Während bei den Kontrollpersonen die höchsten SCMC-und ADCC-Werte mit der von Phagozyten gereinigten Lymphozytenfraktion FFF zu erzielen waren, gefolgt von der phagozytenhaltigen Fraktion F (‚Ficoll“) und defibriniertem Vollblut, verhielt es sich bei den akuten Leukosen umgekehrt. Hier fanden sich die höchsten zytotoxischen Aktivitäten im Vollblut, während die Zellfraktionen F und FFF niedrigere und im Gegensatz zu den Kontrollen meist diskordante SCMC- und ADCC-Werte aufwiesen. Vergleichweise hohe zytotoxische Aktivitäten fanden sich bei 2 Leukämiepatienten, die am Tage vor der Testung Bluttransfusionen erhalten hatten. Im Verlauf einer Erythroleukämie ging die chemotherapeutische Behandlung mit einem drastischen Abfall von SCMC- und ADCC-Aktivität einher. Andererseits hatte eine therapeutische Splenektomie bei einer Haarzell-Leukämie keinen Einfluß auf die zytotoxischen Aktivitäten. Bei den angio-immunoblastischen Lymphadenopathien fiel ein stark erhöhter Prozentsatz an EA-und EAC-Rosetten bildenden Zellen im Blut auf; parallel hierzu war ein ausgeprägter SCMC- und ADCC-Anstieg nach Elimination mononukleärer Phagozyten aus der Effektorpopulation zu registrieren.
    Notes: Summary Blood lymphocytes from 13 untreated acute leukemia patients, 3 pre-leukemias 3 immunoblastic lymphadenopathias and one infectious mononucleosis showed significantly lower spontaneous (SCMC) and antibody-dependent cellular cytotoxicity (ADCC) against51Cr-labeled allogeneic melanoma cells of the IGR3 cell line than effector lymphocytes from 20 age- and sex matched control persons. While control lymphocytes exhibited the highest cytotoxic activity after depletion of mononuclear phagocytes (Fraction FFF), followed by the “Ficoll” purified Fraction F and defibrinated whole blood, the reverse was true for acute leukemias: here, the highest cytotoxicity was found in whole blood followed by the lymphocyte fractions F and FFF. Comparatively high cytotoxicity was found with two leukemia patients who had received blood transfusions the day before testing. During the course of an acute erythroleukemia chemotherapy drastically reduced SCMC and ADCC activities. A therapeutical splenectomy, on the other hand, did not affect cellular cytotoxicity in the case of a hairy cell leukemia. The angioimmunoblastic lymphadenopathies showed strikingly high percentages of EA- and EAC-rosettes forming cells and showed a marked increase of SCMC and ADCC activities after elimination of mononuclear phagocytes from the effector cell population.

    Description / Table of Contents: Summary A new technique is described which enables a selective tinctorial visualization of human blood basophils in autoradiographic preparations. Consecutive administration of 5-aminoacridine hydrochloride and aldehyde fuchsin rendered a sufficient preservation of basophils preventing leaching of their water soluble granules. Extensive control tests provided sufficient evidence to exclude chemographic artifacts produced by other histochemical procedures. The technique eliminates the obstacles to study the kinetics of human blood basophils and their relationship to atopic hypersensitivity reactions applying tritiated thymidine and radioiodine labeled lgE or corresponding antisera.
    Notes: Zusammenfassung Es wird eine neue Technik beschrieben, die eine selektive Darstellung der menschlichen Blutbasophilen in autoradiographischen Präparaten ermöglicht. Durch sukzessive Anwendung von 5-Aminoacridin-Hydrochlorid und Aldehyd-Fuchsin werden die wasserlöslichen Strukturen der menschlichen Blutbasophilen für den autoradiographischen Prozeß stabilisiert, ohne daß dabei chemographische Artefakte auftreten. Die Methode erlaubt es, die Kinetik der Blutbasophilen mit markierten Kernbasen sowie ihre Rolle im Rahmen atopischer Reaktionen durch Anwendung von radiojodmarkiertem Immunglobulin E oder von entsprechendem Antiserum zu studieren.

    Description / Table of Contents: Summary Karyometric investigations of bone marrow smears have shown distinct groups of nucleal volumes which represent a geometric progression. In view of the definite nucleal alterations which, according toRubrmann, are implied by smear preparation technique we have prefered measurements in tissue sections (fixation in methanol-formol, acrylate embedding). Compiling of test data in terms of nucleal volumes and logarithmical classification is called in question and substituted by discriminant-analytical evaluation. We cannot confirm the above mentioned mathematical correlations which have been used for the interpretation of the dynamics of bone marrow cell proliferation. Furthermore, isolated nucleal measurements have not proved to be apt for a well-defined distinction of bone marrow cells.
    Notes: Zusammenfassung Die bislang fast ausschließlich in Ausstrichpräparaten durchgeführten karyometrischen Untersuchungen des Knochenmarks haben bestimmte Kernklassen ergeben, welche Volumenhalbierungsschritte darstellen. Da die Kernveränderungen im Ausstrich nachRubrmann zu erheblichen Fehlern führen, haben wir Messungen in Schnittpräparaten durchgeführt (Fixierung in Schafferscher Lösung, Methylacrylateinbettung). Die bisher übliche Ordnung der Meßwerte durch Volumenberechnung und logarithmische Klasseneinteilung stellen wir in Frage und ziehen eine diskriminanzanalytische Auswertung vor. Die obengenannten gesetzmäßigen Beziehungen, die zur Deutung der Proliferations-dynamik des Knochenmarks herangezogen worden sind, können wir nicht bestätigen. Auch zu einer eindeutigen Unterscheidung der Knochenmarkszellen sind isolierte Kernmessungen nicht geeignet.

    Notes: Abstract Cytogenetic analyses were performed on 266 bone marrow and peripheral blood samples from 179 patients with myelodysplastic syndromes (MDS). According to the FAB classification, 42 patients presented with RA, 18 with RARS, 37 with RAEB, 22 with CMML, and 29 with RAEB-T. Nine patients showed a secondary MDS (S MDS). FAB classification was not available for 22 patients. Clonal karyotype anomalies were found in 92 patients (51.4%). Complex chromosome abnormalities occurred in 17 (18.5%) of them. An evolution of the karyotype was detected in 16 cases (17.4%). Cytogenetically independent cells or cell clones were found in eight patients. Nonclonal chromosome abnormalities were uncovered in 29 (16.2%) of the 179 MDS patients. Consecutive studies were performed in 48 patients and revealed a good correlation of initial karyotype and clinical course. The most frequent single anomalies were 5q- in 29 (31.5%), −7 in 22 (23.9%), trisomy 1q in 14 (15.2%), and +8 in 13 (14.1%) of 92 patients respectively. Our cytogenetic findings are presented in detail and discussed in relation to patients' age, morphological classification, clinical course, and prognostic impact. The contribution of cytogenetic findings to the delineation of multistep pathogenesis of MDS with special emphasis to karyotype instability is demonstrated.

    Notes: Abstract Background: In October 1992, an ongoing prospective study on primarygastrointestinal (GI) lymphoma was initiated to evaluate histologicalfeatures, sites of involvement, and management. Patients and methods: Until May 1996, 352 patients were enrolled, with 279being evaluable for clinical features (208 patients presented with primarygastric lymphoma). Standardized diagnostic workup included central histologicreview and endoscopic and radiologic evaluation of the complete GI tract.Primary surgery or conservative management depended on the physician'sdecision, followed by radiotherapy with or without chemotherapy. Treatmentoutcome is evaluable in 122 patients with gastric lymphoma. Results: In 279 evaluable patients, the distribution of NHL was asfollows: stomach 74.6%, small bowel 8.6%, ileocoecal region6.5%, multilocal GI involvement 6.8%. In gastric lymphoma,low-grade NHLs accounted for 39%. Of the remaining high-grade NHLs,36.1% showed simultaneous low-grade components, thus being also of MALTorigin. Of 208 patients with gastric NHL, 71.1% were classified asstage I and II1. CCR rate in stomach lymphoma is significantlyhigher compared to those of the small bowel, whereas involvement of multipleGI organs has the worst prognosis. So far only 7 patients with gastric NHL in stages I and IIpresented with progressive disease or relapse. Over all stages there seems tobe no difference in therapeutic outcome in surgically or conservativelytreated patients. Even after R0-resection in limited stages patients appearto have no better outcome. Conclusion: The value of surgery in treatment of primary gastriclymphoma – as favored by most authors – should be reexamined.

    Notes: Summary Recently the bone-marrow cavity blood concentration of parathyroid hormone (PTH) has been shown to exceed that of the peripheral blood. As PTH is a primary modulator of bone cell activity, altered levels of the hormone in the bone-marrow blood may play a significant role in the aetiology of bone disease. We therefore measured PTH concentrations in marrow cavity and venous blood of 9 osteoporotic and 14 control subjects using sequence specific radioimmunoassays for intact and mid-carboxyl (Mid-C) regional human PTH (hPTH). Intact and Mid-C PTH levels were identical in the peripheral blood of control and osteoporotic subjects. Furthermore, bone-marrow cavity blood concentrations of Mid-C PTH, whilst universally higher than those found in peripheral blood, were also comparable in the osteoporotic and control subjects. The sole difference in the PTH composition of bone-marrow cavity blood from osteoporotic subjects was an increased concentration of intact PTH. The origins and consequences of elevated levels of intact PTH within the marrow cavity blood of osteoporotic subjects are discussed.
