    Description / Table of Contents: Zusammenfassung Seit 1975 wurden 140 Patienten mit retroperitoneal-metastasierten nicht-seminomatösen Hodentumoren nach Orchiektomie und retroperitonealer Lymphadenektomie sequentiell alternierend mit den Zytostatika-Kombinationen Velbe/Bleomycin und Adriamycin/Cisplatin plus/minus Radiotherapie behandelt. Davon erhielten 68 Patienten nach totaler retroperitonealer Lymphadenektomie mit postoperativ normalisierten Tumormarkern (Stadium IIA) 6 Chemotherapie-Kurse, woran sich bei 35 Patienten eine Strahlentherapie anschloß. Vierzig Patienten wurden nach subtotaler retroperitonealer Lymphadenektomie oder bei postoperativ erhöhten Tumormarkern (Stadium IIB) und 32 Patienten nach palliativer Lymphadenektomie (Stadium IIC) mit mindestens 12 Chemotherapie-Kursen und fakultativer intermittierender Radiotherapie und/oder Relaparotomie behandelt. Der Vergleich der Behandlungsergebnisse bei den Stadien IIA und IIB ergab unabhängig von der zusätzlichen Radiotherapie nach der „Life-table“-Methode Vier-Jahres-Überlebensraten zwischen 80 und 100%. Diese günstigen Resultate sind mit den Ergebnissen bei 34 nicht adjuvant behandelten Patienten ohne histologisch nachweisbare retroperitoneale Metastasierung (Stadium I) vergleichbar. Ausdruck einer statistisch signifikant schlechteren Prognose bei fortgeschrittener retroperitonealer Metastasierung ist eine Vier-Jahres-Überlebensrate von 12% bei den Patienten im Stadium IIC.
    Notes: Summary Following orchiectomy and retroperitoneal lymph node dissection (RND) 140 patients with stage II non-seminomatous testicular cancer were treated by sequential combination chemotherapy consisting of vinblastine/bleomycin and adriamycin/cis-dichlorodiammineplatinum(II) (DDP), plus/minus radiotherapy. 68 stage IIA-patients (complete RND and normal tumor-markers thereafter) received 6 courses of chemotherapy, followed by radiotherapy in 35 patients. 40 stage IIB-patients (minor residual disease after RND or elevated tumor-markers after RND) and 32 stage IIC-patients (advanced residual disease after RND) were treated by at least 12 chemotherapy courses and optional intermittent radiotherapy and/or relaparotomy. In stage IIA and IIB disease the actuarial 4-year survival rates were between 80 and 100%. These favourable results were not significantly influenced by additional radiotherapy and corresponded to the survival rates for 34 stage I-patients. For stage IIC-patients the prognosis was significantly worse with a 12% 4-year survival rate.

    Description / Table of Contents: Zusammenfassung Mittels implantierter perforierter Kapseln wurde die Konzentration von Cephalotin, Cephazolin und Cefamandol in der interstitiellen Flüssigkeit des Subkutangewebes und der Niere gemessen und mit der Konzentration im Serum und Urin bei Hunden verglichen. Die Antibiotika-Konzentration in der renalen interstitiellen Flüssigkeit war niedriger als im Urin, überstieg aber die Serumkonzentration. Die drei strukturell ähnlichen Antibiotika mit unterschiedlicher Eiweißbindung im Serum und in der interstitiellen Flüssigkeit der Niere zeigten eine direkte Zeit-Konzentrationsabhängigkeit. Dieses Modell trägt zum grundsätzlichen Verständnis der Pharmakokinetik im Niereninterstitium bei, in dem sich die Pyelonephritis abspielt.

This model helps illuminate the basic pharmacokinetics of antibiotic in the renal interstitium where pyelonephritis occurs.

    Description / Table of Contents: Zusammenfassung Die Verteilung von Ciprofloxacin in der Muskulatur, dem subkutanen, perirenalen sowie perivesikalen Fett wurde nach intravenöser Bolusinjektion von 100 mg oder einer oralen Verabreichung von 500 mg untersucht. Die Muskelkonzentrationen betrugen im Mittel 0.75 mg/kg; die Diffusion in den Muskel erfolgte schnell, wohingegen die Elimination langsam war. Ähnliche Spitzenspiegel wurden im Fettgewebe gemessen; jedoch war die Diffusion in das subkutane Fett in einigen Fällen verzögert. Nach einer anfänglichen lag-Phase von bis zu einer Stunde nach i.v. Injektion war aber auch in diesen Patienten die Verteilung gleichermaßen effektiv wie in den anderen Patienten. Die Gewebespiegel nach oraler Verabreichung wurden 12 Stunden nach der Einnahme gemessen. Die mittleren Ciprofloxacin-Konzentrationen im Serum, Muskel und perirenalem Fett betrugen 0.17 mg/l, 0.20 mg/kg und 0.11 mg/kg. Somit verteilt sich Ciprofloxacin sowohl nach intravenöser als auch oraler Verabreichung effektiv im Extravasalraum.
    Notes: Summary Ciprofloxacin distribution in muscle, subcutaneous fat and perirenal and perivesical fat was studied following a single i.v. bolus injection of 100 mg or oral administration of 500 mg. Levels in muscle were on average 0.75 mg/kg; diffusion into muscle was rapid, whereas elimination from muscle was slow. Similar peak levels were recorded in fatty tissues. However, penetration into subcutaneous fat in particular may be delayed in individual cases. Following an initial lagphase of up to one hour after i.v. injection, ciprofloxacin distribution was as efficient in these patients as in the others. Tissue levels following oral administration were monitored 12 hours after intake. On average, ciprofloxacin concentrations in serum, muscle and perirenal fat were 0.17 mg/l, 0.20 mg/kg and 0.11 mg/kg, respectively. Thus, ciprofloxacin is distributed effectively throughout the extravascular space following i.v. as well as oral administration.

    Notes: Abstract The different methods of removal of a non-deflating ureteral catheter were compared with the stylet technique. Two hundred and sixty randomly selected catheters were tested. Overdistention of the balloon with water or air and chemically induced rupture of the balloon produced fragments of various sizes in almost 100%. Our stylet technique failed to demonstrate any of the complications reported in the literature i.e., chemical or mechanical injury to the bladder or retained rubber fragments causing stone formation.
