    Notes: A single-electron tunneling transistor has been directly coupled on-chip to a high electron mobility transistor. The high electron mobility transistor (HEMT) is used as an impedance matching circuit with a gain close to unity. The HEMT transformed the 1.4 MΩ output impedance of the single electron tunneling (SET) transistor by two orders of magnitude down to 5 kΩ, increasing its bandwidth to 50 kHz. This circuit makes it possible to observe the motion of individual electrons at high frequencies. The requirements for the bandwidth in high frequency applications is discussed. © 1996 American Institute of Physics.

    Notes: A reliable process has been developed for the fabrication of multilevel single-electron tunneling (SET) devices. Using this process, we have fabricated SET devices with Au-SiO-Al and Al-AlOx-SiO-Al overlap capacitors. The SET transistors exhibit voltage gain and, despite the complex device structure, have a low charge noise (7×10−5e/(square root of)Hz). Moreover, the use of overlap capacitors in SET devices results in a reduction of cross capacitances down to 8%. © 1995 American Institute of Physics.

    Notes: There are many significant differences, but also considerable overlap between the quantitative histopathological features of mild and marked atypical endometrial hyperplasias and well and moderately differentiated carcinomas, thus preventing its application to individual patient care. To try to overcome this problem, a classification rule for the diagnosis in individual patients, using discriminant analysis has been developed. Utilizing nine quantitative features, all the above four groups can be adequately separated. None of the carcinomas was misclassified as hyperplasia, and only one case of marked atypical hyperplasia was erroneously classified as well differentiated carcinoma, but with a probability of carcinoma 0.75, hyperplasia 0.25. By contrast, the classification probabilities of all the confirmed carcinomas exceeded 0.90. Therefore, using 0.90 as a classification level (‘threshold’), a reliable rule is obtained. A slightly more simple classification rule distinguishes between all the hyperplasias and all the carcinomas. In this way, all the cases of the test set were correctly classified. The classification rules can be used to select patients with benign disease for hormone therapy(Kistner 1973) as an alternative to hysterectomy, and can be programmed in an inexpensive microcomputer. The quantitative techniques are relatively easy, and are capable of being performed in most histopathological laboratories.

    Notes: The numbers of IgA, IgM and IgG-containing cells were studied by means of an indirect immunoperoxidase technique and morphometry in liver biopsies of patients with primary biliary cirrhosis and chronic hepatitis, in whom serum immunoglobulin concentrations were also determined. In patients with primary biliary cirrhosis the absolute and relative number of IgM-containing cells in the liver was significantly higher, whereas the absolute and relative number of IgG-containing cells in the liver was significantly lower compared to patients with chronic hepatitis. IgM-containing cells in liver biopsies of patients with primary biliary cirrhosis correlated strongly with their serum IgM levels. It is concluded that determination of the pattern of immunoglobulin containing cells in liver biopsies may help in the differentiation of primary biliary cirrhosis from chronic hepatitis and that local production of IgM in the liver may contribute significantly to the high serum IgM levels in patients with primary biliary cirrhosis.

    Notes: Discrimination between borderline and malignant mucinous ovarian tumours is a well-known diagnostic problem. In order to obtain objective reproducible and consistent features for differential diagnosis, 32 quantitative microscopical features were assessed in 10 benign, 10 borderline and 22 malignant mucinous ovarian tumours. There were many significant differences between the three groups, but using multivariate analysis there was 93% agreement between the histopathological assessment of these sections and the qualitative analyses. The following features were useful in the quantitative classification: the mean area, the mean perimeter and the mean of the short axis of the nucleus; the volume percentage of the epithelium; the mitotic activity. In three cases, there was a difference between the original histopathological and computer classification. It was debatable whether the original diagnosis was correct, and therefore, all the cases were independently reassessed blind by three pathologists. Their diagnoses lend strong support to the computer classification in two of the three cases. The computer classification seems therefore to be even better than 93%. The present quantitative techniques are inexpensive, relatively easy to use, and, we believe, have a useful place in diagnostic histopathology.

    Notes: The presence of calcitonin and of carcinoembryonic antigen (CEA) was studied in six cases of medullary carcinoma of the thyroid using an immunoperoxidase technique. In five cases the material was obtained surgically and in one at autopsy. Tissue from primary tumours as well as from metastases was studied. Calcitonin and CEA were identified within all the tumours studied, although their pattern of distribution and staining intensity varied both within the cells and within the tumour. Some parts of the tumour contained both CEA and calcitonin, while others stained positively only for one of these substances. In some parts of the tumour there was no positive staining for either substance. Within the cells, CEA showed a typical linear distribution along cell surfaces, while calcitonin showed a more even cytoplasmic distribution and the deposits were more granular. Normal tissue surrounding tumour deposits did not show positive staining. It is considered that cells of medullary carcinoma of the thyroid contain both calcitonin and CEA. Identification of CEA and calcitonin in tumour tissue can be used as a diagnostic aid to identify medullary carcinoma of the thyroid. It is considered that these substances are being produced by this tumour and can be used as tumour markers.

    Notes: In a group of 335 patients with primary breast carcinoma the presence of immunoreactive carcinoembryonic antigen (CEA) and the binding of the lectin peanut agglutinin (PNA) in the primary carcinoma and in axillary lymph node metastases were investigated. The correlation between these results and a variety of established clinical, histopathologic, morphometric and biochemical prognosticators was studied. These features included lymph node status, tumour diameter, tumour type, nuclear grade, histologic grade, oestrogen receptor status, mitotic activity index and a number of nuclear measurements. The results indicate that CEA immunoreactivity of and PNA binding to tumour cells in primary breast carcinomas or lymph node metastases do not correlate with established prognostic factors in breast cancer.

    Notes: Eighteen months experience in the use of an automated staining machine for lengthy histological and immunohistochemical stains in a routine pathology laboratory is described. The apparatus yields an important saving in time for technicians and can maintain the quality of staining at a standardized high level. A disadvantage is the larger volumes of reagents required. Introduction of the machine is recommended when at least five different but lengthy stains have to be performed each day. The considerable gain in time for the technicians has important consequences for the management of pathological laboratories in the near future.

    Notes: Summary Jejunal mucosa biopsies from non-immune deficient patients with Giardia lamblia infestation were examined and showed three different groups of mucosal changes, distinguishable on morphological and immunohistochemical grounds. In three patients no morphological or immunohistochemical abnormalities were found (group A). In five patients a normal villous architecture was seen. These biopsies had increased numbers of interepithelial lymphocytes and of immunoglobulin containing cells in the lamina propria, with a relative increase of the number of IgA and IgG containing cells (group B). Two patients with a malabsorption syndrome due to giardiasis had marked villous atrophy, documented by morphometric measurements and large numbers of interepithelial lymphocytes and of immunoglobulin containing cells in the lamina propria, especially IgA and IgG (group C). These findings differ considerably from those in patients with immunodeficiency or gluten sensitive enteropathy. This suggests that when villous atrophy of the jejunal mucosa is found immunohistochemistry of Jejunal biopsy specimens may be helpful in the differential diagnosis between mere giardiasis and giardiasis superimposed on immunodeficiency or gluten sensitive enteropathy.
