Summary The author describes a classic hemolytic-uremic syndrome in an adult. The patient, who is now 27 years old, had already suffered 4 times from the same syndrome during the previous 10 years. Despite of this, a correct diagnosis was not established. This demonstrates that knowledge of this pathological state, which so far was only well known to the pediatrician, is also important for the internist. In the case being discussed, that even survived the fifth attack of the disease, it appears probable on the basis of nephroangiography and on the basis of the histological findings of a kidney biopsy, that renal damage was not severe enough to produce cortical necrosis and that it was thus possible for the pathological process despite the five attacks to heal itself each time. Marked elevation of antistreptolysin-and antistreptolysin-DPNase-titer during the last attack indicate an initial infection with streptococci, the pathogenetic role of which is discussed in connection with the Sanarelli-Schwartzman-reaction.

Zusammenfassung Es wird über ein klassisches hämolytisch-urämisches Syndrom beim Erwachsenen berichtet. Der 27jährige Patient war in den vorausgegangenen 10 Jahren bereits viermal mit dem gleichen Symptomenkomplex erkrankt, ohne daß er diagnostisch richtig erfaßt worden war. Dies zeigt, daß die Kenntnis des Krankheitsbildes, das bisher nur dem Pädiater geläufig war, auch für den Internisten wichtig ist. Im vorliegenden Fall, in dem sogar die 5. Krankheitsattacke überlebt wurde, machten die Nephroangiographie und der histologische Befund einer Nierenbiopsie wahrscheinlich, daß die Nierenschädigung nicht den Schweregrad einer Rindennekrose erreicht hatte und die Krankheit deshalb trotz fünffacher Manifestation jedesmal ausheilen konnte. Ein starkes Ansteigen von Antistreptolysin- und Anti-Streptolysin-DPNase-Titer während des letzten Krankheitsverlaufs sprechen für einen initialen Streptokokkeninfekt, dessen mögliche pathogenetische Rolle im Rahmen einer Sanarelli-Schwartzman-Reaktion diskutiert wird.
