    Notes: Summary In animal studies of myocardial ischemia/reperfusion l-arginine reduces necrotic injury by preservation of endothelial function and attenuation of neutrophil accumulation in ischemic cardiac tissue. Because release of oxygen radical species by circulating neutrophils is important in endothelial function and ischemia-reperfusion injury, this study investigated the effect of intravenous administration of L-arginine on the in vitro release of superoxide anion of neutrophils in healthy young adults. Neutrophils were obtained at various time points before, during, and after infusion of l-arginine (17 mg kg−1 min−1 for 30 min) and analyzed for superoxide dismutase inhibitable reduction of ferricytochrome c. The spontaneously occurring respiratory burst of polymorphonuclear leukocytes at basal conditions was compared with that after triggering by 1 μmol/l formylpeptide or 50 ng/ml phorbolester. Infusion of l-arginine inhibited both basal (P 〈 0.01) and formylpeptide-triggered (P 〈 0.05) release of superoxide anion did, but not affect release stimulated by phorbol 12-myristate 13-acetate. Pretreatment of neutrophils with 1 mmol/l l-arginine in vitro also significantly reduced formylpeptide-triggered (1 μmol/l) superoxide anion release, suggesting that the affects observed after in vivo pretreatment may be due to direct action of l-arginine on neutrophils. These findings demonstrate the ability of L-arginine to reduce release of oxygen radical species by circulating neutrophils in man.

    Notes: Summary Premature atherosclerosis is often found in patients with diabetes mellitus (DM) type I, and alterations in lipid metabolism seem to play an important role in the development of this complication. Intensified insulin therapy improves glycemic control parameters significantly. To evaluate the effect of this optimized insulin treatment (OIT) not only on glycemic control, but also on plasma lipids, 24 patients with DM type I (19 men and 5 women, 18 to 61 years) were switched from a standard insulin therapy to a regimen of OIT which has been maintained for more than 3 years now. After 2 years on OIT a reduction of HbAlc values from 8.1% to 7.5% (p〈0.0l) was accompanied by an increase in HDL cholesterol from 52 to 67 mg/dl (p〈0.05) and a decrease of triglyceride levels from 319 to 67 mg/dl (p〈0.001). At the end of the second year on OIT some of the patients exhibited a reversal of the favorable trend in HbAlc and lipid values. Intensified instructions regarding the implementation of OIT were therefore repeated and resulted in a renewed improvement of overall HbAlc, HDL cholesterol and triglyceride levels to 6.43%, 67 mg/dl, and 78 mg/dl, respectively. Our findings underline the value of OIT not only for glycemic control, but also for the control of plasma lipids considered to be major risk factors for coronary artery disease.

    Notes: Summary Hepatic candidiasis represents an increasingly difficult problem in immunocompromised patients. It has been recognized as a variant of disseminated candidiasis after a period of neutropenia. Establishment of diagnosis is difficult since liver biopsy as well as culture may be negative, in spite of the presence of fungus. Hepatic lesions are generally not detectable by ultrasound examination or computed tomography until the neutrophil count has returned to normal. The aim of this case report is to describe the problems related to the diagnosis and treatment of hepatic candidiasis following chemotherapy for acute leukemia. The report also provides further evidence that fluconazole, a new broad-spectrum antifungal drug, can be safely and effectively used for the long-term treatment of deep-seated hepatic candidiasis.

    Description / Table of Contents: Summary The author presents a report on eight female patients with various hematological diseases (primary and secondary thrombopenia, thrombasthenia) in whom it was possible to control severe meno- and metrorrhagia by ovulation inhibitors (the combination drugs Anovlar or Lyndiol). The treatment may be continued for a long period of time without detrimental effects on the patient. The effect of the drug is readily reversible.
    Notes: Zusammenfassung Es wird über 8 Patientinnen mit verschiedenen hämatologischen Erkrankungen (primäre und sekundäre Thrombopenic, Thrombasthenie) berichtet, bei denen schwere Meno- und Metrorrhagien durch Ovulationshemmer (Kombinationspräparate Anovlar oder Lyndiol) unterdrückt werden konnten. Die Behandlung kann ohne Schaden für die Patienten über einen langen Zeitraum fortgesetzt werden. Die Wirkung der Präparate ist prompt reversibel.

    Description / Table of Contents: Zusammenfassung In isolierten menschlichen Fettzellen des abdominellen Subcutanfettgewebes wurde der Effekt einer Allgemeinnarkose im Vergleich zur Lokalanaesthesie auf die basale und Noradrenalin-induzierte Lipolyse untersucht. Es konnte gezeigt werden, daß Procainhydrochloridin vitro stark antilipolytisch wirksam war. Präinkubation des Fettgewebes mit Procainhydrochlorid hatte keinen Einfluß auf die lipolytische Aktivität in den isolierten Fettzellen. Zwischen Allgemeinnarkose und Lokalanaesthesie bestand hinsichtlich ihres Einflusses auf die Lipolyse in isolierten Fettzellen kein signifikanter Unterschied.
    Notes: Summary In isolated human fat cells of the subcutaneous abdominal region the effect of general anaesthesia as compared to local anaesthesia on basal and noradrenaline-stimulated lipolysis has been investigated. It could be shown that procain hydrochloride exerted a marked antilipolytic action when addedin vitro. Preincubation of the whole adipose tissue with procain hydrochloride prior to the isolation procedure did not alter the lipolytic activity in the fat cells. There was no significant difference in lipolysis between fat cells obtained from patients under general anaesthesia and cells from individuals under local anaesthesia with procain hydrochloride.

    Description / Table of Contents: Summary On 3 patients suffering from primary refractory anaemia, 3 patients with sideroachrestic anaemia, 7 with stem cell leukaemia and 6 normals, the portion of DNA synthetizing bone marrow precursors was investigated using 3H-TDR puls labelling in vitro. In addition the distribution of the erythroblasts within the different stages of the cellcycle has been evaluated by combining autoradiography with quantitative cytochemical determination of the relative DNA content. In all cases the frequency of DNS-synthetizing erythroblasts was reduced. In particular that reduction was marked on polychromatic erythroblasts. In primary refractory anaemia and in sideroachrestic anaemia an increased number of G2-cells and of erythroblasts which were arrested in the stage of DNA-synthesis were observed and referred to an ineffective erythropoiesis. In contrast on patients suffering from stem cell leukaemia an increased number of erythroblasts displaying diploid DNA-values was demonstrable. Whilst the results on red cell precursors showed clear differences between primary refractory anaemia and stem cell leukaemia; the results obtained on white cell precursors were similar. In 2 out of 3 cases with primary refractory anaemia a decrease in the number of DNA-synthetizing white cell precursors could be observed, which was comparable to the degree of reduction of the proliferative activity in cases of stem cell leukaemia.
    Notes: Zusammenfassung An Knochenmarkvorstufen von 3 Patienten mit primär refraktären Anämien (pRA), 3 essentiellen sideroachrestischen Anämien (eSA), 7 unreifzelligen Leukämien wurde nach 3H-Thymidin-Pulsmarkierung in vitro der Anteil DNS-synthetisierender Zellen ermittelt. Weiter wurde an den Erythroblasten dieser Patienten durch eine Kombination von Autoradiographie mit der quantitativ-zytochemischen Bestimmung des relativen DNS-Gehaltes die Häufigkeitsverteilung dieser Vorstufen innerhalb der verschiedenen Zellzyklusphasen ausgewertet und die Ergebnisse Normalpersonen gegenübergestellt. Erythroblasten im Stadium der DNS-Synthese waren bei allen untersuchten Krankheitsbildern in ihrer relativen Häufigkeit vermindert. Von dieser Störung waren insbesondere polychromatische Erythroblasten betroffen. Unmarkierte Synthesezellen und eine Vermehrung von G2-Zellen wiesen bei pRA und eSA auf eine ineffektive Erythropoese hin. Demgegenüber war bei unreifzelligen Leukämien eine Vermehrung von Erythroblasten außerhalb des Proliferationspools nachweisbar. Während die Ergebnisse an roten Vorstufen zwischen den refraktären Anämien und den unreifzelligen Leukämien deutliche Unterschiede zeigten, waren die Befunde bei der Untersuchung granulopoetischer Vorstufen ähnlicher. Bei 2 von 3 Fällen mit pRA wurden Verminderungen des Anteils DNS-synthetisierender Vorstufen beobachtet, die an den Schweregrad der Teilungsstörung unreifzelliger Leukämien heranreichten.

    Description / Table of Contents: Zusammenfassung Lösliche Proteine einer Granulafraktion menschlicher Granulozyten wurden bei pH 4.3 mittels Acrylamid-Gelelektrophorese aufgetrennt. Die Gele wurden anschließend auf Peroxydaseaktivität und Aktivitäten von 11 Hydrolasen untersucht. Die Hydrolasen waren: saure Phosphatase, alkalische Phosphatase, Alaninaminopeptidase, Arylsulfatase, β-Galaktosidase, β-Glucuronidase, Leucinaminopeptidase, α-Naphthylacetatesterase Naphthol-AS-D-acetatesterase, Naphthol-AS-D-chloracetatesterase und Lysozym. Die Proteine mit der größten zur Kathode gerichteten Wanderungsgeschwindigkeit zeigten ausschließlich Esteraseaktivität (12 Banden) gegenüber den 3 getesteten Ester-Substraten und Lysozymaktivität (1 Bande). 2 der Esterasebanden waren stark basisch und wanderten schneller als das Lysozym. Die Esteraseaktivität konnte mit Diisopropylfluorphosphat gehemmt werden.
    Notes: Summary Soluble granule proteins from human granulocytes were subjected to acrylamide-gel electrophoresis at pH 4.3. The gels subsequently were reacted for peroxidase and 11 hydrolytic enzymes (i.e. acid phosphatase, alkaline phosphatase, alanine aminopeptidase, arylsulphatase, β-galactosidase, β-glucuronidase, leucine aminopeptidase, α-naphthylacetate esterase, naphthol AS-D acetate esterase, naphthol AS-D chloroacetate esterase and lysozyme). Out of these activities only esterase activity (12 bands) on the 3 ester substrates and lysozyme activity (1 band) could be identified with the proteins showing high cathodal migration. 2 of the proteins showing esterase activity migrated faster to the cathode than the lysozyme. Esterase activity was sensitive to di-isopropylfluorophosphate.
