    Description / Table of Contents: Zusammenfassung Die endogene Proteinsynthese und die Poly (U)-abhängige Polyphenylalanine Synthese wurde in Blutlymphozytenextrakten von 50 CLL-Patienten bestimmt und mit dem prognostischen Stadium verglichen. Die Patienten wurden nach der neuen internationalen Workshop Klassifikation in drei prognostische Gruppen (A, B, C) eingeteilt. 23 Patienten befanden sich im Stadium A, 14 im Stadium B und 13 im Stadium C. Extrakte von Patienten mit schlechter Prognose, d.h. mit Anämie und/oder Thrombopenie (Stadium C) hatten eine signifikant höhere Poly (U) Translationsaktivität als die von Patienten mit guter bzw. mittlerer Prognose (Stadium A oder B) (P〈0.01). Die Sensitivität dieses Befundes lag bei 62%, die Spezifität war 100%. In Verlaufsstudien wurde neben einem Anstieg der Poly (U) Translationsaktivität auch ein Anstieg der endogenen Proteinsynthese beim Übergang vom Stadium A bzw. B zum Stadium C beobachtet. Die Aktivität der Proteinsynthese korreliert nicht mit der Lymphozytenver-doppelungszeit, nicht mit der Expression von Oberflächenmarkern und nicht mit den Serum-Immunglobulinspiegeln.
    Notes: Summary Protein synthesis primed by endogenous messenger RNA (mRNA) as well as polyuridylic acid-[poly (U)] directed polyphenylalanin synthesis was measured in extracts of blood lymphocytes from a series of 50 chronic lymphocytic leukemia (CLL) patients and compared with the prognostic stage. Patients were clinically classified according to the new international workshop classification [4]. There were 23 patients at stage A, 14 at stage B and 13 at stage C. Extracts from patients of the high risk group (stage C), defined by anemia and/or thrombocytopenia, exhibited a significantly higher poly (U)-translation activity than extracts from low and intermediate risk patients-stages A and B−(P〈0.01). This finding has a sensitivity of 62% but a specifity of 100%. During follow-up, an increase of poly (U)-translation and endogenous protein synthesis was observed after changing from stages A or B to stage C. Activity of protein synthesis could neither be correlated with proliferation activity, as measured by lymphocyte doubling time, nor with the expression of immunologic surface markers, nor with serum immunoglobuline (Ig) levels.

    Notes: Summary.  We here report a patient who developed a high titer antibody to factor VIII (FVIII) during gram-negative urosepsis caused by enterobacter cloacae after complete resection of rectal cancer. The patient initially presented with a life threatening spontaneous hematothorax and multiple large haematomas. Coagulation studies revealed a severe FVIII deficiency 〈1% with a high FVIII antibody titer of 64 BU. The bleeding responded rapidly to infusions of recombinant factor VIIa. After achievement of a partial remission (FVIII activity 28%) by combined immunosuppressive therapy (prednisone, cyclophosphamide, plasmapheresis and immunoadsorption), subsequently, two relapses occurred following steroid tapering. Resumption of prednisone and cyclophosphamide treatment combined with immunoadsorption induced a second and third remission, respectively. After resection of a papillary carcinoma of the bladder 6 months later and continuous immunosuppressive therapy with cyclophosphamide, FVIII levels remained stable within normal ranges. This clinical course suggests that the cause of inhibitor formation against FVIII resulting in severe acquired haemophilia was multifactorial and was initiated by the gram-negative urosepsis and probably by the underlying malignancies.

    Notes: Summary Synthetic alkyl-lysophospholipids (ALPs) inhibit the proliferation of human hypernephromas in vitro. Cells of ten different tumors were incubated with 4 ALPs for periods of more than 24 h. Eight of ten cell lines showed proliferation rates below 1% of the controls after cultivation. One microgram of ALPs per 106 tumor cells was effective, in some experiments a dose response relation was found for even lower concentrations. Equivalent concentrations of cytostatic drugs did not show reproducible higher antitumor effects in vitro. In two of the tested cell lines ALPs did not show any reproducible tumor growth inhibition, whereas at least some of the cytostatic drugs revealed slight cytostatis.

    Notes: Summary Preliminary results of a cooperative pilot study on the effect of fibroblast interferon in 10 patients with NHL of low-grade malignancy are presented. In eight patients lymphoma progression forced a discontinuation of therapy. At the end of interferon treatment one patient with CBCC showed a stable disease, one patient with CBCC a minor partial remission.

    Notes: Summary The platelet activating factor (PAF), a low molecular phospholipid, plays an important role in inflammation, anaphylaxis, and shock state development. In the isolated perfused guinea pig heart, PAF induces a decrease in coronary flow and cardiac contractility and atrioventricular conduction disturbances. Furthermore, PAF mediates a powerful bronchoconstrictory action causing a severe impairment in respiratory function. In the present study an attempt was made to separate cardiac from respiratory events during PAF-induced shock in vivo. PAF was injected intravenously (0.1–10 μg/kg) into anesthetized guinea pigs ventilated with room air or 100% oxygen. Administration of 10 μg/kg PAF was uniformly lethal: already within 2 min, cardiac output decreased by 60% and end-diastolic left ventricular pressure increased markedly indicating cardiac failure. ECG recordings showed signs of acute myocardial ischemia. Arrhythmias occurred in terms of atrioventricular conduction delay. Blood pressure initially increased, then declined continuously to below baseline within 10 min. All animals died within 25 min. Ventilation with room air was paralleled by development of severe hypoxia. However, under ventilation with 100% oxygen a dissociation between PAF-mediated cardiac and respiratory effects occurred. It is concluded that the PAF-induced shock is primarily based on direct cardiac damage. Furthermore, the ECG signs of ischemia are most likely due to coronary spasms.

    Notes: Summary A case involving a 67 year old man with an immunocytoma initially presenting as Waldenström's macroglobulinemia which transformed into a high grade malignant lymphoma of immunoblastic type after treatment with cyclophosphamide and corticosteroids over 5 months is presented. IgM (kappa) present in the serum in the phase of immunocytoma was demonstrated also on the less differentiated cells of the high grade lymphoma. The permanent production of the same immunoglobuline molecule suggests that both morphologically different malignancies derived from one B cell clone.

    Notes: Summary Recombinant human (rh) erythropoietin (EPO) is attracting increasing interest as an agent for treating cancer-related anemia. Thus, we have tested the effects of rhEPO on the clonal growth of 22 different cell lines derived from a wide range of human solid tumors (head and neck 3, lung 2, breast 2, stomach 1, colorectal 3, hepatocellular 1, pancreas 1, ovary 1, choriocarcinoma 1, osteogenic sarcoma 1, glioblastoma 2, neuroblastoma 1, prostate 1, renal 2) in vitro. RhEPO (dose range 0.01–100 U/ml) caused no significant and reproducible stimulation of clonal growth as measured by a capillary modification of the human tumor cloning assay in agar in any of the cell lines tested. In particular, there was no sensitivity for rhEPO of those cell lines which were shown to be responsive to interleukin-3 and GM-CSF. On the other hand, there were no growth inhibitory effects of rhEPO on the cell lines of this study. Finally, neutralizing anti-human EPO antibody had no effect on the clonal growth of two kidney carcinoma cell lines, making autocrine growth regulation by hEPO in these lines unlikely.

    Notes: Abstract Purpose: To determine the maximum tolerable dose (MTD), principal toxicity, and pharmacologic behaviour of Cemadotin—HCl, a novel antimitotic peptide. Patients and methods: Cemadotin—HCl (10.0 to 27.5 mg/m2/day every three weeks) was administered as a 24-hour intravenous (i.v.) continuous infusion to patients with advanced cancer. Pharmacokinetic analyses were performed during the first treatment cycle. Blood samples were taken over 48 hours and analyzed by radioimmunoassay. Results: Hypertension was the dose-limiting toxicity (DLT). This type of toxicity was observed at all dose levels, but grade 3 (CTC) was observed only at dose levels 20.0, 25.0 and 27.5 mg/m2. This effect was reversible but in three patients associated with signs of cardiac ischemia. Other significant toxic effects were neutropenia, asthenia, tumor pain and transient liver enzyme elevation. A linear pharmacokinetics was observed. The best curve fit was obtained with a two-compartment model with a terminal half-life of ≈10 hours at each dose level, a volume of distribution at steady state of ≈9 l/m2 and a total clearance of ≈0.6 l/hour/m2. Neither partial nor complete responses were observed although minor tumor regressions were seen in a patient with carcinoma of unknown primary (CUP) and in another patient with liver metastases from a colon cancer. Conclusions: Hypertension was the dose-limiting toxicity of Cemadotin—HCl administered as a continuous 24-hour infusion. The recommended dose for further evaluation of its anticancer efficacy in disease-oriented phase II studies with this schedule is 15.0 mg/m2. The nature of the principal cardio-vascular toxicity remains unclear. The observed toxicities appeared to be significant but manageable.

    Description / Table of Contents: Summary Immunity against tetanus in an urban population: gaps of immunity among elderly persons. In a sero-epidemiological survey we investigated the immunity against tetanustoxoid in an urban population. Children and adolescents birn in Germany (n=193) showed an excellent immunity, negative findings being the exception. Among soldiers of the army (n=136), negative findings were rare too. In these boths groups the antibody titiers averaged 4,5 IU/ml. It is generally agreed that an antibody titer of 0,01 IU/ml protects against tetanus. Adults born in Germany (n=295) had a good immunity: 84 % showed antibody — titers against tetanus. Among elderly persons, how ever, above 60 years of age (n=1576) there was a striking incidence of negative findings: in 1043 persons (65 %) no antibodies could be dewtected. The remaining 533 persons with positive findings (35 %) had antibody-titers in the lower range (average 0,482 IU/ml). Also among foreigners and immigrant labourers from mediterrean countries (Greece, Turkey, Yugoslavia) the frequency of negative findings was hishg: 183 persons were examined, 70 (39 %)were negative 113 (61 %) were positive. The incidence of negative findings was highest among women from the mediterrean countries. Several small studies published in Switzerland show a similar tendency. Our paper will encourage further seroepidemiological studies.
    Abstract: Resumé Au cours d'une étude sére-epidémiologique on a examiné l'immunité de la population d'une grande ville à l'égard du toxoide tétanique. Les enfants et les adolescents nés en République fédérale (n=193) étaient presque tous vaccinés; parmi les soldats de l'armée fédérale ne so sont trouvés presque que des résultats positifs également (n=136). Les titres d'anticorps de ces deux groupes étaient en moyenne de 4,5 Ul/ml (un titre d' anticorps d'au moins 0,01 Ul/ml protège contre le tétanos). Les enfants étrangers (nés en Yougoslavie.Crèce et Turquie (n=110), jouissaient d'une bonne protection contre la maladie. Parmi des Allemands adultes (n=295), les résultats positifs prédominaient (84 %). On a constaté un mauvais état de vaccination chez les personnes âgées de plus de 60 ans (n=1576): 533résultats positifs (35 %) contre 1043 résultats négatifs (65 %). Les traivailleurs étrangers en provenance de pays méditeranéens (n=183) étaient aussi insuffisamment vaccinés (39% de résultats négatifs), la situation étant moins bonne encore chez les femmes que chez les hommes. La situation en Suisse devrait être comparable comme le montrent quelches études. De telles enquêtes séro-épidémiologiques devraient être encouragées.

Die Antikörper dieser beidenGruppen lagen imDurchschnitt bei 4,5 IE/ml (ein Antikörpertiter von mindestens 0,01 IE/ml schützt gegen Tetanus). Bei ausländischen Kindern (Geburtsorte in Jugoslawien, Griechenland, Türkei; n=110) war der Impfschutz gut. Unter deutschen Erwachsenen (n=295) überwogen positive Befunde (84 %).
