    Description / Table of Contents: Summary Blood plasma from tuberculous patients contains a factor, which inhibits the blastcell-transformation of lymphocytes in cell-cultures after addition of tuberculin. In plasma donors with active tuberculosis this factor appears to be most effective. In most cases a reduced number of blast-cells in cell-cultures of lymphocytes and plasma from the same donor is without doubt due to this inhibitory factor; however there are some cases in which an inhibition of blastcell-transformation are found even though the culture consist of lymphocytes of an active tuberculous donor and plasma from a non-tuberculous donor. These findings leed one to the conclusion, that the reduced blastcell-transformation-reactivity of the described lymphocytes is based on two factors: first, and most important, is the presence of an inhibitory factor in the plasma of tuberculous patients; second, and less frequent, the probability of the presence of a defect in the lymphocytes of certain tuberculous patients.
    Notes: Zusammenfassung Das Blutplasma an Tuberkulose erkrankter Patienten enthält einen Faktor (Hemmfaktor), der eine in der Zellkultur nach Tuberkulinzusatz auftretende blastenartige Lymphocytentransformation hemmt; bei Plasmaspendern mit einer frischen oder aktiven Lungentuberkulose ist seine Aktivität offenbar am stärksten. In vielen Fällen beruht eine geringe Blastenzahl in dem hier in Rede stehenden System bei Verwendung von Blutplasma und Lymphocyten des gleichen, an aktiver Tuberkulose erkrankten Spenders tatsächlich auf der Wirkung des Hemmfaktors; darüber hinaus gibt es aber Fälle mit aktivem Hemmfaktor, bei denen die Lymphocyten auch bei Züchtung in einem hemmfaktorfreien Plasma keine wesentlich höhere Umwandlungsrate in Blasten aufweisen, als wenn sie dem eigenen Plasma mit seinerstarken Hemmfaktorwirkung ausgesetzt waren. Hieraus resultiert, daß man vorerst den Hemmfaktor und die blastenartige Lymphocytentransformation für die Tuberkulinüberempfindlichkeit getrennt voneinander zu beurteilen hat, bis die Ursache für diese Divergenz gefunden worden ist.

    Description / Table of Contents: Summary Granulocyte function of 10 diabetic children has been investigated. At the time of testing the diabetes was in poor control. Five children were retested one week later after adjustment of diet and insulin dose. In contrast to some reports we did not find a phagocytic defect in the ingestion of particles, but the capacity of intracellular killing of Staphylococcus aureus was impaired. Chemotaxis was also reduced whereas the NBT-index and intracellular killing of Candida albicans were normal. Better control of the diabetes led to an improvement of bactericidal killing capacity.
    Notes: Zusammenfassung Bei 10 Kindern mit Diabetes wurde die Funktion der Granulocyten getestet. Zum Zeitpunkt der Untersuchung waren die Patienten schlecht eingestellt. Fünf Kinder wurden nach Korrektur der Insulindosis und der Diät eine Woche später erneut getestet. Im Gegensatz zu Berichten aus der Literatur fanden wir keine verringerte Ingestion von Partikeln. Dagegen war die intracelluläre Abtötung von Staphylococcus aureus gestört. Die chemotaktische Aktivität war ebenfalls verringert, während der NBT-Index und die intracelluläre Abtötung von Candida albicans normal waren. Optimale Einstellung des Diabetes führte zu einer Verbesserung der Abtötungsfähigkeit gegenüber Bakterien.

    Description / Table of Contents: Summary The history of a 13-year old boy is reported who suffered from frequent bacterial, enteroviral, and protozoal infections since late infancy. A decrease in the serum levels of IgG2, IgG3, IgA, a neutrophil dysfunction, and a partial cellular immune deficiency could be demonstrated. A deficiency of folic acid produced a pancytopenia which enhanced the patient's susceptibility to infections. The combined substitution of gammaglobulins and folic acid only was able to break this vicious cycle.
    Notes: Zusammenfassung Es wird über einen 13jährigen Jungen berichtet, der seit der späten Säuglingszeit an häufig wiederkehrenden bakteriellen, enteroviralen und protozoalen Infekten erkrankte. Neben der Erniedrigung der IgG2- IgG3- sowie IgA-Serumspiegel wurden eine Neutropathie und eine partielle Störung der zellulären Infektabwehr gefunden. Ein Folsäuremangel führte zur Panzytopenie und erhöhte damit noch die Infektanfälligkeit. Erst die kombinierte Substitution von Gammaglobulinen und Folsäure vermochte den Circulus vitiosus zu durchbrechen.

    Notes: Summary Several granulocyte functions were analyzed in vitro in 154 patients with chronic or recurrent infections, as well as in a variety of disorders known or suspected to affect host resistance. Only a few specific abnormalities were diagnostic and occurred in congenital, hereditary disorders. Opposed to these permanent changes are those which were probably acquired or transient and are often multifactorial in origin. In the majority of these patients, an inconstant and nonspecific pattern emerged which is not helpful in the diagnosis of underlying disease. In selected patients, however, and as research procedures, these and related tests should be helpful in elucidating the basic functions of granulocytes and may implicate therapeutical approaches.

    Notes: Summary A case of somatostatinoma syndrome in a 30-year-old woman is presented. Basal levels of growth hormone and of pancreatic and gastric hormones were reduced and the response of growth hormone, insulin and C-peptide to stimuli such as arginine, glucose, glibenclamide and calcium was virtually abolished. Similarly, gastric acid secretion, pancreatic exocrine function and intestinal absorption were significantly reduced. On the other hand, basal and stimulated levels of adrenocorticotropic hormone (ACTH), luteinizing hormone (LH), follicle-stimulating hormone (FSH) and thyroid-stimulating hormone (TSH) were within the normal range. Plasma somatostatin-like immunoreactivity was increased to 600 2,000 pg/ml (normal: 88–140 pg/ml). Immunocytochemical studies demonstrated the presence of somatostatin immunoreactive material in the primary tumour in the head of the pancreas and in the liver metastases. In spite of two courses of chemotherapy with streptozotocin and 5-fluorouracil the patient died due to liver failure 5 months after the first admission to hospital.

    Description / Table of Contents: Zusammenfassung Nicht nur die Endotoxine gramnegativer Bakterien, sondern auch die Mucopeptide verschiedener grampositiver Erreger können eine Gelierung von Amöbocytenlysaten ausLimulus polyphemus (LPL) hervorrufen. Offenbar abhängig von dem Grad der Solubilisierung ist die Wirksamkeit der Mucopeptide zwischen 1000 und 100000mal geringer als die von Endotoxin. Im Gegensatz zu Endotoxin hat die Behandlung von Mucopeptid mit Lysozym eine Abnahme bzw. einen vollständigen Verlust seiner Aktivität im LPL-Test zur Folge. Ein so behandeltes Mucopeptid ist im einfachen Capillarpräcipitationstest unter Verwendung von Antiseren gegen Mucopeptid ebenfalls nicht mehr präcipitierbar. Die Möglichkeit, die LPL-Aktivität von Mucopeptid durch Vorbehandlung mit Lysozym aufzuheben, erlaubt eine Abgrenzung gegenüber der Endotoxin-bedingten LPL-Gelierung. Bei im LPL-Test positiven Blutproben von Patienten mit Verdacht auf Endotoxämie könnte also eine Mucopeptid-induzierte LPL-Reaktion durch Vorbehandlung mit Lysozym ausgeschlossen werden.
    Notes: Summary Gelation of amebocyte lysate fromLimulus polyphemus (LPL) is induced not only by endotoxins of gramnegative bacteria but also by mucopeptides of various grampositive organisms. Solubilization considerably increases mucopeptide LPL-activity. Total activity of mucopeptide, however, is about 1000-100 000 times smaller than that of endotoxin. In contrast to endotoxin, treatment of mucopeptide with lysozyme causes decrease or complete loss of LPL-activity. Also mucopeptide thus handled does not react any more in the capillary precipitin test. The possibility to destroy mucopeptide LPL-activity by pretreatment with lysozyme permits differentiation from LPL-gelation by endotoxin. In blood specimens of patients with suspected endotoxemia and positive LPL-test mucopeptide induced LPL-reaction therefore can be excluded by pretreatment with lysozyme.

Barty kommt es in der Boyden-Kammer zu einer Virusdosis-abhängigen Hemmung der chemotaktischen Reaktionsfähigkeit der Granulozyten auf chemotaktisch aktive Substrate (F-Met-Leu-Phen; Zymosan aktiviertes Serum). Andere Zellfunktionen — Phagozytose, oxydativer Zellstoffwechsel, intrazelluläres Abtöten von lebenden Staphylokokken — werden nicht beeinflußt.
    Notes: Summary Preincubation (37°C, 60 min) of human neutrophilic granulocytes with various concentrations of infectious ECHO virus, type 9, strain A. Barty resulted in virus-dose dependent inhibition of neutrophil chemotactic reactivity (Boyden-chamber assay) to chemoattractants (F-Met-Leu-Phen: zymosan-activated human serum). Other cellular functions-phagocytosis, increase of oxidative cell metabolism, intracellular killing of live staphylococci — remained intact. This selective inhibition of human neutrophilic chemotaxis was not due to viral induced cytotoxicity or competitive inhibition of chemotactic cell surface receptors.

    Description / Table of Contents: Zusammenfassung Bei 15 Patienten mit bekannter stabiler Angina pectoris wurden die Thrombocyten von gleichzeitig aus dem Coronarsinus und der Aorta entnommenen Blutproben elektronenmikroskopisch untersucht. Im Blut aus dem Coronarsinus wiesen alle Plättchen Formveränderungen mit Pseudopodienbildung ohne Degranulation auf. Im Blut aus der Aorta war der größere Teil der Thrombocyten spindelförmig. Während einer Vorhofstimulation zeigten sich bei den meisten Patienten weder im Blut aus dem Coronarsinus, noch im Blut aus der Aorta weitere Veränderungen. Bei drei Patienten beobachteten wir in den Blutproben aus der Aorta Thrombocytenaggregate während Vorhofstimulation. Diese Patienten hatten nur wenige Risikofaktoren und zeigten weniger Coronararterienstenosen. Eine Minderheit unter den Patienten mit Angina pectoris hat offenbar Thrombocyten mit vermehrter Aggregationsneigung.
    Notes: Summary We studied 15 patients with strain-induced angina pectoris performing an ultrastructural analysis of platelets withdrawn simultaneously from the aorta and coronary sinus. In coronary sinus blood, all platelets exhibited shape changes with pseudopod formation but without degranulation. In the aortic blood we observed on average fewer shape changes of the platelets. During tachycardia stress, shape change formation did not increase in most patients. In three patients however, micro-aggregation of the platelets was observed only in the aortic blood. These patients had a paucity of risk factors. They also had fewer critical coronary artery stenoses compared with the other patients. We conclude that there may be minority among the patients with angina pectoris exhibiting hyper-aggregation of the platelets.
