Summary This is the first case of a megakaryoblastic myelosis, diagnosed in its early phase by bone marrow biopsy, with clinical follow up study and confirmation by autopsy findings. In the initial stage megakaryoblastic and erythroblastic proliferation in the bone marrow, later a few atypical megakaryoblasts and erythroblasts as well as hematopoietic stem cells in the peripheral blood, in association with constant pancytopenia. At the end stage of a 18 month clinical course almost exclusively megakaryoblasts in the bone marrow. Autopsy findings with generalized involvement of spleen, lymphnodes and liver by hematoblastosis without transgression of the confinements of individual organs. Quantitative and qualitative evaluations revealed a close relationship between proliferation of megakaryoblasts/erythroblasts and endothelial cells of the blood sinus of the bone marrow, which together with other observations points to a neoplastic proliferation of the later cells as basis for this disease. These findings, moreover, may indicate a post-partum, hematopoietic omnipotence of these endothelial cells.
Notes:
