    Description / Table of Contents: Summary Addition of plasma, heated to 56°, to platelet rich plasma resulted in a significant increase of platelet aggregation. Further increase of aggregation was achieved, when the euglobulin fraction of a heated plasma was used. Lower values were found by using serum fractions. After precipitation of plasma with 2% inulin the sediment showed a strong platelet aggregating effect while the supernatant was free of activity. Similar results could be produced by using zymosan instead of inulin. This aggregating principle was not destroyed by heating to 56°. It is suggested that inulin or zymosan precipitation resulted in a crude purification of a platelet aggregating factor.
    Notes: Zusammenfassung Nach Zugabe eines auf 56° erhitzten Plasmas zu plättchenreichem Plasma nahm die Plättchenaggregation signifikant stärker zu als nach Zugabe eines Nativplasmas. Eine noch stärkere Plättchenaggregation wurde durch die Zugabe der Euglobulinfraktion eines auf 56° erhitzten Plasmas erreicht. Nicht so deutlich war die Aggregationssteigerung der entsprechenden Serumzusätze. Wurde eine Fällung mit 2%iger Inulinsuspension vorgenommen, so nahm die Plättchenaggregation durch das Sediment der Inulinfällung signifikant zu, während dieser Effekt durch Zugabe des Überstandes vermißt wurde. Praktisch gleichsinnige Ergebnisse wurden durch die Zugabe eines durch Zymosan gefällten Plasmasediments erzielt. Erhitzen auf 56° zerstört diesen Faktor nicht. Es wird diskutiert, ob durch die Inulin- bzw. Zymosanfällung ein Plasmafaktor angereichert wird, der die Plättchenaggregation steigert.

    Description / Table of Contents: Zusammenfassung 6 Patienten, 2 Männer und 4 Frauen, im Alter zwischen 15 und 60 Jahren, wurden im Lebercoma Stadium IV und schwersten EEG-Veränderungen mit Hämoperfusion behandelt. Die Ursachen, die zum Coma führten, waren zweimal akute Virushepatitis A, dreimal Hepatitis B und einmal eine Knollenblätterpilz-Intoxikation. Nur die Patientin mit der Pilzintoxikation überlebte. Bei Klinikaufnahme bestanden bei allen Patienten schwerste EEG-Veränderungen des Stadium IV. Sie gingen bei der Hälfte der Patienten, die zu diesem Zeitpunkt allenfalls im Coma Stadium II waren, der klinischen Manifestation des Coma Stadium IV bis zu einem Tag voraus. Bei Therapiebeginn in niedrigeren Coma-Stadien werden bessere Überlebenschancen berichtet. Man müßte also, ausgehend von den bei einem Teil der Patienten dem klinischen Coma vorausgehenden schwer pathologischen EEG-Befunden, einen früheren Therapiebeginn fordern.
    Notes: Summary Six patients (two men and four women) aged between 15 and 60 years in hepatic coma stage IV and with very severe EEG changes were treated with hemoperfusion. The causes leading to coma were acute virus hepatitis A in two cases, hepatitis B in three cases, and Amanita intoxication in one case. Only the patient with Amanita intoxication survived. On admission to the hospital, there were very severe EEG changes of stage IV in all patients. They preceded the clinical manifestation of coma stage IV up to one day in half of the patients who were at most in coma stage II at this time. At the beginning of therapy better survival chances are reported in the lower coma stages. On the basis of the severely pathological EEG findings preceding the clinical coma in a proportion of the patients, an earlier start of therapy should thus be demanded.

    Notes: Summary Hepatitis B core antigen (HBcAg) synthesized in E. coli was used for determination of immunoglobulin M class-specific antibodies against HBcAg. It was found that 98% of cases with acute hepatitis B surface antigen (HBsAg) positive hepatitis type B were anti-HBc immunoglobulin M (IgM) positive. Atypical hepatitis B was detected in 33% of anti-HBc-positive HBsAg-negative cases with acute hepatitis. Anti-HBc IgM was positive for 6 months in acute resolving hepatitis type B, whereas cases resulting in chronic hepatitis B remained anti-HBc IgM-positive for up to 900 days. Chronic HBsAg carriers with severe liver disease had anti-HBc IgM more often than individuals with minor liver damage; 83% of HBsAg-positive liver cirrhoses, 63% of chronic aggressive hepatitis, 50% of HBsAg-positive liver carcinoma, but only 17% of chronic persistent hepatitis or 7% of healthy blood donors were anti-HBc IgM-positive. Determination of anti-HBc IgM is useful in detecting atypical hepatitis B virus infections without HBsAg in serum and, with some restrictions, in discriminating acute and chronic hepatitis type B.

    Notes: Summary The frequency of delta infection was studied in sera of 203 patients with acute hepatitis B, further 461 hepatitis B virus surface antigen-(HBsAg)-positive patients and 117 HBsAg-negative controls by determination of anti-delta by a competitive enzyme immunoassay. Sera have been collected since 1974. None of the sera of acute hepatitis B was anti-delta-positive whereas seven of the HBsAg-positive carriers were anti-delta-positive. Two of the anti-delta-positive patients had chronic hepatitis, four had liver cirrhosis. One of the anti-delta-positive patients with liver cirrhosis died of liver failure. Risk factors included Italian origin and parenteral routes of infection. All sera of 19 relatives of three anti-delta-positive index cases remained anti-delta-negative.

    Description / Table of Contents: Summary A drecrease of platelet ATP under the influence of a heatstable plasma factor is described. This factor can be precipitated by inulin and belongs to the euglobulin fraction. The factor has no relation to the heatstable contact factors of the coagulation system. The process of heating and precipitation represents a purification rather than an activation.
    Notes: Zusammenfassung Es wird über einen hitzestabilen Plasmafaktor berichtet, der einen ATP-Verlust von Blutplättchen verursacht. Dieser Faktor läßt sich mit Inulin und Erniedrigung des pH auf 5,3 fällen. Er hat keine Beziehung zu den hitzestabilen Kontaktfaktoren des Gerinnungssystems. Erhitzen und Fällung stellen keine Aktivierung, sondern eine Anreicherung dar.
