    Notes: Abstract Thrombomodulin is an endothelial cell membrane glycoprotein and is detected in plasma and serum after endothelial injury. In our study comprising 311 patients with systemic lupus erythematosus (SLE) clinical and laboratory associations of elevated thrombomodulin serum concentrations were examined. Elevated thrombomodulin concentrations were detected in 7.1% of the SLE patients and were associated with nephritis including the laboratory parameters proteinuria and erythrocyte casts, vasculitis and neurological involvement of the central nervous system. These correlations remained significant after consideration of the influence of renal function. In SLE, the serum thrombomodulin concentration may become a marker to monitor damage of endothelial cells and involvement of the central nervous system.

    Notes: Abstract Epstein-Barr-virus (EBV)-transformed lymphoblastoid B-cell lines were generated from peripheral blood lymphocytes of 55 patients with systemic lupus erythematosus (SLE) and 44 healthy relatives. All donors have previously been extensively characterized with regard to clinical, serologic, and genetic parameters. Here, peripheral blood lymphocytes and lines were characterized for cell surface antigens. Furthermore, autoantibody production and proliferation rate of the cell lines were monitored. A significant difference between patients and relatives was the lower proliferation rate of EBV-transformed cell lines of the SLE patients. All SLE cell lines are available for interested researches and can be obtained from the European Cell Bank, Salisbury, UK.

    Notes: Abstract Changes in the expression and function of adhesion molecules on the surface of cancer cells are important characteristics in the development of gastrointestinal malignancies and might be used in the future as prognostic factors or as new targets for diagnostic and therapeutic approaches. In esophageal cancer a down-regulation of the E-cadherin receptor and the cytoplasmic protein α-catenin is associated with tumor dedifferentiation, infiltrative growth and lymph-node metastasis. In gastric cancer a reduction of E-cadherin expression due to gene mutations is restricted to diffuse-type tumors while the occurrence of the CD44-standard and the CD44-9v isoform is significantly related to a higher tumor-induced mortality and a shorter survival time. The CD44-6v isoform is predominantly expressed by intestinal-type gastric carcinomas, giving these tumor cells the ability to perform lymph-node metastasis. In pancreatic cancer the expression of integrin adhesion receptors is significantly altered during the malignant transformation while a loss of the E-cadherin receptor can generate dedifferentiation and invasiveness of pancreas carcinoma cells. There is increasing evidence that integrin receptors as well as different isoforms of the CD44 receptor are altered following the malignant transformation of colonic mucosa into adenomas and invasive carcinomas. The expression of the CD44-6v isoform seems to be associated with an adverse prognosis in colorectal cancer due to the development of tumor metastases. A strong correlation has been observed between the expression of the 67-kDa laminin receptor and the degree of differentiation, the invasive phenotype and the metastatic abilities af colorectal cancer cells. Analyzing the expression of the E-cadherin receptor showed that this receptor may serve as an independent prognostic marker in Dukes' stage B colorectal cancer to identify patients with poor prognosis and designate them for intensive adjuvant therapy and clinical observation after curative surgical tumor treatment.

    Description / Table of Contents: Summary Besides a knowledge of material properties geometric properties are needed to investigate the mechanical behavior of the human eardrum. Although the geometry of the eardrum has been examined methodically, its composition has been described in the literature. The true surface geometry of a cadaver specimen of the human eardrum was examined under a laser-scanning microscope. Forty points (85 parameters) depicted the surface of this cadaver specimen and formed the basis for a finite shell model. Furthermore, the coupling with the malleus and its suspension was simulated. Additional geometric and material parameters were chosen from the available literature. Both static behavior and the influence of a parameter variation to modal behavior of the finite element model were tested. ANSYS 5.1 software was used for this study.
    Notes: Zusammenfassung Für die numerische Untersuchung des mechanischen Verhaltens des menschlichen Trommelfells sind neben der Kenntnis der Materialeigenschaften die Geometrieeigenschaften (Gestalt der Oberfläche, Dicke) maßgebend. Die Beschreibung der Geometrie des Trommelfells erfolgte in der Literatur [1, 12, 13, 15, 16] methodisch und inhaltlich unterschiedlich. Hier wurde mit einem Laserscanmikroskop einseitig die reale Geometrie der Oberfläche eines Trommelfellpräparates ausgemessen; 40 Punkte (85 Oberflächenparameter) beschreiben die Oberfläche des Präparates. Diese Oberflächenpunkte waren die Grundlage für die Erstellung eines finiten Schalenmodells des Trommelfells. Es wurde eine Ankopplung des Hammers und dessen Aufhängung über die Hammerbänder und den M. tensor tympani mit Balkenelementen modelliert. Weitere notwendige Material- und Geometriedaten wurden der Literatur [4, 8, 12, 13] entnommen. Das so ermittelte Modell diente der Simulation des statischen Verhaltens und des modalen Verhaltens. Für die Modellierung wurde die Software ANSYS 5.1 (Revision 5.1, Swanson Analysis Systems, Houston, PA) verwendet.

    Description / Table of Contents: Summary A vascular etiology of Parkinson’s disease (PD) has been long debated. In order to search for an ischemic basis of PD we assessed the clinical symptomatology of a consecutive group of 60 PD patients and compaired their frequency of cerebrovascular risk factors, carotid atherosclerosis and ischemic brain lesions with age-matched groups of stroke patients and normals. There were 16 (27%) subjects with PD who also had symptoms of cerebrovascular disease. The frequencies of carotid stenoses, ischemic brain lesions and most of cerebrovascular risk factors seen in the latter group was comparable with those of stroke patients and significantly higher than in the investigational subsets of patients with ”pure” PD and normals. Only one (1.6%) individual with PD presented signs suggestive of an ischemic etiology of parkinsonism. These findings suggest that cerebrovascular disease occurs in approximately one fourth of patients with PD, but seldomly is causally related.
    Notes: Zusammenfassung Eine mögliche vaskuläre Genese des Morbus Parkinson steht immer wieder zur Diskussion. Ziel der vorliegenden Arbeit war die Erhebung einer möglichen vaskulären Ätiologie der Parkinson-Symptomatik bei einer konsekutiven Gruppe von 60 Parkinson-Patienten unter Einbeziehung der klinischen Symptomatik, Verteilung zerebrovaskulärer Risikofaktoren, Präsenz computertomographisch erfaßbarer morphologischer Hirnveränderungen und sonographisch darstellbarer Hirnarterienveränderungen und Vergleich der Ergebnisse mit denen einer altersvergleichbaren Insult- und Normalgruppe. 16 (27%) Parkinson-Patienten wiesen anamnestisch oder klinisch zusätzlich Zeichen einer zerebrovaskulären Insuffizienz auf. Die Frequenz der meisten zerebrovaskulären Risikofaktoren, zerebraler Infarkte und Karotisstenosen in dieser Patientengruppe war vergleichbar mit jener der Insultpatienten und signifikant höher als bei Parkinson-Patienten ohne Zeichen einer zerebrovaskulären Insuffizienz und Normalpersonen. Eine mögliche vaskuläre Ätiologie der Parkinson-Symptomatik konnte nur in einem Fall (1,6%) diskutiert werden. Zusammenfassend fand sich im Rahmen der vorliegenden Studie bei ungefähr einem Viertel der Parkinson-Patienten eine additive zerebrovaskuläre Erkrankung, ein kausaler Zusammenhang mit der Parkinsonsymptomatik kann jedoch als selten angesehen werden.

    Description / Table of Contents: Summary Increased use of gradient echo T2*- weighted gradient echo sequences in magnetic resonance imaging (MRI) of patients suffering from primary ICH called attention to foci of signal loss which were suggested to represent remnants of cerebral microbleeds. In a post mortem correlative MR and histopathological study we provide support for this notion. We found areas of signal loss on gradient echo T2*-weighted sequences in 7 out of 11 brains of patients who had died of intracerebral hematoma. Histopathologically, these areas represented hemosiderin deposits indicating previous extravasation of blood. To provide data about the prevalence of these MRI findings in a healthy elderly population a subgroup of participants of the Austrian Stroke Prevention Study was analyzed. We detected foci of signal loss on gradient echo T2*-weighted sequences in 18 out of 280 volunteers (6,4%). MR-based evidence of previous microbleeds may indicate a potentially higher risk of suffering from intracerebral bleeding which could have therapeutic implications for the treatment of acute stroke and for secondary prevention. This hypothesis will have to be tested in future prospective trials.
    Notes: Zusammenfassung Vermehrte Anwendung T2*-gewichteter Gradienten-Echo-Sequenzen bei Magnetresonanztomographie- (MRT-) Untersuchungen von Patienten nach einem intrazerebralen Hämatom machte auf kleine, hypointense Areale aufmerksam, von denen bisher nur angenommen wurde, daß sie abgelaufene Mikroblutungen darstellen. In einer Post-mortem-Studie mit MRT und vergleichenden histopathologischen Untersuchungen zeigen wir Daten, die diese Hypothese stützen. Bei 7 von 11 Patienten, die an primärem intrazerebralem Hämatom verstorben waren, fanden sich hypointense Areale in T2*-Gradienten-Echo-Sequenzen. Histopathologisch zeigten diese Areale Hämosiderin-Ablagerungen, welche auf abgelaufene Blutungen hinweisen. Um Aussagen über die Prävalenz dieser MRT-Befunde in einem Kollektiv klinisch unauffälliger Probanden mittleren Alters machen zu können, wurden Teilnehmer derÖsterreichischen Schlaganfall-Vorsorge-Studie untersucht. Bei 18 von 280 Probanden (6,4%) fanden sich Signalhypointensitäten in T2*-Gradienten-Echo-Sequenzen. Der MR-tomographische Nachweis abgelaufener Mikroblutungen könnte ein Hinweis auf ein erhöhtes zerebrales Blutungsrisiko sein, was therapeutische Konsequenzen für die primäre Therapie und Sekundärprophylaxe beim Schlaganfall haben könnte. Hierzu sind noch weitere prospektive Studien notwendig.

    Description / Table of Contents: Zusammenfassung Retrospektiv wurden 49 Patienten (Durchschnittsalter 54 Jahre) mit operativ versorgten Sprunggelenkfrakturen durchschnittlich 36 Monate nach der Operation klinisch, radiologisch und mittels dynamischer Ganganalyse nachuntersucht. Obwohl die Mehrzahl der Patienten subjektiv mit dem Operationsergebnis zufrieden war, klagten jedoch mehr als 1/3über intermittierende Schmerzen, und bei etwa der Hälfte fanden sich radiologische Zeichen der Sprunggelenksarthrose. Klinisch war aber bei keinem Patienten eine Gangstörung aufgefallen. In der plantaren Druckverteilung zeigten jedoch Patienten mit und ohne Beschwerden signifikante Belastungsunterschiede. Die Pedographie kann die postoperative Qualitätssicherung um den funktionellen Aspekt erweitern.
    Notes: Summary Forty-nine patients (mean age, 54 years) admitted for displaced ankle fractures were observed retrospectively to determine by clinical examination and measurement of plantar pressure distribution whether successful surgical treatment of ankle fractures had led to gait symmetry. The mean follow-up was 36 months (range, 19–54 months). The deviation in gait was quantified using peak pressure. Using a clinical score, most of the patients had satisfactory results. The plantar pressure distribution showed significant load asymmetries of patients with satisfactory results and those with non-satisfactory results. Dynamic gait analysis allows quantification of gait asymmetry and clinically non-visible gait disorder.

    Notes: Abstract A modern diatom-pH calibration data-set consisting of surface-sediment diatom assemblages from 118 lakes and 530 taxa is presented. The AL:PE data-set is from high-altitude or high-latitude lakes in the Alps, Norway, Svalbard, Kola Peninsula, UK, Slovenia, Slovakia, Poland, Portugal, and Spain (pH range = 4.5-8.0; DOC range = 0.2-3.2 mg l-1). In addition, 92 epilithon samples from 22 high-altitude or high-latitude lakes comprise an AL:PE epilithon diatom-pH data-set. Weighted averaging partial least squares regression is used to develop pH-inference models. The AL:PE data-set has a root-mean-square-error of prediction (RMSEP) of 0.33 and a maximum bias of 0.36 pH units and r2 of 0.82, as assessed by leave-one-out cross-validation. The epilithon data-set has, after data-screening and the deletion of one very obvious outlier, a RMSEP of 0.23 and a maximum bias of 0.18 pH units and r2 of 0.88. The 167 sample SWAP diatom-pH data-set from lowland or upland lakes in the UK, Norway, and Sweden has a RMSEP of 0.29 and a maximum bias of 0.23 pH units and r2 of 0.86. The pH optima, as estimated by weighted averaging and Gaussian regression, are compared for the three data-sets (AL:PE, SWAP, AL:PE epilithon). There is a good correspondence between the AL:PE and the AL:PE epilithon optima, but a consistent bias between the AL:PE and SWAP optima, with the SWAP optima being lower than the AL:PE estimates. The predictive performances of the AL:PE and SWAP calibration data-sets are compared using independent test samples and six core sequences, all from high-altitude lakes, one in south-east Siberia and five in eastern Scotland. The results show the importance of using the AL:PE data-set for inferring lake-water pH from diatom assemblages in high-altitude or high latitude lakes with low DOC concentrations.

    Notes: Abstract  Four paroxysmal nocturnal hemoglobinuria (PNH) patients with severe thrombocytopenia, hemolytic anemia and neutropenia were treated using a combination of filgrastim (G-CSF) and cyclosporin. In all patients a trilineage response of hematopoiesis was achieved. In addition, the proportion of glycosyl-phosphatidylinositol (GPI)-deficient granulocytes decreased. All patients mobilized CD34+ hematopoietic progenitors into peripheral blood after starting treatment with G-CSF. The majority of early progenitors (CD34+CD38–) after mobilization into peripheral blood was found to be unaffected by the GPI-anchoring defect. No patient developed leukemia while under therapy. We conclude from these data that the combination of G-CSF and cyclosporin represents an efficient option for the treatment of hypoplastic PNH.
