    Notes: Summary The present study was undertaken to determine how free fatty acid (FFA) release by adipose tissue can occur in the absence of pancreatic glucagon, the most potent lipolytic hormone in the duck. Two possible explanations have successively been investigated. Replacement therapy experiments showed that glucose uptake was implicated in the phenomenon; indeed, when pre-operative insulin and glucose levels were restored after total pancreatectomy, plasma FFA progressively fell to a level significantly lower than the pre-operative value. The same effect was observed when the ducks were kept in a continuous state of hyperglycaemia, whereas “normal” levels of insulin or glucose alone were shown to be ineffective. On the other hand a compensatory mechanism for the lack of glucagon, by other lipolytic agents [with the intestine or the anterior pituitary as possible sources] has been excluded by associating the removal of these two organs with pancreatectomy. These experiments suggest that the lack of pancreatic glucagon induced by total pancreatectomy is essentially masked by a failure in glucose uptake due to the operation.

    Description / Table of Contents: Summary In patients with haemopoietic failure, the low incidence of cells in mitosis often prevents successful chromosome preparations. By performing liquid cultures of myeloid cells a high incidence of mitoses could be obtained even in those cases failing to provide mitoses when the direct preparation technique was used. The cytogenetic studies demonstrated that the mitoses achieved by culture techniques were aneuploid and, thus, there is evidence that they are of leukaemic origin. The combination of short-term liquid culture and chromosome analysis appears to be a diagnostic approach in selected cases of hemopoietic insufficiency.
    Notes: Zusammenfassung Cytogenetische Untersuchungen sind bei Patienten mit Knochenmarkinsuffizienz häufig nicht möglich, da nur wenige oder keine Zellen in Mitose vorhanden sind. Mit Hilfe von Suspensions-Kulturen hämopoetischer Zellen konnte jedoch selbst in denjenigen Fällen eine große Zahl von Mitosen gewonnen werden, bei denen mit der direkten Präparationstechnik keine Mitosen gefunden werden. Durch cytogenetische Untersuchungen konnte nachgewiesen werden, daß die gewonnenen Mitosen aneuploid und somit leukämischen Ursprungs sind. Die Kombination von Kurzzeit-Suspensions-Kulturen und Chromosomenanalyse ist in bestimmten Fällen mit Knochenmarkinsuffzienz von diagnostischer Bedeutung.

    Description / Table of Contents: Summary The monocytes of 7 patients with advanced Hodgkin disease (stages III and IV) and of two patients with generalized lymphosarcoma exhibited a highly significant impairment of the phagocytosis of IgG-coated red cells, regardless of receiving therapy or not. In contrast three patients with M. Hodgkin, stage II B, and one with lymphorsarcoma in complete remission showed a rather elevated monocyte phagocytic activity. The nitroblue tetrazolium reduction by monocytes in the mean was significantly enhanced in all patients investigated, compared with normal persons, although only in one patient a bacterial infection was apparent at the time of the test. The possible implication of the findings in the well known immunodeficiency present in M. Hodgkin and lymphosarcoma is discussed.
    Notes: Zusammenfassung Die Monocyten von 7 Patienten mit fortgeschrittenem M. Hodgkin (Stadium III und IV) und von 2 Patienten mit generalisiertem Lymphosarcom zeigten eine hoch signifikant verminderte Phagocytose von IgG-beladenen Erythrocyten, unabhängig davon, ob eine Therapie bereits durchgeführt war oder nicht. Demgegenüber war die Phagocytose-Aktivität bei 3 Patienten mit M. Hodgkin, Stadium II und einem mit Lymphosarkom in Vollremission eher gegenüber der Norm gesteigert. Die Reduktion von Nitroblau-Tetrazolium durch Monocyten war bei allen untersuchten Patienten im Vergleich zu Normalpersonen im Mittel signifikant gesteigert, obwohl nur ein Patient zum Zeitpunkt der Untersuchung einen gesicherten bakteriellen Infekt aufwies. Die mögliche Bedeutung dieser Befunde für die bei diesen Erkrankungen bestehenden Immundefekte wird diskutiert.

    Description / Table of Contents: Summary Thirty adult patients suffering from acute nonlymphocytic leukemia (ANLL) were treated according to a modified COAP regimen. Vincristine, cyclophosphamide, and prednisone were given by push injection, while cytosine arabinoside was infused over periods of 8 h. Nineteen patients (63%) achieved complete remission. Remission maintenance therapy consisted of 6-mercaptopurine daily and methotrexate twice weekly. Later in the study, COAP consolidation and reinduction was added, which improved the median duration of complete remission from 7 to 24 months. Comparison of the results with the literature shows that the modified COAP regimen is one of the most effective treatment schedules for adult ANLL.
    Notes: Zusammenfassung Dreißig Erwachsene mit akuter, nicht lymphozytärer Leukämie (ANLL) wurden mit einem modifizierten COAP-Schema behandelt. Vincristin, Cyclophosphamid und Prednison wurden i. v. injiziert, während der Antimetabolit Cytosin-Arabinosid in Form 8stündiger Dauerinfusionen appliziert wurde. Neunzehn Patienten (63%) erreichten eine komplette Remission. Die Remissionserhaltung bestand in oraler Gabe von 6-Mercaptopurin täglich und Amethopterin zweimal wöchentlich. Im Laufe der Studie wurde die Erhaltungstherapie durch COAP-Konsolidierung und COAP-Reinduktionsbehandlungen ergänzt. Dadurch konnte die mediane Remissionsdauer von 7 auf 24 Monate verbessert werden. Ein Vergleich dieser Ergebnisse mit der Literatur zeigt, daß das modifizierte COAP-Schema zu den wirkungsvollsten Behandlungsverfahren bei akuter Leukämie im Erwachsenenalter gehört.
