    Notes: Summary In a prospective study the efficacy of two regimens for selective decontamination of the digestive tract was studied in patients with acute leukaemia during remission induction therapy. Seventy-eight patients were randomized to receive either a combination of cotrimoxazole, polymyxin B and nystatin (group A) or a combination of nalidixic acid, polymyxin B, neomycin and nystatin. With both regimens the gastrointestinal tract could be decontaminated equally effectively from potential pathogens. In the oropharyngeal region the decontamination from Enterobacteriaceae was significantly better in group A (P〈0.01). In both groups less than 10% of the acquired infections were caused by gram-negative bacilli and no gram-negative septicaemia occurred in either group. The median time interval until the first acquired infection was 17 days in group A and 36 days in group B, respectively (P〈0.05). It is concluded that regimen A might be more effective than regimen B though both regimens prevent reliably severe gram-negative infections.

    Description / Table of Contents: Summary A simple continuous filtration leukapheresis has been devised whereby two Leuko-Pak filters can be charged and eluted within 5 h with approximately 12 l of blood in a closed system of tubes. Blood flows by means of gravity without the use of pumps. Without premedication with prednisolone the average yield from 10 donors was 3.66×1010 granulocytes per leukapheresis and after premedication with 150 mg prednisolone orally it was 6.44×1010 granulocytes per leukapheresis. The in vitro function of filter granulocytes was not reduced even after premedication with prednisolone. In comparison with leukapheresis by blood cell separators this modified method of filtration leukapheresis offers the following advantages: 1) the granulocyte yield per litre of whole blood processed is higher, 2) technical assistants in a blood-bank can carry out the method on several donors simultaneously, 3) the method is more economical than leukapheresis by blood cell separators.
    Notes: Zusammenfassung Es wurde eine einfache kontinuierliche Filtrationsleukapherese (FL) zur Gewinnung von Granulozyten entwickelt, mit der zwei Leuko-Pak-Filter innerhalb von 5 h mit etwa 12 Liter Blut im geschlossenen System beschickt und eluiert werden können. Der Blutfluß erfolgt dabei ohne Pumpe allein durch Schwerkraft. Die durchschnittliche Ausbeute betrug bei 10 Spendern ohne Vorbehandlung mit Prednisolon 3,66×1010 Granulozyten pro Leukapherese, nach Vorbehandlung mit 150 mg Prednisolon p.o. 6,44×1010 Granulozyten pro Leukapherese. In vitro-Tests zeigten, auch nach Gabe von Prednisolon, keine Einschränkung der Funktionsfähigkeit der Filtergranulozyten. Die vorliegende modifizierte Form der FL hat im Vergleich zur Leukapherese durch Blutzellseparatoren folgende Vorteile: 1. Die Granulozytenausbeute bezogen auf Liter separiertes Vollblut ist höher. 2. Die Methode kann durch Hilfspersonal in einer Blutbank an mehreren Spendern gleichzeitig durchgeführt werden und sie ist 3. wirtschaftlicher als die Leukapherese mit Blutzellseparatoren.

    Notes: Abstract Fifteen children with haematological diseases received an average of 10 granulocyte transfusions each, with 4.7×1010 granulocytes per m2 body surface area (BSA) per transfusion. All patients except one had less than 100 granulocytes per μl blood combined with aplasia or hypoplasia of granulopoiesis. Thirteen patients were transfused therapeutically because of fever and/or severe local lesions unresponsive to antibiotic therapy. Two patients, who were transfused prophylactically after bone-marrow transplantation (BMT), were in excellent clinical condition during the entire period of granulocyte transfusions in spite of severe granulocytopenia. All patients except one showed definite clinical benefit from granulocyte transfusions. The non-responding patient had leukocyte antibodies.

    Description / Table of Contents: Zusammenfassung Patienten mit schwerer Granulozytopenie unterschiedlicher Genese erhielten Granulozytentransfusionen. Als in vivo Parameter für die Viabilität von transfundierten Granulozyten wurden die Fähigkeit zur Migration in die Mundhöhle (“Orogranulozyten”) und zur Phagozytose untersucht. Zwischen Granulozyten, die mit dem kontinuierlichen Blutzellseparator und Granulozyten, die durch Filtrationsleukapherese gewonnen worden waren, bestand kein Unterschied. Unter Granulozytentransfusionen bildeten sich Schleimhautulcera im Bereich der Mundhöhle zurück und heilten ab. Waren Schleimhautulcera vorhanden, kam es nach Granulozytentransfusion zu einem Anstieg der Orogranulozyten (OG). Zwischen der Granulozytenzahl im peripheren Blut und der Anzahl der Orogranulozyten bestand eine statistisch signifikante Beziehung. Als Kontrolle wurden Gesunde mit intaktem Gebiß, Zahnlose mit normalem Blutbild und eine Patientin mit akuter Agranulozytose untersucht.
    Notes: Summary Patients with severe granulocytopenia of different origin received granulocyte transfusions. The migration and phagocytic ability of the transfused granulocytes in the oral cavity (“orogranulocytes”) served as a parameter for measuring the viability of the granulocytes in vivo. There was no difference between granulocytes collected by continuous flow centrifugation and those collected by filtration leukapheresis. Under the influence of the transfusion treatment mucous membrane ulcers (MMU) subsided and healed. Where ulcers were present the number of orogranulocytes (OG) increased after granulocyte transfusion. In addition there was a significant correlation between the number of granulocytes in the peripheral blood and the number of orogranulocytes. As a control, orogranulocytes were studied in a group of healthy persons with intact teeth, toothless persons with normal blood counts and one patient with an acute agranulocytosis.
