    Notes: Summary Stereoscopic structures of cancellous bone of human vertebral bodies were studied with a scanning electron microcope at autopsy of 2 cases of primary and 4 of secondary hyperparathyreoidism. One principle finding are enlarged resorbing surfaces all over numerous bone trabeculae. There is a compensatory activity of forming surfaces, which brings about mineralizing fronts and extended active osteoid seams. Active fiber osteoid predominates in cases with secondary (renal). HPT. No morphological differences can be demonstrated on bone trabeculae of dialyzed or non-dialyzed patients. These results are in a certain contrast to SEM findings in rat parietal bone after experimental PTH administration, which showed a striking increase of resting surfaces. These short-term animal experiments, however, are not really comparable to human chronic hyperparathyreoidism. The osteocytes of lamellar compact bone prove to be enlarged in cases of primary and secondary hyperparathyreoidism as compared to specimens of normal compact bone. The difference is slight but is statistically significant. We explain this not as periosteocytic osteolysis but as an effect of the disturbed maturation brought about by defective mineralization of the osteocyte capsules.

    Notes: Summary A slide rule has been devised which is based on the general mathematical models of pharmacokinetics. It permits calculation of exact dosage regimens for individual patients from certain basic parameters. First, from the patient's renal clearance, the proportionality constant characterizing renal excretion of a certain drug (a) and its non-renal rate constant of elimination (k nr), the rate constant of total elimination (k e) can be calculated. Second, fromk e, the apparent volume of distribution (V d) and the desired final mean concentration of a drug (c∞), exact values can readily be obtained for the loading dose (D*) and the dosage schedule, which consists of the maintenance dose (D), the dosing intervals (τ) and the infusion rate for intravenous administration. In addition the slide rule provides information about the rate at which c∞ is reached ifD alone is administered at τ, and the fluctuation in the concentration around c∞ to be anticipated during τ. By use of this calculation, the slide rule facilitates the decision whether a loading dose should be given, and what dosage schedule is best suited to the therapeutic problem. It is possible, therefore, to calculate exact dosage regimens for individual patients, even for those with excretory dysfunction. The slide rule should also help physicians to comprehend the nature and significance of pharmacokinetic mechanisms.

    Description / Table of Contents: Summary The one-dimensional flow induced by an arbitrary motion of a piston into a gas with arbitrary internal rate processes is studied. For times, which are smaller than the relaxation times, the flow is described by an asymptotic expansion in the Damköhler-number. The first order quantities are given analytically and discussed in two special cases. The first case is an example for relaxation to favour the formation of a shock wave. In the second case the influence of relaxation on a rapid pulse is investigated.
    Notes: Übersicht Es wird die durch eine beliebige Bewegung eines Kolbens in einem zylindrischen Rohr verursachte Strömung eines Gases untersucht, in dem beliebige Relaxationsprozesse ablaufen können. Die Strömung wird für Zeiten, die klein gegen die Relaxationszeiten sind, durch eine Entwicklung nach der Damköhlerzahl beschrieben. Die Strömungsgrößen 1. Ordnung werden analytisch angegeben und für zwei Spezialfälle diskutiert. Der erste Fall ist ein Beispiel dafür, daß die Relaxation die Entstehung eines Verdichtungsstoßes beschleunigen kann. Im zweiten Fall wird der Einfluß der Relaxation auf ein kurzes Signal untersucht.

    Description / Table of Contents: Summary Study of the literature and experimental investigations have shown that storage of blood in the ACD stabilizer is not optimal when compared with a phosphate-dextrose-saccharose stabilizer. Further it was shown that the metabolic kinetics and the speed of permeability for anions did not correspond with Arrhenius's law. The divergence was most marked below 10°C. In the usual ice-chest range of temperature (about 4°C) the difference between the course of metabolic processes and permeability leads to increase of lactic acid in the erythrocytes, causing a shift of intracellular pH. In order to intimate physiological conditions as closely as possible, it is suggested that blood should be stored in synthetic containers permitting dialysis and that it should be surrounded by a suitable dialyzable fluid.
    Notes: Zusammenfassung Wir konnten auf Grund von Literaturangaben und experimenteller Untersuchungen im Vergleich mit einem Phosphat-Dextrose-Saccharose-Stabilisator zeigen, daß die Lagerung von Blut in ACD-Stabilisator nicht den optimalen Bedingungen entspricht. Ferner wurde festgestellt, daß die Stoffwechselkinetik und die Permeabilitätsgeschwindigkeit für Anionen nicht dem Gesetz vonArrbenius gehorcht. Besonders augenfällig sind die Abweichungen bei Temperaturen unter 10°C. Die Differenz zwischen Stoffwechselablauf und Permeabilität bei den üblichen Kühlschranktemperaturen um 4°C führt zu einer starken Milchsäureanreicherung in den Erythrozyten, die eine intrazelluläre pH-Verschiebung verursacht. Um die physiologischen Bedingungen anzunähern, wird vorgeschlagen, das Blut in dialysierfähigen Kunststoffbeuteln zu lagern und mit einer geeigneten Dialysierflüssigkeit zu umspülen.

    Description / Table of Contents: Zusammenfassung Zwei innerhalb eines Jahres sezierte Fälle von plötzlichem Tod bei elektrischer Dauerstimulation des Herzens durch implantierte Schrittmacher gaben zu der Überlegung Anlaß, wie häufig mit Komplikationen und Spättodesfällen bei solchen Patienten gerechnet werden muß, da dies für die Ausübung spezieller Berufe und insbesondere für die aktive Teilnahme am Straßenverkehr von Bedeutung erschien. Eine Zusammenstellung mehrerer Statistiken mit insgesamt 1341 Behandlungsfällen zeigte, daß Reoperationen infolge von Störungen unterschiedlicher Art in einem großen Teil der Fälle notwendig wurden. Spättodesfälle traten 97–105mal auf 691–969 Patienten auf. In nur 2% waren technische Störungen die Ursache. Auf Grund der plötzlichen Spättodesfälle und der häufig außerhalb der Klinik auftretenden Komplikationen, welche durch den Stress des Straßenverkehrs u. U. begünstigt werden, erscheinen elektrisch dauerstimulierte Patienten für bestimmte Berufe mit Verantwortung für Menschenleben und insbesondere für die aktive Teilnahme am Straßenverkehr ungeeignet.
    Notes: Summary Two cases of sudden death of individuals with cardiac pacemakers were autopsied within a one year period. Speculations regarding reintegration of such patients into certain professions and particularly their ability to participate in road traffic gave rise to the current report. A survey of several statistical evaluations comprising 1341 patients with pacemakers indicated the necessity of repeated surgical intervention because of various failures of stimulation in a large number of cases. Delayed deaths occurred 97–105 times in 691–969 patients. In only 2% of these deaths technical failure was the cause. In consideration of the suddenness with which late complications occur and their aggravation by stress associated with road traffic it appears that patients with pacemakers are unsuitable for certain professions involving the lives of others and should not be granted driving permits.
