominal programming of the MD algorithm at ≥60%, sensitivity and specificity for all episodes were 82.6%/77.2%. For sinus tachycardia, atrial fibrillation and atrial flutter the specificities were 80.6%, 69.6% and 75%, respectively. In patients with primarily appropriate MD detection, sensitivity and specificity significantly improved to 95.8%/91.7%. Programming the sudden onset criterion with 〈100ms and the stability criterion with 〈50ms, sensitivity and stability of the combined application of the MD algorithm and sudden onset and MD algorithm and stability were 96.2%/52.2% and 94.4%/63.8%, respectively. The MD criterion in combination with other enhanced detection criteria might significantly improve specificity of tachyarryhthmia detection of ICD therapy.

    Description / Table of Contents: Summary The aim of this study is the analysis of electrical failures in transvenous cardioverter/defibrillator (ICD) lead systems with regard to the importance of device implemented diagnostic and measurement functions and the potential role of an automated device-control in the detection of lead failures. Methods: All consecutive ICD patients at our institution were enrolled in this retrospective investigation. The routine follow-up controls consisted in a complete evaluation of all diagnostic and measurement ICD features and additional controls in when spontaneous arrhyhtmia episodes occurred. Results: Two hundred thirty patients, 193 male and 37 female, were enrolled in this study (mean age: 61.5±10.2 years; mean LVEF was 32±9%). During a mean follow-up period of 29.5±18.4 (6–76) months, lead failure occurred in 19 patients (8%). 16 patients were implanted with an ICD, capable of diagnostic and measurements functions. All nonadequate device discharges could be classified as sensing-failure by stored electrograms. Device implemented measurement features revealed clinical important information in 13/16 patients (81%). In 14/16 patients, the lead defect could not be detected during routine follow-up. At the time of documented lead failure the safety of the implanted devices was already lost in 6/16 patients (38%). Conclusions: Device implemented diagnostic and measurement options are of great importance in the early detection of ICD lead failures. The implementation of automated measurements of lead related parameters in connection with a patient alert function may contribute to a further increase in the safety of ICD therapy.
    Notes: Zusammenfassung Das Ziel dieser Studie ist die Analyse elektrischer Dysfunktionen transvenöser Kardioverter/Defibrillator-(ICD-)Elektrodensysteme hinsichtlich der Bedeutung systemintegrierter Diagnose- und Meßfunktionen und der Möglichkeit einer automatisierten Systemkontrolle von ICD-Generatoren für eine sichere und schnelle Erkennung von Elektrodendefekten. Methoden: Alle konsekutiven ICD-Patienten unserer Klinik wurden in die vorliegende, retrospektive Untersuchung einbezogen. Die Verlaufskontrolle umfaßte die regelmäßige, vollständige Systemprüfung der ICD-Systeme und zusätzliche Kontrollen bei spontanen Arrhythmieepisoden. Ergebnisse: 230 konsekutive Patienten, 193 Männer, 37 Frauen, wurden in die Untersuchung einbezogen (mittleres Alter: 61,5±10,2 Jahre; mittlere LVEF 32±9%). Während einer Nachbeobachtungsperiode von 29,5±18,4 Monaten (6–76 Monate) wurden bei 19 Patienten (8%) Defekte der ICD-Elektrodensysteme dokumentiert. 16 Patienten trugen einen ICD-Generator mit systemimplementierten Diagnose- und Meßfunktionen. Alle nichtadäquaten ICD-Entladungen konnten durch die Analyse gespeicherter Elektrogramme als Fehlfunktion klassifiziert werden. Die systemintegrierten Meßfunktionen zeigten bei 13 von 16 Patienten (81%) diagnostisch verwertbare Ergebnisse. Bei 14/16 Patienten konnte der Elektrodendefekt im Rahmen der Nachsorge nicht rechtzeitig erkannt werden. Zum Zeitpunkt der Diagnosestellung war bei 6/16 Patienten (38%) die Therapiesicherheit des implantierten ICD-Systems nicht mehr gewährleistet. Schlußfolgerung: Systemintegrierte Diagnose- und Meßfunktionen sind eine wichtige Option zur Erkennung von ICD-Elektrodendefekten. Die Einführung systemintegrierter, automatisch durchgeführter Kontrollen aller elektrodenbezogenen Meßwerte in Verbindung mit Patientenwarneinrichtung könnte zu einer weiteren Steigerung der Sicherheit der Therapie mit implantierbaren Kardioverter/Defibrillatoren beitragen.

    Notes: Summary Glycoconjugates associated with the basal cell layer of various types of epithelia in the mouse and rat were examined histochemically with a battery of lectinhorseradish peroxidase (HRP) conjugates of differing sugar binding specificities. Basal cells in paraffin sections of composite tissue blocks stained with an isolectin from Griffonia simplicifolia (GSA I-B4) specific for terminal α-galactose residues but failed to react with the other lectins. Basal cells in epithelium lining striated and excretory ducts of salivary and lacrimal glands, tongue, esophagus, trachea, renal calyx, ureter, urinary bladder, urethra, epididymis and vas deferens stained selectively and intensely for content of a glycoconjugate with terminal α-galactose. This galactoconjugate appeared associated with the plasmalemma of basal cells. Basal cells with a galactocalyx formed an intermittent to continuous layer generally increasing in prevalence distally in glandular duct systems. A minor population of pyramido-columnar cells with cytosolic GSA I-B4 reactivity occurred in striated ducts and appeared less numerous in intralobular excretory ducts and more prevalent in extraglandular ducts. In trachea and renal pelvis, the GSA I-B4 positive cell profiles ranged from low cuboidal to tall pyramidal in contour, but the latter appeared not to reach the lumen. In contrast, no GSA I-B4 positive basal cells were seen in any segment of the pancreatic or bile ducts or in the epithelium of the gastrointestinal tract. These findings suggest that the basal cells found in similar sites in different epithelia and possessing in common a unique α-galactoconjugate may function in a manner common to all and not simply in providing progenitor cells for epithelial renewal. The location and distribution of GSA I-B4 reactive basal cells in diverse epithelia suggests that through their α-galactocalyx they serve in maintaining the established composition of luminal fluid perhaps by impeding the transepithelial movement of fluid and ions.
