Zusammenfamung In der Dekade 1960–1970 warden an der Würzburger Chirurgischen Universitätsklinik 437 Patienten mit Thoraxverletzung behandelt. Leicht verletzt waren 285, schwerverletzt mit Ateminsuffizienz and gleichzeitigen komplikationsträchtigen Verletzungen 152. 1960–1965 waren jährlich nur 1–3, 1966 bereits 7 solche Patienten zu behandeln, seither stieg ihre Zahl von 25 über 28, 33 auf 46 1970. 1967–1970 betrug die Letalitat 32%, sie beruht zum großen Teil auf Kombinationstraumen wie Kopfverletzungen, Wirbelsäulenverletzungen mit und ohne Querschnittslähmung, Beckenfrakturen, Zwerchfellruptur, Aorten- oder Herzverletzung, Tracheal- und Oesophagusruptur. Anfangs warden ateminsuffiziente Patienten mit assistierenden Geräten, zum Schluß fast nur noch kontrolliert beatmet. Die Letalitat betrug bei 48 Patienten 75%. Die Patienten, die innerhalb der ersten 24 Std nach dem Unfall, and jene, die nach dem 5. Tag beatmet werden mußten, hatten ebenso wie Patienten jenseits des 60. Lebensjahres wenig Aussicht zu überleben. Die Todesursache lag zumeist neben den Kombinationsverletzungen in Lungenembolie, Pneumonie oder Sepsis. Von 19 Patienten, die in bedrohlichem Zustand eingewiesen warden, waren nur 3 intubiert and beatmet, die anderen mußten sofort bzw. innerhalb der nächsten Stunden intubiert werden. Nur 7 aus dieser Gruppe haben iiberlebt.

Summary From 1960 to 1970 there were 437 patients with injuries of the thorax treated in the Surgical Clinic of the University of Würzburg. Of these, 285 were slightly injured and 152 were seriously injuredwith respiratory insufficiency and suffered from simultaneous injuries laden with complications. From 1960 to 1965 only one to three such patients a year were treated, in 1966 seven, since then the number was risen from 25 to 28 to 33 a year and finally in 1970 to 46. The lethality amounted to 32% in the years from 1967 to 1970 and is the consequence of a combination of traumas such as head injuries, extended fractures of the extremities, injuries of the abdomen, injuries of the spinal column, fractures of the pelvis, rupture of the diaphragm, injuries of the heart and aorta, and rupture of esophagus and trachea. In the beginning patients with the respiratory insufficiency were respirated with assisting machines, however in the later years nearly all were subjected to controlled respiration. In 48 patients the lethality amounted to 75%. Patients who had to be ventilated within the first 24 h after the accident and those over 60 years old had little chance to survive. The cause of death was due, next to the combination of injuries, to an emboly of the lung, pneumonia or sepsis. Of the 19 patients who were delivered in serious condition only 3 were intubated and respirated at the time of admission, the others had to be intubated either immediately or within the next few hours. Only 7 of this group survived.
