    Notes: Abstract  Quantitative histomorphometric assessment of bone biopsies represents a powerful and informative method for the study of metabolic bone diseases. It is the gold standard against which the noninvasive ”diagnostic” markers of bone metabolism as well as newly available therapeutic modalities are tested. With the rapid progress in technology of molecular biology, identification of systemic and local biomolecules known to regulate bone metabolism can now be achieved. The study of localization, levels of expression, and synthesis of these factors in bone and its microenvironment is possible through applications of in situ hybridization histochemistry (ISHH) and immunohistochemistry (IHC). Application of ISHH allows study of specific mRNA expression. IHC determines the presence and distribution of target protein in cells. These two methodologies provide the link between the cellular processes of mRNA transcription and translation to the working protein. Combining the established bone histomorphometric techniques with ISHH and IHC elevates the study of bone to new heights, i.e., cellular and molecular mechanistic issues can now be studied.

    Notes: Abstract  Bone structure and muscular strength of 30 children with renal disease were investigated by peripheral computed tomography and grip strength. Sixteen children suffered from nephrotic syndrome (NS) and had previously been treated with corticosteroids. Fourteen children suffered from chronic renal failure (CRF) ranging from mild renal failure to end-stage renal disease. Six children had received kidney transplants and corticosteroids for immunosuppression. There was a significant decrease in grip strength of children with NS (SD –0.91± 1.5; P=0.042) and children with CRF (SD –1.38±1.4; P〈0.001) compared with normal children. Furthermore, there was a significant correlation between cortical area and grip strength in all children with renal disease (r=0.92; P〈0.0001). Trabecular bone mineral density did not correlate well with grip strength. These findings resemble results found in healthy children. Trabecular bone mineral density was significantly elevated in children with CRF compared with normal children (SD 1.14±1.4; P=0.008).Grip strength as a marker of muscle mass and cortical area as a marker of bone strength correlate well in children with renal disease, similar to the correlation in healthy children. Grip strength is significantly lower in children with NS and CRF compared with normal children. These data suggest that muscular impairment could be involved in renal osteopathy.

    Description / Table of Contents: Abstract. Tissue engineering offers the possibility to fabricate living substitutes for tissues and organs by combining histogenic cells and biocompatible carrier materials. Pluripotent mesenchymal stem cells are isolated and subcultured ex vivo and then their histogenic differentiation is induced by external factors. The fabrication of bone and cartilage constructs, their combinations and gene therapeutic approaches are demonstrated. Advantages and disadvantages of these methods are described by in vitro and in vitro testing. The proof of histotypical function after implantation in vivo is essential. The use of autologous cells and tissue engineering methods offers the possibility to overcome the disadvantages of classical tissue reconstruction – donor site morbidity of autologous grafts, immunogenicity of allogenic grafts and loosening of alloplastic implants. Furthermore, tissue engineering widens the spectrum of surgical indications in bone and cartilage reconstruction.
    Notes: Zusammenfassung. Tissue engineering ermöglicht die Herstellung lebender Konstrukte zum Gewebeersatz durch die Kombination histogener Zellen und biokompatiblen Trägermaterialien. Pluripotente mesenchymale Stammzellen können isoliert, ex vivo vermehrt und ihre gewebetypische Differenzierung durch Faktoren induziert werden. Es wird die Herstellung von Knochen- und Knorpelkonstrukten, deren Kombination sowie gentherapeutische Ansätze dargestellt. Vor- und Nachteile der Methoden werden durch in vitro und in vivo Testung beschrieben. Essentiell ist der Nachweis der gewebetypischen Funktion der Konstrukte nach Implantation in den Defekt. Die Verwendung autogener Zellen und Methoden des Tissue engineerings bietet die Möglichkeit die Probleme des klassischen Gewebeersatzes – Morbidität der autogenen Spenderstelle, Immunogenität des allogenen Transplantats und Lockerung von alloplastischen Implantaten – zu überwinden und erweitert das Indikationsspektrum in der rekonstruktiven Chirurgie.

    Notes: Abstract Aims/hypothesis. The enteric incretin hormone, glucagon-like peptide-1 (GLP-1), is a potent insulin secretagogue in healthy humans and patients with Type II (non-insulin-dependent) diabetes mellitus. In this study we assessed the impact of short-term GLP-1 infusion on pulsatile insulin secretion in Type II diabetic patients. Methods. Type II diabetic patients (n = 8) were studied in a randomised cross-over design. Plasma insulin concentration time series were obtained during basal conditions and during infusion with saline or GLP-1 (1.2 pmol/l · kg–1· min–1) on 2 separate days. Plasma glucose was clamped at the initial concentration by a variable glucose infusion. Serum insulin concentration time series were evaluated by deconvolution analysis, autocorrelation analysis, spectral analysis and approximate entropy. Results. Serum insulin concentrations increased by approximately 100 % during GLP-1 infusion. Pulsatile insulin secretion was increased as measured by secretory burst mass (19.3 ± 3.8 vs 53.0 ± 10.7 pmol/l/pulse, p = 0.02) and secretory burst amplitude (7.7 ± 1.5 vs 21.1 ± 4.3 pmol/l/min, p = 0.02). A similar increase in basal insulin secretion was observed (3.6 ± 0.9 vs 10.2 ± 2.2 pmol/l/min, p = 0.004) with no changes in the fraction of insulin delivered in pulses (0.50 ± 0.06 vs 0.49 ± 0.02, p = 0.84). Regularity of secretion was unchanged as measured by spectral analysis (normalised spectral power: 5.9 ± 0.6 vs 6.3 ± 0.8, p = 0.86), autocorrelation analysis (autocorrelation coefficient: 0.19 ± 0.04 vs 0.18 ± 0.05, p = 0.66) and the approximate entropy statistic (1.48 ± 0.02 vs 1.51 ± 0.02, p = 0.86). Conclusion/interpretation. Short-term stimulation with GLP-1 jointly increases pulsatile and basal insulin secretion, maintaining but not improving system regularity in Type II diabetic patients.     Notes: Abstract Objective: We analysed the kinetics and effects of glibenclamide (Gb) on glucose, insulin and proinsulin secretion in two ethnic groups (10 in each) of type-2 diabetic patients, one of Caucasian, the other of Chinese origin. Background: Diabetes mellitus type 2 is a global disease affecting all ethnic groups. There are ethnic differences in both the prevalence and metabolic characteristics of the disease. Important interethnic pharmacodynamic and pharmacokinetic differences have been reported for several drugs. With few exceptions, detailed studies on sulphonylurea are lacking. Material and methods: The patients were studied on two occasions when either no Gb (control) or 1.25 mg Gb was administered i.v., immediately before the administration of a 75-g oral glucose tolerance test. Concentrations of insulin and proinsulin were determined by means of radioimmunoassay without cross-reactivities. Gb concentration was determined using high-performance liquid chromatography. Pharmacodynamic results were calculated using net areas under the curves, with basal values set as zero. A P value less than 0.05 was considered significant. Results: When glucose was administered orally without Gb, Chinese patients had higher plasma glucose increases at 10 min (7.6 mmol/l × min vs 2.6 mmol/l × min) and higher increases of plasma insulin levels than Caucasians at both 10 min (198 pmol/l × min vs 54 pmol/l × min) and 30 min (2286 pmol/l × min vs 1198 pmol/l × min). When Gb was administered, the plasma glucose increases were reduced, and the increases of serum insulin and proinsulin levels were greater in both ethnic groups. Compared with the basal values (−1 min), proinsulin/insulin ratios (RPI) were lowest at 10–30 min, followed by an increase. Chinese patients had higher increases of serum insulin levels at 10 min (1109 pmol/l × min vs 550 pmol/l × min) and a lower RPI at 30 min (6.0% vs 7.6%) and 240 min (15.0% vs 21.0%) relative to Caucasians. Serum Gb data were best fitted to a biexponential i.v. model. There were no interethnic differences in any of the pharmacokinetic parameters. Conclusion: In summary, following oral glucose administration without Gb, Chinese type-2 diabetic patients had higher plasma insulin levels but also higher plasma glucose levels during the first 10 min, which might reflect reduced insulin sensitivity or more rapid glucose absorption. Gb augmented glucose-induced release of both insulin and proinsulin in both ethnic groups; the effect on insulin secretion was more pronounced. In conclusion, minor pharmacodynamic but no pharmacokinetic differences were found between the two groups. It seems appropriate to employ the same dosage principles when using Gb in Caucasians and Chinese.

    Notes: Abstract. A rare case of progressive pseudorheumatoid dysplasia (PPD) in a 9-year-old girl is presented. Clinically, chronic painless swollen joints, accompanied by progressive motion restriction and progressive walking difficulties, were found. Radiologically, there was enlargement of the epimetaphyseal portions of the large joints, metacarpal heads, and phalanges, and generalized platyspondyly with irregular delineation of the endplates of the vertebral bodies. The radioclinical features at the peripheral joints were originally misdiagnosed as juvenile rheumatoid arthritis (JRA), and the structural spinal abnormalities were neglected and interpreted as Scheuermann's disease. However, the absence of active inflammatory parameters argues against JRA, whereas the low age of onset of the irregularities at the vertebral endplates is an argument against the diagnosis of Scheuermann's disease. The combination of the dysplastic abnormalities of the spine, with platyspondyly and Scheuermann-like lesions at an unusually low age of onset, and radiological features mimicking JRA of the peripheral joints, is the clue to the diagnosis of this rare autosomal-recessive disease. This case is the first to document the MRI features of PPD of the spine.

    Notes: Abstract A suprasellar arachnoid cyst may cause disorders of growth, puberty and hypothalamic-pituitary function, due to the proximity of the cyst to the hypothalamic-pituitary area. A total of 30 patients (17 boys) with cyst diagnosed at 4.3 ± 1 years were routinely evaluated at 5.4 ± 1 years; 24 of them had one or multiple cyst derivations. Some 23 cases had an abnormal height, weight or puberty: short (〈−2SD, 5 cases) or tall (〉2SD, 10 cases) stature, overweight (body mass index, BMI, 〉2SD, 6 cases), central precocious puberty (10 cases) and/or no progression of pubertal development (3 cases). The growth hormone (GH) peaks after pharmacological stimulation test were low (〈10 μg/l) in 16 patients, confirmed by a second evaluation in 8/11 of them. The plasma free thyroxine was low in five patients, prolactin was high in two and the cortisol and concomitant plasma and urinary osmolalities were normal. BMI was correlated negatively with the GH peaks (r=−0.37, P 〈 0.01) and positively with the plasma leptin concentrations (r=0.55, P 〈 0.01). The plasma fasting insulin concentrations were also correlated negatively with the GH peaks (r=−0.55, P 〈 0.02) and positively with the plasma insulin-like growth factor I concentrations (r=0.64, P 〈 0.002). The adult height (12 cases) was at 4SD in 1 and 〈−2SD in 4 patients, two of whom had precocious puberty untreated with gonadotropin releasing hormone (GnRH) analogue, and two had untreated GH deficiency. The adult height of those treated was normal. One girl had primary amenorrhoea and two boys had low plasma testosterone, despite a normal gonadotropin response to a GnRH test. Conclusion Suprasellar arachnoid cysts may cause deficiencies of growth hormone and thyrotropin, stimulation of the hypothalamic-pituitary-gonadal axis, tall stature and/or overweight. These last two disorders may be due to hyperinsulinism, itself due to suprasellar arachnoid cyst.

    Notes: Abstract In a 1960 paradigm of skeletal physiology, effector cells (chondroblasts, fibroblasts, osteoblasts, osteoclasts, etc.) regulated by nonmechanical agents wholly determined the architecture, strength, and health of bones, joints, fascia, ligaments, and tendons. Biomechanical and tissue-level phenomena had no roles in that paradigm. Subsequent studies and evidence slowly revealed skeletal tissue-level mechanisms and their functions, including biomechanical ones, as well as "game rules" that seem to govern them. That slow discovery process found that effector cells are only parts of tissue-level mechanisms, as kidney cells are only parts of nephrons and wheels are only parts of cars. Normally all those things help to determine skeletal architecture, strength, and health, and adding them to the 1960 paradigm led to the still-evolving Utah paradigm of skeletal physiology that concerns, in part, how load-bearing skeletal organs adapt to the voluntary mechanical loads on them. That caused controversies this article does not try to resolve; instead, it describes some issues they concern. In that regard, controversy can depend on how one assesses the relevance of facts to a problem more than on their accuracy. If a paradigm added new facts to a former one and the new one's advocates viewed all those facts as relevant, but the former's advocates questioned the relevance of some of the new facts, their views about a problem could differ even though each view depended on accurate facts. Readers would make their own judgments about the bearing of those ideas on this article's content.

    Description / Table of Contents: Abstract. Gene therapy in orthopedic surgery is a new technic based on the idea of biological tissue healing. External gene segments are transferred to cells that overexpress growth factors locally to achieve this effect. The influence of growth factors on fracture healing is very well documented in the literature. Experimental data demonstrate that defect healing in bone can be accelerated by the application of different cytokines in vivo. Gene transfer is a promising, new technic of in situ tissue engineering that will enter clinics within the next decade.
    Notes: Zusammenfassung. Gentherapie in der Orthopädischen Chirurgie ist eine neue Technik, die auf der Idee basiert, die biologische Gewebeheilung zu unterstützen. Um dies zu erreichen, werden externe Gensegmente auf Zellen transferiert, die in Folge vermehrt Wachstumsfaktoren im Bereich der Fraktur produzieren. Die Wirkung von Wachstumsfaktoren auf die Frakturheilung ist gut dokumentiert. Die bislang durchgeführten tierexperimentellen Studien demonstrieren eine gute Beeinflussbarkeit der knöchernen Defektheilung. Der Gentransfer kann als eine neue erfolgversprechende Technik des in situ Tissue Engineering angesehen werden, mit deren klinischer Anwendung in der nächsten Dekade gerechnet werden darf.

    Notes: Abstract Transgenic and knockout mice have become valuable experimental systems with which tostudy specific molecular events within the mammary gland of an intact animal. These modelshave provided a wealth of information about the effects of a number of oncogenes and growthfactors. This review focuses on results obtained from the application of transgenic and knockoutmodels to determine the roles of insulin and insulin-like growth factors (IGF)3 in the regulationof mammary gland development, lactation and tumorigenesis. Transgenic models whichoverexpress IGF-I or -II display specific alterations in mammary gland development and an increasedincidence of mammary tumors. Analysis of mammary gland development in knockout micewhich are deficient in IGF-I or the IGF-I receptor supports the conclusion that the IGF systemis important for normal mammary gland development. This review discusses these observationsin detail and attempts to fit them into a larger picture of IGF and insulin action in themammary gland.

    Notes: Abstract The onset of insulin-dependent diabetes mellitus (IDDM) is often associated with the infiltration of pancreatic cells by macrophages. Upon activation, macrophages release nitric oxide (NO) and superoxide (O 2 - ). These species or their reactive intermediates can be cytoxic, mutagenic, or carcinogenic. Previous studies have reported both positive and negative effects of extracellularly generated NO on insulin secretion and viability of pancreatic cells. Inherent problems of several previous studies assessing the effects of NO on insulin secretion include unsteady state NO concentration exposures and the generation of other potentially damaging species. In this study, these problems were eliminated by using a modified experimental system in which NO delivery was achieved via diffusion across a gas-permeable tube and O 2 - delivery was maintained using an enzymatic reaction. The delivery rates were constant, leading to steady state concentrations of NO and O 2 - in the experimental system. Based on reaction kinetics, a model was developed to predict NO, O 2 - and peroxynitrite ONOO- concentrations during the experiment. This study showed that NO, O 2 - and ONOO- at predicted concentrations as high as 2.8 μM, 0.25 μM, and 0.1 nM, respectively, do not affect the insulin secretion rates of βTC3 pancreatic cells over short times. © 2000 Biomedical Engineering Society.     Notes: Abstract A mathematical model is developed to explain the fundamental conundrum as to how during cyclic mechanical loading there can be net solute (e.g., nutrient, tracer) transport in bone via the lacunar-canalicular porosity when there is no net fluid movement in the canaliculi over a loading cycle. Our hypothesis is that the fluid space in an osteocytic lacuna facilitates a nearly instantaneous mixing process of bone fluid that creates a difference in tracer concentration between the inward and outward canalicular flow and thus ensures net tracer transport to the osteocytes during cyclic loading, as has been shown experimentally. The sequential spread of the tracer from the osteonal canal to the lacunae is investigated for an osteon experiencing sinusoidal loading. The fluid pressure in the canaliculi is calculated using poroelasticity theory and the mixing process in the lacunae is then simulated computationally. The tracer concentration in lacunae extending radially from the osteonal canal to the cement line is calculated as a function of the loading frequency, loading magnitude, and number of loading cycles as well as the permeability of the lacunar-canalicular porosity. Our results show that net tracer transport to the lacunae does occur for cyclic loading. Tracer transport is found to increase with higher loading magnitude and higher permeability and to decrease with increasing loading frequency. This work will be helpful in designing experimental studies of tracer movement and bone fluid flow, which will enhance our understanding of bone metabolism as well as bone adaptation. © 2000 Biomedical Engineering Society.     Notes: Abstract When models are used to measure or predict physiological variables and parameters in a given individual, the experiments needed are often complex and costly. A valuable solution for improving their cost effectiveness is represented by population models. A widely used population model in insulin secretion studies is the one proposed by Van Cauter et al. (Diabetes 41:368–377, 1992), which determines the parameters of the two compartment model of C-peptide kinetics in a given individual from the knowledge of his/her age, sex, body surface area, and health condition (i.e., normal, obese, diabetic). This population model was identified from the data of a large training set (more than 200 subjects) via a deterministic approach. This approach, while sound in terms of providing a point estimate of C-peptide kinetic parameters in a given individual, does not provide a measure of their precision. In this paper, by employing the same training set of Van Cauter et al., we show that the identification of the population model into a Bayesian framework (by using Markov chain Monte Carlo) allows, at the individual level, the estimation of point values of the C-peptide kinetic parameters together with their precision. A successful application of the methodology is illustrated in the estimation of C-peptide kinetic parameters of seven subjects (not belonging to the training set used for the identification of the population model) for which reference values were available thanks to an independent identification experiment. © 2000 Biomedical Engineering Society.     Notes: Abstract  Bisphosphonates are now the standard treatment for tumor-induced hypercalcemia (TIH), and pamidronate can normalize serum Ca in at least 90% of the patients treated for the first time. However, there are few data on the treatment of TIH when it recurs, and published results are contradictory. We studied 29 patients with solid tumors, 14 of whom had breast cancer and all of whom were naive to bisphosphonate therapy. They were retreated with pamidronate (median dose 1 mg/kg for both courses) for recurrence of TIH after a median interval of 78 (range 7–297) days. Fourteen of them, 7 of whom had breast cancer, were treated a third time 28 (range 5–79) days after the second course (median dose of pamidronate 1.5 mg/kg). Baseline Ca levels were not significantly different before each course, but the nadirs after each treatment progressively increased, 9.3±0.2 mg/dl, 10.5±0.3 mg/dl, and 12.3±0.4 mg/dl after the 1st, 2nd and 3rd administrations, respectively (P〈0.05). The percentage of treatment failures also progressively increased: 10%, 31% and 85% (P〈0.05). This decreased hypocalcemic effect was essentially observed in patients without bone metastases or with tumors other than breast cancer. Thus, in patients without bone metastases, Ca levels did not decrease at all after the 3rd course, whereas the responses were not significantly different between the three courses in patients with bone metastases. Baseline urinary hydroxyproline, a marker of bone resorption, increased progressively from course to course, especially in patients with bone metastases or breast cancer, but this was not the case for parameters of bone formation. There was also a progressive increase in PTHrP levels accompanied by an increase in the number of patients with enhanced kidney reabsorption of Ca and a decrease in the threshold for Pi excretion, which was significant in patients without bone metastases. In conclusion, pamidronate was progressively less efficient when hypercalcemia recurred. This was observed mainly in patients with hypercalcemia of humoral origin. Tumor progression is accompanied by an enhanced release of osteolytic factors, notably PTHrP, that increase bone resorption and enhance kidney calcium reabsorption, especially in patients without bone metastases. When both phenomena occur, the response to bisphosphonates becomes minimal and the usefulness of therapy questionable.

    Notes: Abstract  An ochronotic femoral head has been studied morphologically under the light and the electron microscope. Its articular cartilage showed the alterations already reported in the literature, mainly consisting of erosions of the surface, pigment accumulation in chondrocytes and intercellular matrix, chondrocyte degeneration, the formation of pigmented, calcified and uncalcified microshards, and the presence of granulation tissue with macrophagic cells. The changes in bone were less severe than those in cartilage. Pigment was present in the calcified matrix. This did not seem to disturb the organization of the bone tissue, although it was diffusely osteoporotic, perhaps because of limb disuse. The preservation of calcified matrix might depend on the fact that its collagen fibrils are encrusted by mineral substance, which avoids the dangerous effects that the deposition of ochronotic pigment induces in the fibrils of soft connective tissues. On the other hand, the newly formed osteoid matrix remains uncalcified for too short a time to be modified by the pigment. Diffuse or granular pigmentation was found in a few osteocytes, while several of them were condensed or reduced to cellular fragments. Bone resorption often occurred near these osteocytes. However, this did not seem to alter the degree of bone remodelling, possibly because of the relatively low numbers of degenerated or dead osteocytes. Pigment was also contained in the cytoplasmic vacuoles of otherwise active osteoclasts, whereas it was not found in osteoblasts. On the whole, ochronosis in bone seems to induce the same changes as in other connective tissues. However, their severity appears to be limited by calcification, which prevents modifications in collagen fibrils, and by bone remodelling, which to some extent eliminates the oldest, pigment-richest parts of the tissue.

    Notes: Abstract The rate of muscle glycogen synthesis during 2 and 4 h of recovery after depletion by exercise was studied using two energy equivalent carbohydrate drinks, one containing a polyglucoside with a mean molecular mass of 500 000–700 000 (C drink), and one containing monomers and oligomers of glucose with a mean molecular mass of approximately 500 (G drink). The osmolality was 84 and 350 mosmol · l−1, respectively. A group of 13 healthy well-trained men ingested the drinks after glycogen depleting exercise, one drink at each test occasion. The total amount of carbohydrates consumed was 300 g (4.2 g · kg−1) body mass given as 75 g in 500 ml water immediately after exercise and again 30, 60 ad 90-min post exercise. Blood glucose and insulin concentrations were recorded at rest and every 30 min throughout the 4-h recovery period. Muscle biopsies were obtained at the end of exercise and after 2 and 4 h of recovery. Mean muscle glycogen contents after exercise were 52.9 (SD 27.4) mmol glycosyl units · kg−1 (dry mass) in the C group and 58.3 (SD 35.4) mmol glycosyl units · kg−1 (dry mass) in the G group. Mean glycogen synthesis rate was significantly higher during the initial 2 h for the C drink compared to the G drink: 50.2 (SD 13.7) mmol · kg−1 (dry mass) · h−1 in the C group and 29.9 (SD 12.5) mmol · kg−1 (dry mass) · h−1 in the G group. During the last 2 h the mean synthesis rate was 18.8 (SD 33.3) and 23.3 (SD 22.4) mmol · kg−1 (dry mass) · h−1 in the C and G group, respectively (n.s.). Mean blood glucose and insulin concentrations did not differ between the two drinks. Our data indicted that the osmolality of the carbohydrate drink may influence the rate of resynthesis of glycogen in muscle after its depletion by exercise.

    Notes: Abstract We performed a back-scattered electron microscopy analysis of the interface between newly formed bone and hydroxyapatite coating, in an experimental rabbit model. Twenty cylinders made of Ti6A14V and coated with hydroxyapatite at different crystallinity were implanted in the distal femural canal and retrieved at 4, 8, 26 an 34 weeks. Crystallinity of the coating varied from 90% to 60% and thickness varied between 50 and 100 μm. Osteocytes were detectable a few micrometers in proximity of the coating. They produced new bone which was so tightly apposed to the coating that high magnification BSEM did not resolve any discontinuity at the interface. This was not observed in uncoated implants. Degradation of the hydroxyapatite coating is not a simple hydrolytic process because newly formed bone is remodelled in areas were a tight apposition with hydroxyapatite is present. The coatint itself is likely to be attacked by the resorptive action of multinucleated giant cells and osteoclasts. In conclusion, response to coated samples is morphologically characterized by tight apposition with bone. The substitution of areas of the coating by newly formed bone is possible.

    Notes: Abstract Serum activity of bone specific alkaline phosphatase, a product of differentiated osteoblasts, is thought to mirror fracture healing. The precise time course after various conditions involving bone healing is, however, poorly described. The aim of our study was to evaluate sequential changes of bone alkaline phosphatase over a period of 20 weeks in traumatized patients with and without bone injuries. The bone isoenzyme of alkaline phosphatase was determined in frozen serum samples using a new automated procedure based on the wheat germ lectin precipitation method. Patients were stratified into different groups: 1. ST – soft tissue injury without bone participation; 2. DF – diaphysial fracture of tibia or femur; 3. MT – patients suffering from multiple traumas; 4. PF – proximal femur fracture, treated with dynamic hip screw; and 5. EP – femur neck fracture, treated with cemented endoprosthesis. Similar values were obtained in all measured groups on the day of admission (ST 35.83±6.15 U/l; PF 27.37±4.43 U/l; EP 31.09±7.42 U/l). In the following measurements, enzyme activity decreased significantly in all groups to reach a nadir within the first week, ranging between 41 and 82% of the activity immediatly postoperative. Thereafter, a substantial increase occurred in all groups investigated. In the ST group, this increase led to an activity level that was comparable to the first posttraumatic value (39.06±7.81 U/l). In contrast, average values in all other groups revealed a further increase, which was significantly elevated compared to measurements taken the first day after trauma (DF 74.36±10.84 U/l; MT 177.87±30.0 U/l; PF 93.39±22.08 U/l; EP 51,52±7.33 U/l). Additionally, the time to reach the peak in enzyme activity differed between groups. In the MT group, it was observed as early as 3 weeks after injury, in the DF group the peak was reached as late as 6 weeks after trauma. The results of the study indicate that bone alkaline phophatase activity undergoes a specific pattern of changes after trauma. It may be assumed that the initial decrease is part of a general stress response to trauma and operation. The subsequent increase seems to depend from the magnitude of bone repair and the type of fracture healing, e.g. diaphysial fracture healing takes longer time than cancellous bone healing. These results seem to be a useful basis in order to establish a laboratory monitoring of fracture healing in subsequent studies.

    Notes: Abstract The expression of mRNAs of creatine kinase (CK)-B and CK-M has been show to be affected by insulin, and myocardial CK-MB activity is suppressed in insulin-deficient rats. We investigated the dose-related effect of insulin on CK-MB activity in cardiac and skeletal muscles. Male Wistar rats were divided into three groups: (1) control group, (2) diabetic group, injected with 65 mg/kg streptozotocin for 4 weeks, and (3) atenolol group, administered 30 mg/kg per day atenolol. Each group was further divided into three subgroups and administered either saline, or 20 (Ins 20) or 30 (Ins 30) U/kg per day insulin. After 3 weeks, the isoenzyme activity of CK and lactate dehydrogenase (LDH) in the left ventricle of the heart (LV) and the major pectoral muscle (PM) was measured. Serum insulin increased and plasma glucose decreased in Ins 20 and Ins 30, dose-dependently, in all three groups. Both CK-MB and -BB activity in LV increased dose-dependently with insulin treatment in the control, diabetes, and atenolol groups, although these changes did not occur in skeletal muscles. CK-MB in LV correlated with the serum insulin levels in all rats, while no correlation was found in the skeletal muscles. These findings suggest that insulin possibly regulates the distribution of CK isoenzymes in rat heart muscle, and that the effect of insulin is not due to the sympathetic drive induced by hypoglycemia.

    Notes: Abstract To investigate the proximate influence of a changing food availability on the seasonal fattening of migratory birds, garden warblers (Sylvia borin) following postnuptial moult were food restricted once a week. Body mass, food intake, plasma hormone and metabolite levels were measured and compared to birds which always had ad libitum food access. The food-restricted birds increased their body mass significantly earlier than the controls. The accelerated fattening was initially not accompanied by hyperphagia and may be due to either an increased food utilisation efficiency or a reduced metabolic rate. An increase of basal glucagon and corticosterone and a decrease of insulin levels prior to fattening were not significant, however, they resulted in a significant decrease of the insulin:glucagon ratio. This ratio was also lower in food-restricted birds than in control birds and may account for the difference in the fattening progress. We conclude that seasonal fattening may be stimulated by a catabolic impulse which could be imposed in free-living birds by a decrease of food availability and/or by an increase of energy expenditure. A negative energy balance is hypothesised to be a common proximate factor affecting migratory as well as winter fattening.

    Notes: Abstract  Many of the present generation biomaterials are still based upon the early concept that implantable materials should be bioinert and therefore designed to evoke minimal tissue response, if none. However, a growing body of clinical data demonstrates that the long survivability of these materials is hampered by high rates of failure, which is primarily attributed to interfacial instability. It has therefore become understood that this approach is not optimal. Modern approaches implicate the use of biomaterials that can actively interact with tissues and induce their intrinsic repair and regenerative potential. This involves control over the cell cycle, the molecular framework that controls cell proliferation and differentiation. Class A bioactive glass-ceramic materials were the first materials shown to endorse these properties and, depending upon the rate of resorption and release of ions, can create chemical gradients with specific biological actions over cells and tissues. Optimising this bioactive regenerative capacity of Bioactive glass-ceramics offers great hope for producing biomaterials that can stimulate growth, repair, and regeneration of any human tissue.

    Notes: Abstract  In order to study commercially pure (CP) titanium bone interaction and integration mechanism, titanium implant bone interface formed for 1, 3 and 6 months was examined and analyzed by advanced TOF–SIMS analysis. The results obtained show: 1) Titanium and bone tissue integrated closely; 2) The action between CP titanium and bone tissue is a reactive process; both physical and chemical integration occur at the titanium–bone interface; 3) Titanium diffuses into the bone tissue, though the diffusion density is limited; 4) The diffusion area of titanium into bone tissue noted during the follow-up period is up to 100 μm. In this paper the titanium bone integration mechanism was studied, both at molecular and anatomic level.

    Notes: Abstract  This study is part of a series of studies evaluating self-reinforced polyglycolide (SR-PGA) membranes. The aim of this study was to evaluate the effect of SR-PGA membrane on osteogenesis in order to assess its use in tissue engineering and bone regeneration. SR-PGA membranes (15×20 mm) were implanted over the femoral diaphyseal bone of 27 Wistar rats (over the periosteum). Each membrane was stabilized using a Dexon suture cerclage. Rats were followed-up for 1, 2, 3, 6, 8, 15 and 30 weeks. Histology and microradiography were used to evaluate bone formation. The membranes were excised and tested for their tensile strength properties, the results of which are published in a separate report. Bone formation periosteally was seen in 21/27 cases (77.8%, confidence interval 57.7–91.4). It occurred in all cases at 1, 2 and 3 weeks. At 6 weeks, it was seen in three out of four cases but at 8 weeks, only in one out of three cases. Bone formation seen as thickened cortex was detected at 15 weeks in all of the three cases and at 30 weeks in three out of six cases. Hence, bone formation was seen in most of the cases when SR-PGA membranes were applied around rats’ femora. They can be recommended for use in bone tissue engineering and regeneration.

    Notes: Abstract  Objective. To determine whether the load-bearing capacity of human proximal femora with metastatic defects can be predicted from the bone mineral content. Design. The bone mineral content (BMC) of the total proximal femur was measured by dual-energy X-ray absorptiometry (DXA). The femurs were loaded so as to simulate stair ascent or external rotation. Patients. Simulated lytic defects were created using specialized cutting tools in the intertrochanteric region of 32 human cadaveric femora. Bone density measurements were made before and after creating defects. Results. A linear relation could be used to predict failure load from BMC or bone mineral density. The slope of the linear relation was greater for loads representing external rotation than for loads representing stair ascent. The linear relations suggest that the BMC measurements account for both the density of the host bone and the amount of bone removed by the defect. Conclusions. The data suggest that between 70% and 80% of the variation in failure load of human femora with lytic metastatic defects can be predicted from the BMC and that relations between BMC and failure load are sensitive to the type of loading. Combined with information on the loads associated with the activities of daily living, these data may be used to help identify patients in whom prophylactic stabilization might prevent a pathologic fracture.

    Notes: Abstract  Functional heterogeneity among pancreatic beta cells is a characteristic feature of the islets of Langerhans. Under physiological conditions, beta cells in the pancreas of fed rats exhibited heterogeneous immunohistochemical staining for insulin and glucokinase. Intracellular beta cell glucokinase staining was either faint or dense. In the pericapillary space beta cell glucokinase immunoreactivity had a polar orientation, with the highest density in cytoplasmic regions close to the blood vessels. Starvation resulted in a loss of heterogeneity with homogeneous insulin staining in all beta cells of the islets, and this was accompanied by a loss of heterogeneous glucokinase staining. The intracellular polarity of glucokinase staining in contact to blood vessels also disappeared after starvation. Refeeding resulted in the reappearance of intercellular heterogeneity. In dependence on the functional demand, the endocrine pancreas recruited insulin from beta cells according to a well-defined hierarchy, with an initial preferential mobilization of medullary beta cells. In the course of this process intracellular polarity of glucokinase staining reappeared in areas of the beta cell with functional contact to the GLUT2 glucose transporter in the plasma membrane. This can be regarded as the morphological correlate of an activation of the glucose signal recognition apparatus. Interestingly, the study also provides evidence that the changes in glucokinase distribution apparently preceded those in insulin distribution, which is in keeping with the central role of glucokinase as the glucose sensor of the pancreatic beta cell.

    Notes: Abstract Advanced glycation endproducts have been implicated in the development of diabetic complications. In addition, these products could also mediate certain bone alterations such as diabetic osteopenia. Several receptors specific for advanced glycation endproduct-modified proteins have been characterized in different cell types, contributing to the recognition and degradation of senescent proteins. In the present report, we investigated the possible presence of advanced glycation endproduct-binding proteins on osteoblast-like cells. Both UMR106 and MC3T3E1 cell lines express specific advanced glycation endproduct-binding sites, with an affinity constant between 0.4 and 1.7 · 106 M−1, depending on the stage of osteoblastic differentiation; and with a receptor capacity of 1.5–2.0 · 107 sites/cell. Osteoblast-like cells were also found to participate both in the uptake and degradation of advanced glycation endproduct-modified bovine serum albumin at 37°C. Radiolabelled ligand blotting studies confirmed the presence of several membrane binding proteins, with apparent molecular masses of 50, 45–40, 30, 25 and 18 kDa; the major bands corresponded to 30 and 25 kDa proteins. This study provides evidence of the presence of advanced glycation endproduct-specific binding sites, and for their regulation with the stage of differentiation, in two osteoblast-like cells in culture.

    Notes: Abstract In this prospective study we aimed to compare insulin plus acarbose with insulin plus gliclazide with respect to their effect on insulin requirement, lipid profiles and body mass index (BMI) while achieving good glycemic control. Forty patients with type 2 diabetes mellitus who were on conventional insulin therapy (subcutaneous insulin therapy consisting of regular and NPH insulin, two times a day) were included in the study. They were randomized to double blind treatment with insulin in combination with gliclazide or acarbose for 6 months. For both groups, acceptable glycemic control was achieved at the end of study period. The mean HbA1c levels decreased from 8.32±0.26 to 7.13±0.18% in acarbose group and 8.6±0.15 to 7.48±0.21% in the gliclazide group. The difference between groups was not significant (P 0.29). In the acarbose group, total cholesterol and LDL concentration decreased significantly while other parameters did not change. In the gliclazide group, HDL levels decreased significantly from 46.6±2.48 mg/dl to 41.3±2.09 mg/dl (P 0.001) BMI increased significantly from 27.60±1.21 kg/m2 to 28.69±1.26 kg/m2. (P 0.003) Total daily insulin dose was not changed in the acarbose group significantly, but increased from 42.6±2.73 to 49.27±3.58 U/day, which was significant in gliclazide group of (P 0.016). In the acarbose group, there were no significant differences between responders and nonresponders with respect to fasting and stimulated C-peptide, HbA1c levels and baseline BMI values. But in the gliclazide group, baseline BMI values were significantly higher in the nonresponding group compared to responders (P 0.02). In conclusion, combination of insulin with acarbose can be a good alternative for type 2 diabetic patients on insulin therapy; seems more beneficial than combination with gliclazide; may have advantage of achieving good glycemic control without increasing insulin dose and BMI; also may have the advantage of providing a decrease in LDL level, which are all important to prevent atherosclerosis.

    Notes: Abstract Cryopreservation is the only available technique for long-term storage of pancreatic islets. The freezing/thawing protocol may cause considerable loss of viable islet tissue and impair its function in vivo. The aim of this study was to investigate glucose and insulin levels after transplantation of fresh and cryo/thawed rat islets. Rat pancreatic islets were isolated following intraductal collagenase injection and Ficoll gradient purification. After isolation, islets were cultured for 24 h and then either transplanted or frozen after stepwise addition of DMSO according to Rajotte et al. and stored in liquid nitrogen. After rapid thawing islets were stepwise transferred into RPMI medium and cultured for another 24 h. The recipients were athymic mice with streptozotocine-induced diabetes. Two hundred fresh (n=13) or cryo/thawed (n=15) islets were transplanted beneath the renal capsule. Glucose levels were measured for 14 days and blood samples for insulin determination were obtained 15 min after i.p. glucagon (10 mg/kg) administration on day 14. Glucose levels were normalized (〈9 mmol/l) in all recipients within 3 days since transplantation. On day 14, mean fasting values±SE in fresh and cryo/thawed islet groups were 4.0±0.6 and 4.4±0.4 mmol/l, respectively (P〉0.05). Fasting insulin levels were higher in the cryo/thaw than in the fresh islet group (1.67±0.33 vs 0.57±0.13 ng/ml; P〈0.01). Post-glucagon levels did not differ significantly (1.45±0.24 vs 0.86±0.24 ng/ml; P=0.06). While glucagon significantly increased insulin levels (P〈0.01) in the fresh islet group, no change in insulin levels was observed (P〉0.05) in the cryo/thaw group. Immunohistochemical staining demonstrated fragmentation of viable islet tissue which was more apparent in the cryo/thaw group. We conclude that in a short-term study cryo/thawed rat islets produce higher insulin levels than fresh islets transplanted into nude mice. This may be due to better islet survival or loss of feed-back regulation.

    Notes: Abstract  A transgene reporter consisting of the bombyxin gene promoter and the green fluorescent protein coding region was introduced into intact brains of the silkworm Bombyx mori by in vitro electroporation. After in vitro culture of the brains, the fluorescence derived from the introduced reporter gene was observed in all cases in eight neurosecretory cells that had previously been identified as bombyxin-producing cells (BPCs). Although the fluorescence was not always observed in all cells, it was specific to BPCs, indicating that the reporter was under the control of the bombyxin gene promoter in a BPC-specific manner. Electroporatical introduction of a reporter gene was therefore found to be a suitable method for analyzing cell-specific expression in intact tissues and to be substitute for germ-line transmission of reporters in the transgenic system. Application of this technique enables us to analyze the cell-specific expression of transgene reporters within a few days and treat more than several dozens of the reporters within 1 month, which is difficult to do with the transgenic system.

Méthode: Les dossiers cliniques de 27 patients ayant subi une scintigraphie de l'os, ont été étudiés afin d'analyser les plaintes et la façon de décider le diagnostic. Ces patients avaient des symptômes persistants après l'arthroplastie totale du genou. En plus ont été étudiées les radiographies conventionelles et les scanographies isotopes, sans connaître les dossiers cliniques. Sur la base de ces données nous avons évalué les cas dans lesquels la scanographie isotope du genou a contribué de façon décisive au traitement future. Résultats: La scanographie isotope ne contribue pas à la planification du traitement, si l'examen physique ou les radiographies conventionelles n'ont pas apporté de diagnostic clinique. Il s'agissait ici de 37 % des patients de cette série. Dans les cas où le diagnostic clinique était fait, la scanographie isotope a influencé la décision de diagnostic de 65 % des patients. Cependant, pour les autres patients avec diagnostic clinique (35 %), ce ne fut pas le cas. Conclusion : La répresentation radionucléide est de valeur limitée dans le diagnostic d'une arthroplastie symptomatique totale du genou. Elle peut supporter ou rejeter un diagnostic clinique mais entraînera rarement un changement de traitement, dans l'absence de constatations anormales pendant les examens physiques ou radiologiques.
    Notes: Summary Objective: Determination of the value of bone scanning in symptomatic total knee arthroplasties. Setting Academic Hospital Maastricht and Maasland Hospital Sittard. Design: Retrospective clinical investigation. Method: The clinical reports of 27 patients, who underwent a bone scintigraphic study, were investigated for complaints and analysis of diagnostic decision making. These patients had persistent symptoms after total knee arthroplasty. In addition, conventional radiographs and the isotope scans were studied without knowledge of the clinical records. Based on these data we evaluated in which cases isotope scanning of the knee had contributed decisively to the further treatment. Results: Isotope scanning did not contribute to the treatment planning if physical examination or conventional radiographs had not led to a clinical diagnosis. This comprised 37% of the patients in our series. If a clinical diagnosis was present, isotope scanning did influence decision making in 65% of the patients. However in the remaining patients with a clinical diagnosis (35%) this was not the case. Conclusion: Radionuclide imaging is of limited value in the diagnostic process of symptomatic total knee arthroplasty. It may support or reject a clinical diagnosis, but in the absence of abnormal findings at physical examinations or radiologic studies, it seldom will induce a change in treatment

    Notes: Summary Biocompatible calcium phosphate ceramics has been used for several years in orthopeadic surgery. We have been using two new synthetic biphasic calcium phosphate ceramics (BCP) since September 1996 for bone defect filling in any orthopaedic or trauma operation where autograft use was not possible or even wanted. The first, Eurocer 400® has 300 to 500 micron wide macropores with a totally interconnected porosity. This salt seed like product can be used in bone defect filling, when solidity is not a major concern. The second, Eurocer 200® has not totally interconnected 200 micron large pores. Its main characteristic is a mechanical resistance up to 30 Mpa. We use it in any case of weight-bearing surgery. Different sizes and presentation forms are available and will be chosen according to the recipient site shape. We report one hundred and fifty cases with a six to thirty month follow-up. In one third of the patients hip revision surgery was addressed. Another third concerned recent trauma or sequelae cases,.whereas the last third involved cold orthopaedic procedures. General principles are the need of a living and non-infected site after thorough debridement if necessary. Osteocompatibility of calcium phosphate ceramic is confirmed by our results. We report no mechanical failure. In all cases X-rays show a progressive integration, with new bone formation. Our substitutes have been histologically studied in nine cases, four to fifteen months after implantation. New bone formation around and in the substitute is impressive. Indeed, their good mechanical properties without loss of biological quality is the most relevant feature of these BCPs, leading to a wider indication field; therefore we have now abandoned the use of any bony auto, allo or xenograft.

    Notes: Abstract  In the present work, we examined the effects of in vivo administration of insulin to rats made hypokalaemic by feeding a K+-free diet. The i.p. injection of insulin in the hypokalaemic rats provoked muscle paralysis within 3–5 h. Consistent with this observation, the skeletal muscle fibres of the paralysed rats were depolarized. In contrast, in the normokalaemic animals, insulin neither provoked paralysis nor produced significant fibre hyperpolarization. In the hypokalaemic rats, insulin almost completely abolished the sarcolemma adenosine triphosphate (ATP)-sensitive K+ currents without altering the sensitivity of the channels to ATP or glibenclamide. In contrast, in the normokalaemic rats, insulin enhanced ATP-sensitive K+ currents that became also resistant to ATP and glibenclamide. Our experiments indicate that the modulation of the sarcolemma ATP-sensitive K+ channels by insulin is impaired in the hypokalaemic state. This phenomenon appears to be related to the fibre depolarization and paralysis observed in the same animals.

    Notes: Abstract Insulin has been suggested to prevent the induction of nitric oxide synthase (NOS) in vitro in arterial smooth muscle, but whether such a mechanism is operative in vivo is not known. Therefore, we evaluated the sensitivity of smooth muscle NOS to induction by interleukin-1β (IL-1β) in aortic rings of lean and obese Zucker rats, a model of experimental hyperinsulinaemia. In order to modulate the insulin and glucose balance of the rats, a 22-week-long treadmill exercise was included in the study. The training attenuated weight gain and reduced blood glucose in the obese and lean rats, whereas the abnormally high plasma insulin of the obese rats remained unaffected. A 6-h incubation of aortic rings with IL-1β (10 ng/ml) increased cyclic GMP in smooth muscle by approximately threefold in all groups, and this effect was prevented by methylene blue. The contractile sensitivity of endothelium-denuded aortic rings to phenylephrine was reduced by incubation with IL-1β (1 ng/ml and 10 ng/ml) in the exercised obese and lean rats, whereas no significant change was observed in the sedentary groups. The aortic maximal contractile force induced by phenylephrine was reduced in sedentary and exercised obese rats by incubation with IL-1β, while no change was detected in the lean rats. The aortic relaxation to exogenous L-arginine was augmented by IL-1β in all groups, while the relaxation sensitivity to L-arginine after induction by IL-1β was enhanced by exercise in the obese but not in the lean rats. Finally, the relaxation to nitroprusside was not significantly affected by IL-1β in any of the study groups. In conclusion, since maximal contractile force generation to phenylephrine was reduced by IL-1β in the obese but not in the lean rats, the sensitivity of NOS to induction by IL-1β was higher in arterial smooth muscle of the obese than the lean Zucker rats. Thus, this model of hyperinsulinaemia was not associated with reduced sensitivity of smooth muscle NOS to induction by IL-1β. Regular exercise did not change plasma insulin concentrations, but it enhanced the action of insulin in both strains as reflected by reduced blood glucose, and increased the sensitivity of smooth muscle NOS to induction by IL-1β.

    Notes: Abstract PURPOSE: This report presents a patient who developed severe buttock pain because of an ossified intramuscular metastasis from a sigmoid colon cancer. METHODS: This is a case report and review of the literature for intramuscular metastasis from colon cancer. RESULTS: Computed tomography and magnetic resonance imaging showed a soft-tissue mass with heavy calcification. Histologically, mature compact bone was observed with adenocarcinoma cells dispersed among the bony trabeculae. CONCLUSION: When an intramuscular mass is seen, even if it contains extensive calcification, metastasis from colon cancer should be included in differential diagnosis.

    Notes: Abstract Embolism of bone marrow to the lungs is a quite frequent finding after trauma but transport and deposition of solid bone is rarely seen, which may simply be because pulmonary calcifications are not recognized as bone fragments. We report on three patients with embolism of bone spicules to small lung arteries of about 0.5 mm in diameter which were plentiful in two of the patients on postmortem examination. However, the true nature of the emboli was only recognizable after decalcification of lung tissues. It appears that trauma occurring in a septic bone lesion has the greatest chance to provoke bone embolism. The bone spicules do not usually occlude vessel lumina and thus do not severely disturb the blood circulation in the lungs. The bone fragments become covered by endothelium and can remain recognizable for months or even years.

    Description / Table of Contents: Early radiation effects on human bone may lead to osteoradionecrosis (ORN). Direct bone cellular lesions [28] as well as fibrous degeneration of blood vessels [21] are considered to be pathologically relevant. Only few data on initial subclinical radiation effects are available. Patients were grouped according to the dose of radiation and clinical findings. Group 1: sound human bone of lower jaw, mostly collected during orthodontic surgery (n = 10 patients); group 2: specimens of lower jaw from patients with ORN (n = 12 patients); group 3: specimens of lower jaw from patients with head and neck cancer who were preoperatively treated with 36 Gy radiation; group 4: specimens of lower jaw from patients with head and neck cancer (n = 9) who were treated with 60–70 Gy radiation. Specimens were studied by confocal laser scanning microscopy (CLSM), by conventional light microscopy (DL) and by flourescence darkfield microscopy (DFM) after bisbenzimide staining (H 33258) of the viable cellular nuclei. For the correlating study of identical areas in CLSM and DL the specimens were prepared according to the sawing and grinding technique [8]. All the radiated bony specimens, regardless of the dose of radiation, showed areas of extensive or total loss of vitality of the osteocytes. This finding was also evident after 36 Gy and a short interval between radiation and sample collection (group 3). Additionally, in CLSM micromorphologic lesions of the lamellate structure were seen. With these results we can confirm the loss of vitality of the osteocytes as an initial radiation effect as described earlier [10, 23, 28]. In addition to these findings, alteration of the lamellate microstructure was found in the early phase after radiation. The functional and mechanical significance of these findings should be the subject of further studies.
    Notes: Für die infizierte Osteoradionekrose (IORN) des Kiefers werden unterschiedliche pathogenetische Mechanismen initialer Strahlenfolgen im Knochen diskutiert: Wichtige Parameter sind dabei die direkte Zellschädigung der Osteozyten [28] und die Gefäßfibrosierung [21]. Bislang liegen wenige Kenntnisse zum Ausmaß der initialen, subklinischen Veränderungen vor. 4 Probengruppen wurden anhand verschiedener Patientenkollektive definiert: Gruppe 1: gesunde Unterkieferproben meist von orthodontischen Operationen (n = 10 Patienten); Gruppe 2: Unterkieferproben mit manifester IORN (n = 12 Patienten); Grruppe 3: Unterkieferproben nach 36 Gy präoperativer Radio(chemo)therapie (n = 9 Patienten); Gruppe 4: Unterkieferproben nach 60–70 Gy Bestrahlung ohne Hinweise auf IORN (n = 9 Patienten). Alle Proben wurden histotomographisch mit konfokaler Laser-Scanning-Mikroskopie (CLSM), durchlichtmikroskopisch (DL) und dunkelfeldmikroskopisch (DFM) nach Fluoreszenzmarkierung des intakten Zellkerns (Bisbenzimid H 33258) untersucht. Korrelierende Histologien identischer Probenareale erfolgten an Trenndünnschliffen [8]. Als Ergebnisse wurden gefunden: 1. Alle unterschiedlich bestrahlte Knochenproben zeigten Areale mit weitgehendem bis vollständigem Verlust vitaler Osteozyten. Diese Strahlenfolge trat auch schon nach 36 Gy und kurzem Intervall bis zur Operation (Gruppe 3) auf. 2. Im CLSM zeigten sich zusätzlich mikromorphologische Veränderungen der lamellären Struktur, die zwischen den Probengruppen progredient waren. Hieraus können folgende Schlußfolgerungen gezogen werden: 1. Der vorbeschriebene Vitalitätsverlust der Osteozyten [10, 23, 28] konnte als sehr frühe, initiale, radiogene Läsion bestätigt werden. 2. Zusätzlich zeigten sich Veränderungen der lamellären Mikroarchitektur des Knochens als frühe und im Gruppenvergleich progrediente Strahlenläsion. Die mikromechanisch funktionelle Bedeutung dieser Veränderung muß in weiterführenden Untersuchungen eruiert werden.

    Notes: Abstract  The role of membrane traffic in the stimulation of apical Na+ permeability caused by increases in cytoplasmic cyclic AMP was assessed by measuring the effects of forskolin on transepithelial capacitance (C T), transepithelial conductance (G T), and short-circuit current (I sc) in A6 cultured toad kidney cells. Apical water permeability was probed by recording cell volume changes after reducing the osmolality of the apical bath. We found that forskolin does not increase the osmotic water permeability of the apical membrane of A6 cells, and thus does not stimulate the insertion of water channels. Comparison of the effects of forskolin and insulin on Na+ transport demonstrated that both agents produce reversible increases in C T, G T and I sc. G T and C T increased proportionally during the rising phase of the insulin response. However, a non-linear relationship between both parameters was recorded when forskolin was given in NaCl Ringer’s solution. The relationship between C T and G T became linear after the effects of forskolin on Cl– conductances were eliminated by substituting Cl– by an impermeant anion. In contrast, in Cl–-containing Na+-free solutions, the non-linearity became more pronounced. Successive additions of insulin and forskolin caused additive increases in C T. Because increases in C T and Na+ transport occurred in the absence of stimulation of water permeability and increases of C T and G T were directly proportional when Na+ was the major permeating ion across the apical membrane, we suggest that the increase in apical Na+ permeability in the presence of either forskolin or insulin is due to the insertion of channels residing in intracellular pools. In contrast, the increased Cl– permeability caused by forskolin may be related to the activation of channels already present in the membrane.

    Notes: Abstract An in vivo model has been developed for chronic observation of the effects of ischemia on cortical bone remodeling and perfused vascularity. Diaphragm occluders were implanted around the right common iliac artery of four rabbits and inflated to produce 10 h of ischemia to the limb. Microcirculation was monitored with intravital microscopy of injected fluorescent microspheres and FITC-Dextran 70 through a bone window, the tibial bone chamber implant (BCI). Bone resorption and apposition in the BCI were indicated with mineralization dyes. Between 2 and 12 h following release of the occluder, secondary ischemia/no-reflow and other evidence of reperfusion injury were observed. Vessel damage was suggested by abnormally high leakage of FITC-D70 from the few vessels perfused during secondary ischemia. In the weeks following occluder release perfused vasculature increased beyond pre-occlusion levels. Net bone resorption reached a maximum when vascularity passed normal levels. In order to further validate the arterial occlusion model for osteonecrosis, techniques for (1) confirming bone death and (2) detecting increased leukocyte adherence to endothelial cells were added. The dead cell stain Ethidium homodimer-1 was used to tag dead osteocytes immediately after occlusion and produced a measure designated “osteonecrosis index.” To detect leukocytes adhering to vessel walls, carboxyfluorescein diacetate, succinimidyl ester was injected at occluder release.     Notes: Abstract The aim of this study was to describe the contrast-enhanced magnetic resonance imaging (MRI) appearance of bone tunnel enlargement detected on radiography after anterior cruciate ligament (ACL) reconstruction with semitendinosus and gracilis tendon endobutton (STG-endobutton) fixation technique. Fourteen patients with a STG-endobutton ACL reconstruction were examined 3 months (n = 1), 1 year (n = 1) and 2 years (n = 12) postoperatively. An age- and sex-matched group with a bone–patellar tendon–bone (BTB) autograft ACL reconstruction with similar follow-up was taken as control. Data on clinical examination, laxity and isokinetic muscle torque measurements, anteroposterior and lateral view radiography were obtained, and knee scores (Lysholm and Tegner) were collected. Contrast-enhanced MRI was performed in the STG-endobutton group with a 1.5-T imager. There were no statistical differences between the groups with respect to clinical findings, stability tests, or knee scores. In the STG-endobutton group the average femoral and tibial bone tunnel diameter detected on anteroposterior view radiography had increased at 2-year follow-up by 33% and 23%, respectively. On MRI the ligamentous graft itself was not enhanced by the contrast medium whereas periligamentous tissue within and around the STG graft bundles showed mild contrast enhancement. In conclusion, the MRI results suggest that enhancing periligamentous tissue accumulated in and around the STG graft associated with the tunnel expansion. In spite of the significant bone tunnel enlargement observed on the follow-up radiography the STG-endobutton knees were stable and the patients satisfied.

    Notes: Abstract We investigated the effect of different doses of cyclosporin A (CyA) on glucose and insulin levels, as well as its residual effects on pancreatic islets ultrastructure after discontinuation of the drug. We studied four groups of Wistar rats. One control- (n = 5) and three experimental groups, n = 10 each, were treated with different doses of CyA IM for 14 days: group I, 5 mg/Kg; group II, 15 mg/Kg; and group III, 25 mg/Kg. Five animals of each group were sacrificed after 14 days, and the remaining five after 21 days to assess residual CyA effects. On the day of sacrifice, the rats underwent maltose absorption test, and glucose and insulin levels were measured. Pancreatic biopsies were obtained on day 21 to evaluate islets ultrastructure by electron microscopy. As a result, statistically significant, dose dependent (P 〈 0.05) increases in glucose and insulin levels were observed in CyA-treated groups. Groups II and III showed insulin levels significantly higher after fasting (P 〈 0.05) on day 14 comparing to the controls, while in groups I and II values returned to normal after CyA discontinuation. Group III showed persistently increased insulin levels on day 21. Pancreatic ultrastructural changes were observed only in group III. We can conclude that CyA effects on glucose and insulin levels were temporary and reversible at low doses. Ultrastuctural changes in the pancreatic islets may occur with high doses of CyA.

Autoptisch wurde eine (heimliche) Applikation von Insulin durch letal hohe Insulinspiegel im zentralen und peripheren Blut mit Hypoglykaemie diagnostiziert. Nach der Anamnese, den Ergebnissen des Ermittlungsverfahrens und der Autopsie konnte die Applikation von Insulin aus suizidaler und homizidaler Absicht oder im Rahmen eines Münchhausen-Syndroms ausgeschlossen werden. Vielmehr handelte es sich in der Gesamtschau um eine Hypoglykaemia factitia aus diätetischen Gründen.
    Notes: Abstract The medical history of a 28-year-old non-insulin-dependent and non-diabetic patient with a hypoglycaemic episode before death is reported. At autopsy, an (surreptious) insulin administration was documented by lethally high serum insulin in central and peripheral blood during hypoglycaemia, According to the anamnesis, the judical inquiry and the autopsy findings, a suicidal intent, murder by insulin or Munchhausen-syndrome could be excluded. We therefore diagnosed a hypoglycaemia factitia caused by the diet.

    Notes: Abstract In subjects who maintain a constant body mass, the increased energy expenditure induced by exercise must be compensated by a similar increase in energy intake. Since leptin has been shown to decrease food intake in animals, it can be expected that physical exercise would increase energy intake by lowering plasma leptin concentrations. This effect may be secondary either to exercise-induced negative energy balance or to other effects of exercise. To delineate the effects of moderate physical activity on plasma leptin concentrations, 11 healthy lean subjects (4 men, 7 women) were studied on three occasions over 3 days; in study 1 they consumed an isoenergetic diet (1.3 times resting energy expenditure) over 3 days with no physical activity; in study 2 the subjects received the same diet as in study 1, but they exercised twice daily during the 3 days (cycling at 60 W for 30 min); in study 3 the subjects exercised twice daily during the 3 days, and their energy intake was increased by 18% to cover the extra energy expenditure induced by the physical activity. Fasting plasma leptin concentration (measured on the morning of day 4) was unaltered by exercise [8.64 (SEM 2.22) 7.17 (SEM 1.66), 7.33 (SEM 1.72) 1μg · l−1 in studies 1, 2 and 3, respectively]. It was concluded that a moderate physical activity performed over a 3-day period does not alter plasma leptin concentrations, even when energy balance is slightly negative. This argues against a direct effect of physical exercise on plasma leptin concentrations, when body composition is unaltered.

    Description / Table of Contents: Abstract The aneurysmal bone cyst localizised at the longfingers of the hand is an extremely rare clinical entity, particularly considering the age and the distinct aggressiveness in our case. In this specific case of a 53-year-old male the differentialdiagnosis as well as problems of the aneurysmal bone cyst with affection of the phalangeal bone will be discussed. Furthermore the various possibilities of therapy will be focussed on. At first the patient was treated with curettage followed by bone grafting. Already 5 months later a local recurrence appeared. The proximal phalangeal bone and the metacarpophalangeal joint V were almost distroyed. Aiming at preservation of a high funcionality of the hand we amputated the 5th finger in the metacarpophalangeal joint.
    Notes: Zusammenfassung Die aneurysmatische Knochenzyste mit Lokalisation an der Hand, insbesondere an den Phalangen, ist ein extrem seltenes Krankheitsbild. Auch das Alter des Patienten und die ausgeprägte Aggressivität unseres Falles sind bemerkenswert. Anhand der Krankengeschichte eines 53jährigen Mannes mit Befall der Grundphalanx des fünften Fingers der rechten Hand werden die Problemstellung bzw. Differentialdiagnose verdeutlicht und die verschiedenen Therapiemöglichkeiten besprochen. In unserem Fall wurde zunächst eine Kürettage mit anschließender Spongiosaaufftüllung durchgeführt. Bereits nach fünf Monaten kam es zu einem aggressiven Rezidiv mit Zerstörung der gesamten Grundphalanx und des Grundgelenks, so daß wir uns schließlich unter Berücksichtigung der bestmöglichen Funktion der Hand für eine Amputation des Kleinfingers entschieden haben.

    Description / Table of Contents: Summary Almost each cell in the body is estrogen-receptive. The strength of estrogen influence on organs or organ systems, however, may vary. Thus, estrogen withdrawal may have more or less severe pathophysiological consequences. Recently, the existence of two estrogen receptors has been established (estrogen receptor α and β = ERα and ERβ and several splice variants of these two basic receptor types have been desribed). This, together with our increasing knowledge about tissue-specific enhancer and repressor genes now allows a better understanding of the organ-specific effects of estrogens. In view of gynecological endocrinology, hormone replacement therapy with estrogens is beneficial for most organs in the body (bone, cardiovascular system, CNS). However, pure estrogen therapy may have deleterious effects in the endometrium of the uterus because it may initiate the development of uterine cancer. In the uterus of most species studied so far, no ERβ but only the classical ERα gene has been described. Hence, drug companies have now intensively begun to search for ERβ-specific estrogens. The first, relatively specific, non-steroidal product that addresses primarily the ERβ is the drug raloxifen, which has been recently introduced clinically. However, also some natural phyto-estrogens appear to be ERβ-specific.
    Notes: Zusammenfassung Fast jede Körperzelle ist estrogenrezeptiv. Es gibt allerdings Organe und Organsysteme, die besonders unter dem Einfluß von Estrogenen stehen und die sich bei Estrogenmangel krankhaft verändern können. Die vielfältigen Wirkungen von Estrogenen konnten nicht vollständig verstanden werden, solange die Existenz von nur einem Estrogenrezeptor quasi ein Dogma war. Erst die Klonierung eines zweiten (Estrogenrezeptor β = ERβ) sowie das bessere Verständnis von gewebespezifischen Enhancer- und Repressor-Genen konnten die vielfältigen und z. T. organspezifischen Wirkungen der Estrogene aufklären. Aus der Sicht der gynäkologischen Endokrinologie ist die Hormonersatztherapie mit Estrogenen für fast alle Organe des Körpers (Knochen, Herz-Kreislauf-System, Zentralnervensystem) segensreich. Eine nicht gestagenbegleitete Estrogentherapie kann allerdings verheerende Folgen am Endometrium mit Entwicklung eines Corpus-Karzinoms haben. Da der Uterus praktisch kein ERβ sondern nur den klassischen ERα exprimiert, hat eine intensive Suche nach ERβ-spezifischen Estrogenen begonnen. Ein erstes, halbwegs spezifisches nicht-steroidales Produkt scheint das kürzlich in die Klinik eingeführte Raloxifen zu sein, aber auch einige Phytoestrogene scheinen ERβ-spezifisch zu wirken.

    Notes: Abstract We undertook a genome-wide scan using 190 markers with an average separation of 20 cM in 49 Canadian Oji-Cree sib pairs affected with type 2 diabetes. Four of these markers, one each on chromosomes 6, 8, 16, and 22, showed both suggestive linkage and suggestive association with type 2 diabetes in the Oji-Cree. None of these markers corresponded to any chromosomal region or marker that has so far been linked with type 2 diabetes in other populations. Thus, there might be several genetic loci that confer susceptibility to type 2 diabetes in this study sample. We are following up on these preliminary leads by increasing the density of the markers within these linked and associated regions, and also by increasing the number of study subjects. Also, we found instances in which there were wide disparities between the Oji-Cree and reference Caucasians with respect to marker heterozygosity. This suggests that a particular set of markers for genome-wide scanning will have different informativeness in different ethnic groups. Thus, different marker sets will likely be required for different ethnic groups in order to maximize their information content for linkage calculations.

    Notes: Seen mit Wasser, so sauer wie verdünnte Essigsäure, so sauer wie manche Kraterseen, gibt es solche Seen in Deutschland? Ja, sie gibt es, und zwar sogar recht zahlreich in den alten und vor allem in den neuen Bundesländern. Alle diese Seen sind künstlicher Natur und treten im Zusammenhang mit dem Abbau von Bodenschätzen auf. Durch diesen Vorgang werden tiefere Schichten des Deckgebirges belüftet und mit Wasser versorgt, so daß eine chemische und vor allem mikrobiologische Oxidation von reduzierten Schwefelkver bindungen (Sulfiden) einsetzen kann. Dieselben Vorgänge laufe auch auf und in den Abraumhalden ab. Sulfide sind sehr häufige Begleiter von Braun- oder Steinkohlevorkommen. Ein wesentlicher Teil der Wasserkontaminationen im mitteldeutschen und Lausitzer Raum ist in der Tat mit der Braunkohleförderung verbunden. Ferner sind die meisten, abbauwürdigen Metallvorkommen ebenfalls in sulfidischen Erzen zu finden, so daß deren Abbau zu denselben Oxidation führt, die international als Als Mine Drainage bezeichnet werden.

    Notes: Als Fertilitätsrate bezeichnet man die Zahl der Geburten pro Frau in ihrer Lebensspanne. Wenn Zu- Oder Abwanderung in einer Population gering sind, läßt eine Fertilitätsrate von 2,0 eine konstante Bevölkerungszahl erwarten. Bei der Entwicklung der Erdbevölkerung beobachtet man in den industrialisierten Ländern derzeit keinen Anstieg. Die Fertilitätsrate liegt in den USA bei 2,1 und in Europa durchschnittlich bei 1,4. Mit 1,2 und 1,15 haben Italien und Spanien dabei die weltweit niedrigsten Fertilitätsraten. Somit ist örtlich sogar ein Bevölkerungsrücksgang zu erwarten. Auch in ostasiatischen Ländern wie China und Korea hat sich der Bevölkerungszuwachs verlangsamt. In Südasien und in afrikanischen Ländern wie China und Korea hat sich der Bevölkerungszuwachs verlangsamt. In Südlich der Sahara finden wir jedoch nach wie vor relativ hohe Gebeurtenraten. Im westafrikanischen Nigeria liegt die Fertilitätsrate bei 6,3. Für Indien und Pakistan werden Werte von 3,5 und 5,6 angegeben. Es gibt also noch viele Regionen mit einer hohen Fertilitätsrate. Manche Wissenschaftler gehen sogar davon aus, daß sich die Zahl entsprechender Staaten eher noch erhöhen wird ‘7’. Deshalb weichen auch die Schätzungen voneinander ab, bei welcher Bevölkerungszahl Wachstumsstillstand eintreten wird. Viele Demographen gehen davon aus, daß sich die Erdbevölkerung bis zum Jahr 2050 bei 10 Milliarden stabilisieren wird ‘9’. Andere schätzen, daß danach ein weiterer Ansteig auf etwa 12 Milliarden menschen zu erwarten ist ‘7’. Der Rückgang der Geburtenzahlen in den industrialisierten Ländern wurde mit kontrazeptiven Maßnahmen erzielt, die uns schon lange zur Verfügung stehen und die gut eingeführt sind. Auf den ersten Blick Sch eint es somit aus bevölkerungspolitischer Sicht auf dem Gebiet der Kontrazeption keinen zwingenden Bedarf für Neuentwicklungen zu geben. Dennoch sind Forschungsaktivitäten im Gang, die auf eine pharmakologische Kontrazeption beim Mann gerichtet sind. Im vorliegenden Artikel werden neben den traditionellen Möglichkeiten, die dem Mann zur Kontrazeption offenstehen, der wissenschaftliche Hintergrund und der Entwicklungsstand einer “Pille für den Mann” vorgestellt.

In Deutschland zeigen sich indessen eher verhaltene Auswirkungen dieser Entwicklung ‘3’. Die wirtschaftliche Entfaltung der Gentechnik und Biotechnologie ist in Deutschland noch gering. Als ein Maß für den Entwicklungsstand der Gentechnik in der Pflanzenzüchtung ‘2’ und damit unter anderem auch deren Auswirkung auf die Futter- und Lebensmittelproduktion kann die Anzahl von Freisetzungsvorhaben mit gentechnisch veränderten Organismen (GVO) gelten. Von den rund 1300 Freisetzungsvorhaben im Bereich der EU entfallen 90 auf Deutschland. Im Vergleich zu den anderen EU-Mitgliedsstaaten rangiert Deutschland damit auf dem siebenten Platz.

    Notes: Die heutigen Wälder, die man in Europa vorfindet, waren nicht immer dort. VOr 22000 Jahren - im Hochglazial - gab es nur im Mittelmeergebiet die klimatischen Voraussetzungen für das Wachstum von Bäumen. Allerdings waren dies meist keine dichten Wälder, sondern Steppen mit einzelnen Baumgruppen. Das bedeutet jedoch, daß die heute in Europa wachasenden Bäume während der letzten 12000 Jahre - also seit dem Ende der letzten Eiszeit - nach Mittel- und Nordeuropa eingewandert sein müssen. Am Beispiel der Waldbäume Eiche (Quercus), Hainbuche (Carpinus), Fichtet (Picea) und Tanne (Abies) soll diese Einwanderung aufgezeigt werden.

    Notes: Quecksilber und seine Verbindungen haben trotz ihrer Giftigkeit vielfältige medizinische und industrielle Anwendungen. Die Nutzung des Quecksilbers durch den Menschen hat jedoch erhebliche Umweltbelastungen, insbesondere quecksilberbelastete Abwässer und Böden, zur Folge, die auch mit physikalisch-chemischen Sanierungsverfahren nicht befriedigend beseitigt werden können, da die Verfahren häufig teuer oder nicht spezifisch genug sind. Mikroorganismen haben im Laufe ihrer Evolution einen Mechanismus entwickelt, um Quecksilberverbindungen zu entgiften. Er beruht auf den Aktivitäten von zwei durch das mikrobielle mer-Operon codierten Enzymen, Quecksilberreduktase und Quecksilberlyase, die in der Lage sind, Organoquecksilberverbindungen und ionuisches Quecksilber in metallisches Quecksilber zu überführen. Die mikrobielle Quecksilberreisistenz kann eingesetzt werden, um quecksilberhaltige Abwasserströme mittels eines einfachen, umweltfreundlichen Verfahrens zu reinigen.Innenansicht einer Chlor-Alkali-Elektrolyseanlage zur Gewinnung von Chlor und Natronlauge nach dem Amalgamverfahren. Bei diesem Prozeß wird auch heute noch in großem Umfang Quecksilber eingesetzt. In den Elektrolysezellen dieser Halle befinden sich insgesamt etwa 50 Tonnen Quecksilber.

    Notes: Die fossile Überlieferung aus den letzten präkambrischen Jahrmillionen deutet darauf hin, daß gelatinös organisierte Formenj, also quallenähnlich gebaute Organismen, die das marine Pelagial (den freien Wasserkörper des Meeres) besiedelten ‘3’. Was sie dort vorfanden war jedoch wenig labensfreundlich. Zwar wimmelte das Meer von bakterien und Protozoen, aber im wesentlichen waren die riesigen Wasserräume leer und unbesiedelt. Crustaceen und andere Organisationsformen waren - wenn die Befunde stimmen - noch in der Entwicklungsphase beziehungsweise noch nicht zu der pelagischen Lebensweise übergegangen. Wie überlebten die quallenförmigen Organismen in dieser Umwelt, und warum leben sie noch heute in allen Bereichen des Meeres, von der Flach- bis in die Tiefsee, von den Polen bis zu den Tropen? Der pelagische Lebensraum beinhaltet Tausende von Arten, die viel “höher” organisierten Gruppen angehören, die schneller und kräftiger sind. Wieso wurden die gelatinösen Plankter nicht in irgendeine skurrile Nische gedrängt oder über die Konkurrenz völlig ausgeschaltet? Mögliche Antworten auf diese Fragen finden wir in der Gegenwart.

    Notes: Nach dem Grundsatz “Vermeiden geht vor Wiederverwertung oder Recycling und letzteres vor Ablagerung auf einer Deponie”, schreibt das Kreislaufwirtschafts- und Abfallgesetz 3 zwingend die Wiederverwertung von Wertstoffen vor. Schlacke aus Müllheizkraftwerken (MHKWs) wird vom Gesetzgeber als ein Wertstoff klassifiziert und unterliegt damit dem Wiederverwertungszwang. Verwendung findet die MHKW-Schlacke im Straßenbau als Ersatz für Sand und als Versatzmaterial in au fgelassenen Bergwerken. Während der letztere Verwendungszweck relativ unumstritten, wenn auch sehr teuer ist, stößt der erstere auf herbe Kritik seitens ökologisch orientierter Gruppen, insbesondere wegen des hohen Schwermetallgehalts der MHKW-Schlacke. Dieses geschieht unbeschadet der Tatsache, daß der Einsatz von Schlacke als Baumaterial starken Einschränkungen unterliegt, die insbesondere einen Kontakt der Schlacke mit Grundwasser und Niederschlägen und damit eine Elution ausschließen ‘6’. In Zukunft dürfte die Schlackeproblematik noch mehr als bisher Gegenstand umweltpolitischer Disku ssion sein, da ab dem Jahre 2005 alle herkömmlichen Hausmülldeponien für unverbrannten Restmüll de facto geschlossen werden. Dieses wird zwangsweise zu einer vermehrten Verbrennung von Restmüll und somit zu einer verstärkten Schlackeproduktion führen. Damit erhebt sich erneut die Frage nach der Wiederverwendung von Schlacke und damit nach ihrer Umweltverträglichkeit. Als sowohl ökonomisch als auch ökologisch vertretbare Alternativen stehen bislang nur neue kostengünstige Formen der Deponierung im Raum. Der folgende Artikel befaßt sich mit dem Pro und Contra der Wiederverwendung von MHKW-Schlacke aus Sicht der Pflanzenphysiologie und gibt eine Übersicht über die Streßfaktoren, denen Pflanzen bei der Kultur auf Schlacke ausgesetzt sind.

    Notes: Vielfalt in allen Erscheinungsformen kennzeichnet schon die unbelebte und in noch stärkerem Maße die belebte Natur. Dagegen drängt die Nutzung von Naturkräften durch den Menschen, insbesondere in ihren weiter entwickelten und technisierten Formen stets auf Vereinfachung. So werden landwirtschaftliche Kulturpflanzen bei ein- bis mehrjähriger Nutzung im Regelfall in Reinkultur angebaut, ganz im Gegensatz zur standorttypischen Artenvielfalt eines natürlichen Pflanzenbestandes. Lediglich die verbliebene Unkrautflora erinnert dann and die ursprüngliche Artenvielfalt einer Pflanzengesellschaft. Es kommt hinzu, daß sich landwirtschaftlicher Pflanzenbau auf nur wenige anbauwürdige Kulturpflanzenarten konzentriert. Weltweit nimmt das Ackerland etwa 10 % der Landoberfläche der Erde ein, die mit nur etwa 30 Kulturpflanzenarten von überregionaler Bedeutung bestellt werden. Allein 37,4% davon sind dem Anbau von nur dreif für die Ernährung der Menschheit besonders bedeutsamen Arten (Weizen, Resi und Mais) gewidmet. In Deutschland werden gegenwärtig sogar 33% der Landoberfläche als Ackerland genutzt. Von den insgesamt etwa 14 bei uns häufiger anzutreffenden Kulturpflanzenarten besetzen die drei dominierenden Arten (Winterweizen, Mais und Wintergerste) dabei 48% der Anbaufläche. Landwirtschaftlicher Pflanzenbau wird also generell artenarm betrieben.Die zweite Ebene bilogischer Vielfalt im landwirtschaftlichen Pflanzenbau stützt sich auf die mehr oder weniger regelmäßige Folge verschiedener in Reinkultur angebauter Kulturpflanzenarten. Bozogen auf die Gesamtfläche eines Betriebes order die Feldflur einer Gemeinde ergibt sich dann noch immer ein abwechslungsreiches Gesamtbild verschiedener Aussaat-, Blühund Erntezeiten. Für artenreiche Fruchtfolgen lassen sich darüber hinaus viele, für die Bodenbewirtschaftung vorteilhafte Argumente anführen. Die hohe Schlagkraft der vollmechanisierten Betriebe und die beliebige Verfügbarkeit von Dünge- und Pflanzenschutzmitteln in Verbindung mit artunterschiedlichen Ertrags- und Preisrelationen haben jedoch auf intensive bewirtschafteten Betrieben in günstigen Lagen zu einer oft drastischen Reduktion der Pflanzenproduktion (auf den Anbau von nur noch drei, manchmal sogar nur zwei Arten) geführt, von denen der jeweils höchste Beitrag zur Deckung der Betriebskosten erwartet werden kann.Inzwischen hat sich jedoch die Erkenntnis durchgesetzt, daß die Aufrechterhaltung “biologischer Diversität” ein wesentliches Element zur Sicherung der “Nachhaltigkeit” der landwirtschaftlichen Pflanzenproduktion darstellt. Dies hat in der Konferenz von Rio (1992) im Rahmen der Agenda 21 zum Abschluß einer “Konvention über Biologische Vielfatl” geführt, der auch die Bundesrepublik Deutschland beigetreten ist.Mit dem Begriff biologische Vielfalt werden in dieser Konvention nicht nur Naturschutzverpflichtungen zur Sicherung des Erhalts bedrohter Tier- und Pflanzenarten übernommen, sondern es geht zu einem wesentlichen Teil auch um die 3. Ebene der biologischen Vielfalt in Form der genetischen Vielfalt innerhalb der für die Welternährung grundlegend bedeutsamen Kulturpflanzenarten. In diesem Zusammenhang soll hier gefragt werden, ob mit der Einführung transgener Nutzpflanzen neben ihrem wirtschaftlichem Nutzen auch Risiken für die biologische Vielfatl verbuden sind.

    Notes: Es ist schon erstaunlich: Man setzt sich eine einfache Brille auf, die als “Gläser” zwei farblose Folien enthält, und schon spielt die räumliche Sehwelt verrückt. Farbige Felder in bunt gemusterten Flächen springen in der Wahrenhmung auf uns zu, und andere weichen zurück. Betrachtet man eine Person vor dunklem Hintergrund, die einen roten Pullover and eine blause Hose anhat, so sieht man die obere Hälfte viel näher als die untere. Und noch verrückter: Ähnlich wei bei Michael Endes “Scheinriese” Tur-Tur ‘1’ nimmt der Tiefeneindruck zu, je weiter man sich von dem betrachteten Objekt entfernt. Wie kommt dieser Effekt zustande?Das Grundprinzip der ChromaDepth-Brille (C3DTM glasses), die von der Firma Chromatek in Georgia/USA hergestellt wird, ist die Farbdispersion: Rotes Licht wird durch das Gitter der brillenfolie stärker seitlich abgelenkt (gebeugt) als blaues Licht (Abbildung 1). Das dies für beide Augen gegensinnig erfolgt, ergeben sich Bildunterschiede zwischen rechtem und linkem Auge; Disparitäten, die das Sehsytem im Gehirn wie gewohnt als Raumtiefe deutet(Farbtiefeneffekt, Abbildung 2) Deshalb seiht man mit der ChromaDepth-Brille bunt gemusterte Flächen räumlich, genauer formuliert: Setzt man die Brille auf, so springen die Bilder, die wir mit unseren beiden Augen sehen, etwas aufeinander zurote Bildteile mehr, blaue weniger, Die Folge ist: Vor dunklem hintergrund erscheinen rote Flachen näher als blaue. Vor hellem Hintergrund ist es genau umge Kehrt: Rot erscheint hinter Blau. Das Ausmaß der wahrogenommenen Tiefe ist beträchtlich. So scheint der rote Fleck eines Laser-Pointers, auf eine vier Meter entfernte weiße Wand fällt, 110 Zentimeter vor dieser Wand in der Luft zu schweben. Im einzelnen kan der Sachverhalt sehr kompliziert sein. Auch wenn wir meinen, eine bestimmte Farbe zu sehen, handelt es sich in Wirklichkeit meist um ein kompliziertes Gemisch verschiedener Wellenlängen, die alle mit der ChromaDepth-Brille auseinan-dersortiert werden und ganz unerwartete Tiefenstaffelungen erzeugen können.

Aus einem einzigen Samen einer selbstbestäubenden Pflanze kann sich bei gleichzetig hoher Reproduktionsrate, schnell eine große Population aufbauen. Viele kleine Brassicaceen (Kreuzblülter), wie etwa die Ackerschmalwand Arabidopsis thaliana, sind dafür gute Beispiele. Doch uneingeschränkte Selbstbefruchtung hat offensichtliche Nachtelie. Ungünstige Mutationen häufen sich im Genom an, und die durch Durchmischung des Erbgutes bewirkten heterosiseffekte entfalten sich nicht. Daher finden sich in etwa der Hälfte aller pflanzenfamilien Arten, bei denen Selbstbefruchtung (Autogamie) ausgeschlossen ist ‘5’. Verbreitete Mechanismen, die eine Allogamie (Frembdefruchtung) begünstigen, sind unterschiedliche Reifezeitpunkte und -orte der weiblichen und mänlichen gameten auf einer Pflanze oder die Diözie (Zweihäusigkeit), bei der weibliche und männliche Gameten auf unterschiedlichen Individuen gebildet werden. Weiterhin gibt es eine Vielzahl morphologischer Barrieren, die das zufällige Bestäuben mit dem eigenen Pollen behindern und Pflanzen häufig an Tiere als Pollenüberträger von Blüte zu Blüte binden. “Echte” selbstinkompatible pflanzen können den eigenen Pollen oder den der “näheren Verwandtschaft” erkennen und abn der Befruchtung hindern, sie sind aber nicht etwa steril, wie der früher gebrächliche Begriff Selbststerilität nahelegte, da sie fertile Gameten bilden. Erkannt wurde das Phänomen schon 1764 von Köreuter für die Königskerze Verbascum phoenichicum, doch die erste eingehende Analyse und Zusammen fassung dieser Daten und eigener Befunde erfolgte später (1867) durch Charles Darwin. Er erkannte als erster, daß die beobachtete Unfruchtbarkeit durchaus als “Begabung” angesehen werden kann.Viele landwirtschaftlich genutzte Arten sind heute selbstkompatibel, weil im Laufe ihrer Kulturgeschichte die Selbstinkompatibilitäts- Mechanismen verlorengegangen sind oder bewuß darauf verzichtet wurde. Dies hat gute Gründe: Einer davon ist vor allem eine Ertragssteigerung durch die ungehinderte (Selbst-) Befruchtung. Allerdings ist mit solchen Pflanzen die Produktion von Hybridsaatgut zur, “Handarbeit” des Züchters geworden. Im Zeitalter teurer menschilcher Arbeitskraft hat daher jede Art von Pollensterilität nicht zu unter schätzende wirtschaftliche Bedeutung gewonnen. Die (Rück-) Übertragung eines funktionierenden Selbstinkomatibilitätssystems auf kulturpflanzen erscheint hier als ein Lösungsweg, insbesondere da solche Pflanzen weiterhin fertilen Pollen produzieren könnten, also nicht steril wären. Allerdings ist man heutzutage noch zu weit von einem umfassenden Verständnis der natürlichen Selbstinkompatibilitäts Systeme entfernt, um deren übertragung auf selbstkompatible Methoden, erreichen zu Können.

    Notes: Nur bei wenigen Wachstumsprozessen wird eine so groß Genauigkeit gefordert und erreicht wie bei der Abstimmung der Augenlänge auf die Brennweite von Linse und Hornhaut. Wenn das Auge um nur ein Prozent zu lang ist, besteht bereist eine Kurzsichtigkeit (Myopie) von fast einer Dioptrie (dpt). Dabei fällt die Sehschärfe in der Ferne auf etwa ein Drittel. ab. Ist das Auge daggen zu kurz (Weitsichtigkeit-Hyperopie), so kann man zwar in der Ferne meist noch gut sehen, aber die Arbeit in der Nähe wird schwierig, weil mehr Akkommodation aufgebracht werden muß. Die hat nichts mit der “Alterweitsichtigkeit” (Presbyopie) zu tun, wo zwar der Bau des Auges normal ist, aber der altersbedingte Verlust der Akkommodation scharfes Sehen in der Nähe unmöglich macht.Beim Menschen, einigen Afen und Vögeln erreicht die Abbildungsgüte im Bereich der Stelle des schärfsten Sehens (Fovea) die Grenze der physikalischen Möglichkeiten. Nicht optische Schwächen oder Ungenauigkeiten im Bau des Auges, sondern die Lichtbeugung an der Pupille, die zwangsläufig aus den Welleneigenschaften des Lichtes folgt, begrenzen weitere Auflösung. Es ist schwer vorstellbar, wie diese Genaukeit erreicht werden kann, wenn nicht eine Steuerung des Wachstums durch einen rückgekoppeltn Regelkreis erfolgt, der sein Fehlersignal aus dem Netzhautbild entnimmt. Bis vor wenigen Jahren herrschte dennoch die Meinnung vor, daß das Augenlängenwachstum weitgehend genetisch bestimmt ist. Inszwischen ist aber durch Experimente mit brillentragenden Hühnern und Affen gezigt worden, daß “normale” Seherfahrung tatsächlich Voraussetzung für genaues Wachtsum ist. Die zugrundeliegenden Mechanismen sind noch nicht vollständig aufgeklärt, ber bisherige Ergebnisse lassen Schlüsse über mögliche Ursachen von Kurzsichtigkeit zu.

    Notes: Kein Schiff überquert sden Ozean ganz ohne blinde passagiere. Muscheln undAsseln bohren sich in Holzplanken. Algen und Seepocken heften sich mi Unterwasserbereich an die Schiffsrümpfe. Planktische sporen und Larven gelangen in das Ballastwasser, das jedes moderne Schiff zur Stabilisierung mit sich führt ‘1 und 3’. Je größer und schneller ein Schiff ist, desto, besser ist dies für die unbeachteten Tramper der Weltmeere. Wer solch eine lange Seereise überlebt, ist auch widerstands- und anpassungsfähig genug, um sich an neuen Küsten zurechtzufinden (Abbildung 2). Günstige Gelegenheiten zur Mitreise ergeben sich auch, wenn Austern oder andere Meerestiere für die Weiterzucht an ferne Küsten versandt werden. Solch ein Handel hat weltweit zugenommen, ebenso wie der interkontinentale Schffsverkehr. Diese Entwicklung führte dazu, daß mehr und mehr Arten zu Bewohnern weit voneinander entfernter Küsten wurden. Der mensch half die natürlichen Ausbreitungsbarrieren zu überwinden und leitete eine Globalisierung der Lebensgemeinschaften an den Küsten ein. - Welchen Umfang aber hat dies im Bereich der Nordsee angenommen? - Welche ökologischen Konsequenzen ergeben sich? - Kann und soll dieser Trend aufgehalten werden? - Diese brisanten Fragen stellen sich weltweit für die Meeresküsten und ebenso auch für die Süßwasser und das Land ‘11-13’.

    Notes: Zu en vielen Schutz- und Verteidigungsstrategien, die von den Organismen entwickelt wurden, um dem Gefressenwerden zu entgehen, Zählen ein sperrigerwerden der Individuen, die Bildung von Panzern, Stacheln und Dornen, das Produzieren und Speichern von schwerverdaulichen Stoffen und Giften, aber auch die Entwicklung eines komplexen verhaltensrepertoires zur Vermeidung von Freßfeinden ‘8’. Bis zur dieses Jahrhunders glaubte man, daß die der Verteidigung dienenden Maßnahmen dauerhaft seien, also im Laufe der Evolution entstanden sind und im Verlauf ddes Lebens eines Individuums zu einem bestimmten Zeitpunkt zur Ausprägung, gelangen, unabhängig von der gerad herrschenden Bedrohung. Heute wissen wir, daß neben einer solchen kionstitutiven Verteidigung in vielen Organismengruppen auch induzierbare Verteidigungsmechanismen existieren, welche nur dann wirksam werden wenn Freßfeinde im Biotop in größerer Dchte auftreten und somit eine ernstzunehmende Bedrohung darstellen. Das Spektrum induzierbarer Feindabwehrmechanismen ist der Palette der kionstitutiven Abwehrmaßnahmen dabei durchaus sehr ähnlich. Der entscheidende Unterschied ist jedoch der, daß der bedrohte Organismus erst auf ein bestimmtes Signal hin von seinem genetisch festgelegten Schutzmechanismus Gebrauch macht. Dadurch werden die Kostenfür die Feindabwehr minimiert. Neben physikalischen Signalen (wie Licht und Temperatur), welche dem Organismus indirekt Auskunft über den Grad einer aktuellen Bedrohung geben können, spielen von den Freßfeinden abgegebene chemische Signale (Kairomone) eine entscheidende Rollen als Auslöser der induzierbaren Verteidigungsmechanismen. An der Aufklärung der chemischen Natur der Kariomione wird zur Zeit intenxiv gearbeitet.

    Notes: Früher wurden über 90% aller Vogelarten als monogam klassifiziert, also als in Einehen lebend ‘5’. Manhatte zwar auch schon damals vereinzelt beobachtet, daß es bei einem Teil dieser Arten manchmal zu Kopulationen mit anderen Individuen als dem Paarpartner kommen kann, maß diesem Verhalten jedoch keine größere Bedeutung bei. Man dachte vielmehr, daß es sichhierbei um eine Art krankhaftes Verhalten handele. Dises Ansicht änderte sich erst, als verschiedene Methoden von Elternschaftsnachweissen gezielt auch bei Vögeln angewenden wurden, um damit Jungtiere zu identifizieren, die aus solchen “Fremdkopulationen resultieren”. Dabei stellte sich heraus, daß die Häfigkeit dieses Verhaltens aufgrund der wenigen Beobachtungen stark unterschätzt worden war und daß bei einer beeinduruckenden Zahl von Vogelarten mit einem monogamen paarungsssytem Jungtiere zu finden sind, die nicht von dem Männchen abstammen, welches die Brut versorgt ‘1’.Die Untersuchungsergebnisse werfen eine Reihe weiterführender evolutionsbiologischer Fragen auf. So stellt sich unter ande rem die Frange nach dem Anpassungswert, halb des Paarbundes. Während der Nutzen f¨r die Männchen auf der hand zu liegen scheint, sie können durch, “Fremdkopulationen” die Anzahl ihrer Nachkommen erhöhen, ist der evlutionsbiologische Vorteil für die Weibchen nicht nämlilck durch Ko pulationen außerhalb des Paarbundes normalerweise nicht die Anzahl ihrer Nachkommen steigern. Sie erhöhen durch Kopulationen mit einem anderen Männchen allenfalls die genetische Variabilität ihree Nachkommen.Ein weiterer Fragenkomplex ergibt sich durch den Umstand, daß brutpflegende Männchen vermeiden sollten, für Jungtiere zu sorgen, die nicht von ihnen selbst stammen. Dises Erwartung basiert darauf, daß Brutpflege gewöhnlich mit Kosten (Enegie und Zeit) verbunden ist: Wenn die Männchen für Jungtiere sorgen, die nicht von ihnen selbst stammen, dann reduzieren sie auf längere Sicht ihren eingene Fortpflanzungserfolg.Im fogenden Aufastz soll versucht werden, die beiden genannten Fragenkomplexe beispielhaft an Untersuchungsergebnissen zum Paarungsverhalten von Kohleisen zu vertiefen. Dabei soll zunächst gezeigt werden, wie mit Hilfe moderner molekulargenetischer Methoden, namentlich dem DNA-Fingerprinting, das Fortpflanzungsverhalten einer Vogelart analysiert werden kann, um dann anschließend auf die eben genannten evolutionsbiologisch relevanten Aspekte einzugehen.

    Notes: Blütenpflanzen können nur über ihre Ausbreitungseinheiten oder Diasporen wandern. Das sind Keimkörper, die entweder auf ungeschlechtliche oder geschlechtliche Weise gebildet werden. Im folgenden werden nur die auf geschlechtlichem Weg entstandenen und für die Bluütenpflanzenevolution so wichtigen Ausbreitungskörper diskutiert.Im einfachsten Fall sind die Ausbreitungskörper bei en Blütenpflanzen die Samen selbst. Es können jedoch auch andere Blütenteile (Fruchtblätter, Blütenaches) und sogar Pflanzenteile außerhalb der Blüte (Tragblätter) in den Aufbau der Diasporen einbezogen sein. Mannigfaltig sind dabei die Anpassungen an die verschiedenen Vektoren, die für den Transport der Diasporen verantwortlich sind. Wind, Wasser, Tiere oder die Pflanzen selbst sorgen für eine geeignete Ausbreitung. Oft wirken mehrere Vektoren zusammen. Große Unterschiede bestehen in der Größe der Diasporen und in der Anzahl, die eine Blütenpflanze hervorbringt. experimente deuten jedoch immer wieder auf ein einheitliches Grundmuster der Ausbreitung hin.

    Notes: A Is unerläßliches Nebenprodukt fördert der Steinkohlenbergbau unverwertbares Gestein, die “Berge”. Nur ein geringer Prozentsatz dieses Materials kann wieder unter die Erde zuruckgebracht werden, der weitaus größte Teil wird zu Bergehalden aufgeschüttete man die Halden in Form von Spitzkegeln auf, später wurden sie als terrassierte Tafelberge gestaltet. Heute versucht man, die Bergehalden als eine Art “Landschaftsbauwerk” in die Umgebung zu integrieren.Das nackte, schwarze Gestein der Aufhaldungen findet bei der Bevölkerung keine Akzeptanz, darüber hinaus verursacht eine unbegrünte Bergehalde erhebliche Staubentwicklung. Diese Gründe machten es erforderlich, die Bergehalden zu begrünung bestockte man die Halden mit Gehölzpflanzen, mittlerweile versucht man, die Halden unter Einbeziehung wissenschaftlicher Erkenntnisse nach ökoogischen Gesichtspunkten zu bepflanzen ‘22’. Von besonderem Interesse ist die Frage nach dem Wert dieser Flächen seitens des Naturschutzes. Dies gewinnt insbesondere deshalb an Bedeutung, da gerade in den industriellen Ballungsräumen im städtischen Bereich oft ein großer Verlust an Grünflaächen Stattgefunden hat. Eine Bewertung der Bergehalden hinsichtlich ihres Nutzens für naturschutzorientierte Zwecke kann jedoch nur anhand mehrjähriger Untersuchungen erfolgen.
