    Notizen: Summary The course is reported of a patient with periarteritis nodosa who initially presented with neurological symptoms. Multiple cerebral lesions were documented by the first magnetic resonance imaging (MRI) investigation. The majority of these had disappeared completely in the follow-up MRI studies. In contrast to neurological improvement the patient eventually died due to multiorgan failure. Postmortem histological examination revealed no pathological findings in the brain except one single necrotic area already known from MRI. Remissions of histological and angiographic alterations in periarteritis nodosa have been described as “local healing” leading to fibrosis and scarring. Our findings suggest that restitutio ad integrum may occur, at least in cerebral lesions.

    Beschreibung / Inhaltsverzeichnis: Résumé A partir d'une exploration IRM de la fosse postérieure de 30 sujets volontaires sains une corrélation anatomoradiologique a été effectuée. L'examen fut pratiqué sur un appareil CGR Magniscan de 0,5 Tesla avec une séquence d'inversion récupération (TI: 21 ms, Tr. 2 000 ms, Ti: 500 ms). Cette séquence nous paraît être celle qui visualise le mieux la substance grise, la substance blanche et le liquide cérébrospinal. Afin d'étudier la configuration des principales structures du tronc cérébral 8 plans de coupes horizontales et 3 sagittales sont proposés.
    Notizen: Summary Magnetic resonance imaging of 30 normal volunteers' posterior fossae was performed on a CGR Magniscan machine with a magnetic field strength of 0,5 Tesla. We chose the inversion recovery signal with a Tr of 2,000 ms, a TE of 21 ms and an inverse time of 500 ms. This sequence gives a better definition of the grey matter, the white matter and the CSF. To study the brain stem we suggest 8 horizontal and 3 sagittal sections.

    Notizen: Abstract In contrast to the only previously published description, the magnetic resonance appearance of a splenic hemangioma was markedly heterogeneous on T2-weighted images due to extensive infarction and large vascular pools. Magnetic resonance images correlated precisely with the pathological findings. The hemangioma caused thrombocytopenia, a well-documented albeit rare hematologic phenomenon associated with visceral hemangioma.

    Notizen: Summary Ninety-six magnetic resonance imaging (MRI) studies demonstrating solitary brain stem lesions were reviewed in order to establish distinguishing features between tumours and other lesions. Histological confirmation of the radiological diagnoses was obtained in 33% of patients. The morphology of the lesion rather than its signal characteristics was the most useful feature in differential diagnosis, except after haemorrhage. However, areas of abnormal T2 recovery time were significantly more extensive than areas of abnormal T1 recovery time in tumours at presentation. In other lesions and in tumours following radiotherapy induced regression, the extent of abnormal T1 and T2 signal was the same. Possible reasons for this observation are discussed. The accuracy of computed tomography (CT) and MRI were similar for lesions causing brain stem expansion but for small lesions MRI was more sensitive and provided better topographical information.

    Notizen: Summary A case of left atrial myxoma presenting exclusively with neurological symptoms, studied with magnetic resonance imaging (MRI) combined with cerebral angiography and computed tomography (CT) is reported. Typical angiographic findings suggested the diagnosis of myxoma. MRI showed multiple ischemic lesions disseminated throughout the entire brain, some of which had been clinically asymptomatic. Because of its sensitivity in identifying small cerebral infarcts, MRI should prove in the future to be a first-choice technique in the evaluation of the presence of and extent of cerebral involvement in embolic left atrial myxoma.

    Notizen: Summary A prospective study of 16 patients was performed to compare quantitatively a contiguous single slice 2DFT version with a 3DFT version of a short TR, variable flip angle, gradient echo (GRASS) pulse sequence. The 3DFT GRASS scans had higher signal-to-noise ratios (SNR) of cord and CSF compared to the single slice 2DFT GRASS scans. The 3DFT GRASS scans, however, had lower CSF-cord and CSF-disc contrast than the single slice 2DFT version. The 3DFT GRASS sequence demonstrated comparable contrast only on the end slices of an imaging volume suggesting influence of an entry phenomenon. The lower CSF-cord and CSF-disc contrast of the 3DFT GRASS technique diminished its usefulness in the diagnosis of cervical disc disease compared to the single slice 2DFT GRASS technique. Two different slice thicknesses (3 mm and 5 mm) were investigated with the 2DFT GRASS technique and found to be comparable although the 3 mm scans had sharper disc and dural margins because of less partial volume artifact.

    Notizen: Summary Eleven operated herniated disks in 10 patients were evaluated preoperatively with plain films, myelography and magnetic resonance imaging. Plain X-ray was a valuable supplement to MRI for studying the bony changes. Myelography showed 7 of 11 herniated disks while MRI gave correct diagnosis in all. It is concluded that MRI can replace myelography and computerized tomography in the preoperative evaluation of cervical herniated disk. The other examinations may be supplementary in some cases.

    Notizen: Summary The Gd-DTPA-enhanced magnetic resonance findings in two patients with herniated thoracic intervertebral discs are reported. The first patient was a 56-year-old woman with a small subligamentous T6-7 disc herniation, slightly lateralized to the right. The second patient was a 51-year-old man with a central and right posterolateral disc herniation, including a large calcified fragment, at the T8-9 level. The nonenhanced MR examination revealed the presence of an extradural mass lesion in both patients, impinging upon the dural sac and compressing and displacing the spinal cord posteriorly. The lesion was slightly hypointense on both T1- and T2-weighted spin echo sequences. Following intravenous injection of Gd-DTPA in a dosage of 0.1 mmol/kg body weight, enhancement of the posterior longitudinal ligament was noted and triangular areas of contrast uptake were seen to occur in the epidural space above and below the herniated disc. At surgery, they were found to correspond to dilated and congested epidural veins.

The view that CS is a dysmyelinating disorder is further substantiated.

    Notizen: Summary Ten patients with biochemical evidence of a hormonally active pituitary adenoma were examined by dynamic contrast enhanced computed tomography (CT) and then by pre and post Gadolinium-DTPA (Gd-DTPA) enhanced magnetic resonance imaging (MRI). Excluding one false positive case, CT and unenhanced MRI were comparable in the detection of microadenoma. Post Gd-DTPA examination gave more clear evidence of the actual adenoma in two patients and aided in the demonstration of a third. However, in two others all imaging techniques failed to demonstrate the microadenoma subsequently found at surgery. On the post enhancement MRI it was easier to assess the relationship of a tumour to the cavernous sinus and to visualise the relationships of the parasellar carotid arteries.

    Notizen: Summary Magnetic resonance (MR) images of nine patients with intracranial vertebral artery occlusion (five proved, four presumed) have been reviewed. In two of nine, both vertebral arteries were occluded and in five of the nine, the basilar artery was also blocked. All occluded arteries showed absence of flow void and were clearly demonstrated as mildly hypo-∼mildly hyperintense structures to brainstem parenchyma on the T1-weighted images. In two of five patients with basilar artery occlusion, retrograde filling of the distal basilar artery was detected. Thus, MR imaging, in particular the T1-weighted image, is a pertinent diagnostic modality for evaluating intracranial vertebral artery occlusion. Angiography does not seem to be required for confirmation.

    Notizen: Summary Two cases of large asymptomatic cysts of the lateral ventricle are presented. The cyst walls or cyst contours were demonstrated well by proton density-weighted or T2-weighted spin echo images. These cysts are being detected as incidental findings in increasing frequency with the increasing use of MR imaging. Our data and review of the literature suggest that surgical intervention is not necessarily warranted, but that careful clinical evaluation is indicated. The diagnostic problem of intraventricular cysts is also discussed.

    Notizen: Summary Brain stem type neuro-Behçet's syndrome was studied with enhanced CT and MRI during the acute and chronic stage of the illness. During the acute stage, brain CT revealed a low density lesion in the brain stem extending from the lower pons up to the midbrain ventrolaterally with marginal enhancement effect. T2-weighted image showed a high signal intensity lesion in the brain stem which mainly involved the basis ponti, tegmentum, tectum and cerebral peduncle. During the chronic stage, the lesion became low in signal intensity with T2-weighted image and reduced in its size without enhancement in brain CT. The prolonged relaxation time of the lesions was gradually normalized with steroid treatment. Sequential brain CT with enhancement and MRI study with T1- and T2-weighted images were useful to detect the lesions and to evaluate the activity in the neuro-Behçet's syndrome.

    Notizen: Summary Twelve patients with cerebral vascular malformations (5 cavernous angiomas, 1 thrombosed arteriovenous malformation, and 6 venous angiomas) were studied with magnetic resonance (MR) imaging. All lesions were clearly depicted. Characteristic MR findings were obtained mainly on T2-weighted images: a markedly low intensity area was always seen. The margins of arteriovenous malformation (AVM) and venous angioma were irregular while those of cavernous angioma were smooth in all planes on T2-weighted images. Gradient-echo (GrE) pulse sequences were more sensitive than T2-weighted spin echo (SE) in lesion detection. MR imaging could play an important role in the differential diagnosis of cerebral vascular malformations.

    Notizen: Abstract Sixteen patients with fibrous metaphyseal defects were examined with both plain radiography and magnetic resonance (MR) imaging. Depending on the age of the fibrous metaphyseal defects, characteristic radiomorphologic changes were found which correlated well with MR images. Following intravenous Gadolinium-DTPA injection, fibrous metaphyseal defects invariably exhibited a hyperintense border and signal enhancement. Healed lesions exhibited a transition to normal bone marrow. A line through the maximum longitudinal diameter of a fibrous metaphyseal defect invariably led to a point of tendinous or ligamentous insertion. Coronal MR images demonstrated the maximum longitudinal extension and the respective inserting tendon or ligamentous structure at the epiphyseal line.

    Notizen: Abstract A prospective magnetic resonance imaging (MRI) study was carried out in 13 patients (19 examinations) with primary bone tumours to assess the relative value of each of four pulse sequences in showing the extent and nature of the lesion. The four pulse sequences used were a T1-weighted spin-echo (SE544/44), a T2-weighted spin echo (SE1500/80), a short TI inversion recovery (STIR) (IR500/100/44), and a partial saturation (PS) (PS500/22) with field echo data collection. For soft tissue disease the combination of PS and STIR gave better definition of the boundary of the tumour than the more conventional T1 and T2-weighted spin echo sequences. For the demonstration of bone cortex, periosteal change and calcification, T1 and T2-weighted spin echo sequences were better. However, for calcified tissues, plain radiographs were better than either MRI combination. On the assumption that plain films will be available in all cases, PS and STIR sequences could therefore be substituted for T1 and T2-weighted spin echo sequences allowing an increase in soft tissue detectability for lesions in both red and yellow marrow.

    Notizen: Abstract Comparison of the radiographic signs of Scheuermann disease and the corresponding disc degeneration on thoracolumbar magnetic resonance (MR) images was made in 21 young patients. Marginal sclerosis, Schmorl nodes and narrowed disc spaces, but not irregular or wedge-shaped end-plates, were significantly associated with disc degeneration. Fifty-five percent of the discs in the patients with Scheuermann disease were abnormal on MRI, compared with 10% in asymptomatic controls. Our study confirms that thoracolumbar disc degeneration is enhanced in 20-year-old patients with low back pain who have radiological evidence of Scheuermann disease.

    Notizen: Abstract Magnetic resonance (MR) imaging is useful in evaluating a variety of spinal pathologies including intervertebral disc disease. Herniated discs are commonly believed to undergo premature degeneration and produce low intensity signal on T2-weighted images. We reviewed 154 patients who were studied for disc herniations or other pathology. Fifty-nine (38%) had disc herniations and 7 of these (5%) had a fragment that was hyperintense with respect to the adjacent intervertebral disc on T2-weighted images. It is hypothesized that some disc fragments may contain a higher water content that causes prolongation of the T2 signal. The bright signal which is therefore seen on long TR long TE (T2-weighted) images may be a useful sign suggesting herniation or extrusion.

    Notizen: Abstract Extremely low field magnetic resonance imaging (MRI) was evaluated in 16 patients with bone and soft tissue tumors, both benign and malignant. Twelve of the lesions were located in the lower extremity, 2 in the gluteal and 2 in the shoulder region. The findings were compared to the results of computed tomography (CT). Low signal-to-noise ratio and poor spatial resolution result in an image quality which is inferior to that obtained with MR imaging at higher field strengths. In spite of this, the diagnostic information seems to be at least comparable to that obtained from CT.

    Notizen: Abstract Thirty-four joints (19 knees, 15 wrists) of 31 patients suffering from rheumatoid arthritis and related disorders were examined prior to and following intravenous administration of Gadolinium-DTPA (0.1 mmol/kg body weight). T1-weighted spin-echo sequences and the gradient-echo technique FLASH were applied. FLASH scanning was used for the registration of the time-dependent changes of signal intensity following Gd-DTPA. Synovial proliferations exhibited a rapid and marked increase of signal intensity whereas fatty tissue, bone marrow, muscle and synovial effusion demonstrated only minor changes, causing enhanced contrast between synovial pannus and joint effusion or other neighbouring structures. Within the synovial pannus, ratios (absolute signal increase) of 131.3±53.4% and 122.9±51.1% were found in T1-weighted spin-echo and in FLASH sequences respectively. The average signal increase gradient of pannus (108.2±70.6%/min) was significantly (p〈0.001) different from muscle (13.4±7.8%/min), fatty tissue (10.2±8.4%/min), bone marrow (5.5±7.1%/min), and joint effusion (14.7±7.8%/min).

    Notizen: Summary Four cases of the cheiro-oral syndrome are reported, with a review of the clinical symptoms and signs and the neuroradiological methods used to demonstrate the responsible lesion. In each case, angiography, computed tomography (CT) and magnetic resonance imaging (MRI) were performed. The lesion was found in the thalamus in three cases and in the pons in one. Infarction had occurred in three cases and haemorrhage in one. Angiography revealed normal findings in all cases. CT at the onset of the symptoms demonstrated a small haemorrhage in the thalamus in one case but was not helpful in the others, and MRI was required to identify infarction. The anatomical sites responsible for the cheirooral syndrome have been reported to be in the central gyrus, in the thalamus, and in the brain stem. The clinical symptoms and signs reported in the literature and in our four cases are reviewed to evaluate aetiological factors and clinical features according to the three different sites of lesions causing this syndrome.

    Notizen: Summary Neurological, computed tomography (CT) and magnetic resonance imaging (MRI) findings were recorded from 13 patients with Japanese encephalitis (JE) in the Kurume region diagnosed by serological criteria. The patients averaged 63 years of age, and 5 were older than 70 years. The serological data mostly indicated a primary response. Hemiplegia and tetraplegia were common, together with extrapyramidal signs. A few cases had a stroke-like onset and cerebral haemorrhage during the course of JE. CT and MRI in 7 cases revealed abnormalities in the thalamus and basal ganglia including the putamen. The CT and MRI findings from the acute stage to the convalescent stage were considered to be characteristic of JE.

    Notizen: Summary Sixty multiple sclerosis (MS) patients (33 definite, 13 probale and 14 suspected were investigated by computed tomography (CT), magnetic resonance imaging (MRI), multimodality evoked potentials (EPs) and cerebrospinal fluid (CSF) electrophoresis. MRI abnormalities were found in 50 cases, while at least one abnormal evoked potential was detected in each of 52 cases. Brain-stem auditory evoked potentials were more sensitive than MRI for the detection of brainstem involvement. All the patients with oligoclonal bands had abnormal MRI and none of the patients with normal MRI had oligoclonal bands in the CSF. The number and the extent of MRI lesions were significantly correlated with the duration of disease and with the degree of disability. Our observations stress the importance of the combined use of MRI and EPs in detecting silent CNS lesions in MS patients.

    Notizen: Summary Four patients with palatal myoclonus (PM) were studied with magnetic resonance imaging (MRI). Increased signal intensity and bilateral enlargement of the inferior olives were recognized in two patients with bilateral PM, pontine haemorrhage and neuro-Behçet disease, and a similar olivary change on the contralateral side was noted in a case of pontine infarction with unilateral PM. These findings were consistent with the pathology. The changes were more obvious in proton density-weighted images than in T2-weighted images, which thus differ from the changes in common gliosis. The other patient with the syndrome of PM and progressive ataxia did not show any olivary change. These changes on MRI are considered to indicate pseudo-hypertrophy of the inferior olives, although this is not consistently shown by the imaging method.

    Notizen: Summary A cavernous angioma of the right optic tract in a 35-year-old man is presented. The patient suffered from headaches and had a left homonymous visual field defect after subarachnoid haemorrhage and an intracerebral haematoma in the right temporomedial region, revealed by computed tomography (CT). Follow-up CT showed a small contrast-enhanced lesion in the right suprasellar and parasellar cistern. Angiography on three occasions did not reveal a vascular lesion. Magnetic resonance imaging was helpful both for diagnosis and planning surgical therapy. It showed typical signs of a cavernous angioma of the right optic tract; the diagnosis was confirmed by surgery and histological examination. This appears to be the first reported case of a cavernous angioma of the optic tract.

    Notizen: Summary Five cases of chronic inflammatory demyelinating polyneuropathy are described in which cranial nerve involvement accompanied a more generalized neuropathy. Clinical, electrophysiological, radiological and nerve biopsy findings are presented. Cranial nerve lesions in this form of polyneuropathy may be related to lesions of the peripheral nerves or of the central nervous system, when they may be accompanied by MRI evidence of more widespread CNS demyelinating lesions. In cases of early onset, the occurrence of focal cranial nerve lesions may serve to distinguish chronic inflammatory from inherited demyelinating polyneuropathies.

    Notizen: Summary Five patients (aged 19–52 years) were treated for a midbrain syndrome due to acute carbon monoxide poisoning and had clinical follow-up investigations for up to 18 months. Three patients recovered with minor neurological and neuropsychological deficits and resumed their premorbid life-style. One patient had normal findings, while the fifth remained in a permanent vegetative state. Initial CT of the brain may fail to detect low-density lesions in the globus pallidus. If present, these lesions may either have disappeared, diminished, or remained unchanged at follow-up. Long-term outcome seems to show a closer link to white matter changes, which are at present best investigated by magnetic resonance imaging. The additional value of determining visual evoked responses at repeated follow-up is suggested by the present investigation.

    Notizen: Abstract Thirty-two cases of pineal cyst diagnosed by magnetic resonance imaging (MRI) were reviewed and are described. The pineal cyst was demonstrated to be an area with slightly less intensity than the surrounding tissue and with slightly greater intensity than the CSF on T1-weighted images. On the T2-weighted images this lesion was identified as a high-intensity area with smooth margins and was homogeneous in nature. In three cases presenting with headache, compression of the vein of Galen was identified, and compression of the quadrigeminal plate was demonstrated in five cases. No patients presented with both pineal and quadrigeminal lesions. Of the cases, 63% were not detected by CT scanning alone. There were two cases in which the cyst ruptured and collapsed spontaneously during follow-up. It is emphasized that the presence of this lesion, which was more frequent than previously expected, should be kept in mind when diagnosing pineal tumors and should not be misdiagnosed. Surgery should not be undertaken unless the lesion produces symptoms due to the compression of the quadrigeminal plate, aqueduct, or the vein of Galen.

It was determined to be a large temporal encephalocele with extensive invasion into the subtemporal, facial and cervical regions. This case is presented because of the unique pathology of this congenital malformation and the extensive surgery that was required. The management and the clinical and radiological features are discussed.

    Notizen: Summary The severity of Friedreich's ataxia was graded in ten patients by clinical examination and in five by use of posturography. These data were compared with neuroradiology findings. CT-confirmed infratentorial atrophy occured only in advanced cases of Friedreich's ataxia; the correlation with the clinical score was poor. On mid-sagittal MRI planes the diameters of fourth ventricle, brain stem at the level of the inferior olive and spinal cord at the levels of the foramen magnum and C3 were measured. Patients with Friedreich's ataxia had significant MRI-confirmed atrophy of the cranial spinal cord as compared with a normal, age-matched control group. This was also observed in patients with Friedreich's ataxia in the early stages. A reliable correlation between atrophy of the cranial spinal cord and the clinical score, however, could again not be found. MRI exploration of the cranial spinal cord may be recommended as an additional diagnostic marker in Friedreich's ataxia.

    Notizen: Abstract In 63 patients with primary extramedullary malignant lymphoma or plasmacytoma, a study was performed in order to evaluate bone marrow involvement. All patients underwent a 99mTc microcolloid bone marrow whole body imaging (scintigraphy), using a gamma camera interfaced with a computer, followed by nuclear magnetic resonance bone marrow imaging (MRI), (1.5 Tesla). MR images were made of the lumbosacral region, the pelvic region, both femoral and other parts of the skeleton, according to focal lesions in the scintigraphy. A posterior iliac crest bone marrow biopsy was used as a standard reference. In the present study, both scintigraphy and MRI showed a dissiminated or focal involvement or a combination of both. In 53 of the 63 patients (84%) the results were in accordance. Pathological MR signals or pathological findings in scintigraphy did not always correspond to tumorous bone marrow involvement, and were shown to reflect reactive changes in the central part of the skeleton in combination with a periphery radionuclide extention interpreted as a periphery compensatory hematopoetic proliferation. The negative predictive value of scintigraphy and MRI was 92% and 100%, respectively. When combining the results of both examinations, the positive predictive value increased from 49% to 58%, if the bone marrow biopsy is accepted as gold standard. The results indicate that bone marrow investigation performed simultaneously using scintigraphy and MRI is superior both to the use of either of the methods alone and to the traditional iliac crest bone marrow biopsy.

    Beschreibung / Inhaltsverzeichnis: Zusammenfassung Zwecks überprüfung, inwieweit die Kernspintomographie, die mit gutem Erfolg bereits in der präoperativen Diagnostik eingesetzt wird, eine sinnvolle Bereicherung des diagnostischen Instrumentariums bei der postoperativen Kontrolle thorakaler Aortenerkrankungen darstellt, haben wir zwischen dem 1. 4. 1986 und 30. 3. 1988 30 Patienten (20 männlich, 10 weiblich, mittleres Alter 53 Jahre ± 13,7 Jahre) nach Operation an der thorakalen Aorta kernspintomographisch untersucht. Es lagen 19 arteriosklerotische Aneurysmen, 10 Dissektionen sowie ein Tumor der , Aorta descendens vor. In allen 30 Patienten gelang es, kernspintomographisch postoperativ die Ergebnisse der chirurgischen Maßnahmen zu dokumentieren. Es konnten im frühen postoperativen Stadium Komplikationen ausgeschlossen bzw., wie in einem Fall eine retrograde Aortendissektion oder einem anderen ein Hämatoperikard nachgewiesen werden. In der postoperativen Verlaufskontrolle aortaler Dissektionen ließ sich eine aneurysmatische Dilatation der dissezierten Aorta distal der implantierten Prothese in allen Fällen darstellen. Wahres und falsches Lumen konnten differenziert werden. In einem Fall ließ sich die Ausbildung eines Aortenbulbusaneurysmas nachweisen. Zusammengefaßt lassen sich postoperative Komplikationen der thorakalen Aortenchirurgie mit der Kernspintomographie zuverlässig nachweisen und bildlich in 3 Ebenen darstellen. Nichtinvasibilität, das Fehlen einer ionisierenden Strahlung und das ausbleibende Risiko einer Kontrastmittelapplikation machten die Kernspintomographie zu einer idealen Methode für Folgeuntersuchungen nach Operation an der thorakalen Aorta.
    Notizen: Summary Operations of the thoracic aorta for aneurysms, dissections, or congenital malformations may lead to early or late complications. Therefore, postoperative control for documentation of the surgical results, for exclusion of early changes and for comparison with later controls is mandatory. To demonstrate that magnetic resonance imaging (MRI), which we already had used with good results for preoperative studies is also able to detect postoperative abnormalities, we examined 30 patients with thoracic aortic disease (20 male, 10 female, mean age 53 ± 13.7 years) with this diagnostic tool. There were 19 arteriosclerotic aneurysms, 10 dissections, and one aortic tumor. With MRI it was possible in all patients to visualize the results of the surgical treatment postoperatively. Early postoperative complications could be excluded, or demonstrated in one case of a retrograde aortic dissection or a hematopericardium. These pathological findings could also be shown by arterial digital subtraction angiography or echocardiography. Using MRI for follow-up of aortic dissections, the development of an aneurysm of the aortic root in a Marfan-patient could be detected. In all these patients, it was possible to differentiate true and false lumen and to detect the origin of major side branches. The diagnostic advantages of MRI, which we compared with other imaging methods, as echocardiography, computed tomography or angiography are that postoperative complications of thoracic aortic aneurysm surgery can be reliably detected and visualized in 3 different planes. Non-invasiveness, the omission of ionizing radiation and no risk of contrast media application underline that magnetical resonance imaging is an ideal method for follow-up after operation of the thoracic aorta.

    Notizen: Summary Twenty cases of intrapetrous lesions were studied by a 1.5 T magnetic resonance (MR) unit. In all cases, comparisons were made between MR, CT and clinical findings. Our present material included 9 cholesteatomas, 1 cholesterol cyst, 3 primary epidermoid carcinomas, 2 metastatic neoplasms, 1 glomus jugulare tumor and 4 facial neurinomas. Gadolinium was injected in 7 cases and seemed to be the best method for studying the intrapetrous tumors. MR permitted accurate topographic study and assessment of tumoral extension, as well as a ready demonstration of tumor vascularity. The present findings also showed that MR is not capable of defining small bony detail or calcifications.

A neuroelectric model employing Frankenhaeuser-Huxley membrane nonlinearities is used to derive excitation thresholds for monophasic and biphasic pulse sequences, as well as sinusoidal stimuli. The model, along with principles of magnetic field induction, is used to derive criteria of acceptability for exposure to time-varying magnetic fields. Applications to pulsed gradient fields from magnetic resonance imaging devices are discussed.

    Notizen: Abstract During the last two and a half years 60 patients with orbital space occupying neoplasms were treated surgically at the Department of Maxillo-facial Surgery of the University Clinic of Essen. With modern imaging techniques exact information on the location and extent of space occupation was available before the operation. Therefore the surgical approach could be optimized. Despite the various and sometimes inaccessible tumor locations, transcranial operation was only necessary in six patients. In the remaining 54 patients anterior orbitotomy allowed either a definitive tumor therapy or histological verification of the diagnosis.

    Notizen: Summary One hundred patients with CT-proven intracranial disease have been studied by magnetic resonance imaging (MRI) before and after intravenous injection with Gadolinium-DTPA (Gd-DTPA), in order to assess the role and clinical efficacy of Gd-DTPA. T2-weighted spin echo sequences, although sensitive to the detection of intracranial disease, in general fail to differentiate macroscopic tumour from oedema. Following Gd-DTPA, T1-weighted spin echo sequences in primary tumours demonstrated a variable degree of contrast enhancement unrelated to histological type. Small tumours, especially acoustic neuromas and meningiomas in the posterior fossa, were rendered more conspicuous. Optimum time for scanning was between five and 25 min following injection for all lesions except those adjacent to normal enhancing structures such as nasal/sinus mucosa and pituitary gland when delayed scans up to 45 min were necessary. No differences were observed between the 0.1 and 0.2 mmol/kg Gd-DTPA concentrations used and no complications attributable to Gd-DTPA were detected. Clinical advantages of Gd-DTPA include shorter scan times, macroscopic tumour/oedema separation and improved detection of certain tumours, particularly acoustic neuromas.

    Notizen: Summary Magnetic resonance imaging (MRI) findings of 20 patients with clinically definite multiple sclerosis (MS) are presented. The studies were performed on a 0.5 Tesla magnet using spin-echo technique. Analysis of the MRI findings included detailed linear measurements of the ventricular and the subarachnoid spaces and reading of the intensity of the grey and white matter and intensity of the MS plaques. The plaques were sorted according to their number and size. The younger patients (20–40 years) had overall more plaques than the older ones (over 40 years). The small plaques were the most numerous and the large ones were the least common. Statistically significant association was found between the number of plaques and the cella media width. The intensity ratios between the nonplaque white matter/grey matter showed a significant correlation with the ventricular score. A significant negative correlation was found between the antero-posterior diameter of the spinal cord and the number of MS plaques in the brain. The plaque/white matter ratio had a significantly negative correlation with the cervical cord's width.

    Notizen: Summary Four patients with dural arteriovenous malformation (AVMs) draining into the cavernous sinus, who presented ophthalmic manifestations, were studied by magnetic resonance (MR) imaging. In all patients signal decrease in the involved cavernous sinus was demonstrated in coronal spinecho (SE) imaging. It is attributable to rapid venous flow in the sinus, and this “high velocity signal loss” is a fairly pathognomonic finding in this condition. We stress the validity of MR imaging in the primary diagnosis of dural AVMs with ophthalmic symptoms.

    Notizen: Summary A case of thoracic myelopathy due to ossification of the ligamentum flavum is presented. A fifty-five year old female complained of sensory and motor disturbance of lower limbs. Neuroradiologically, in addition to ossification of the posterior longitudinal ligament throughout the spines, ossified ligamenta flava were found at the Th10–11 level and magnetic resonance imaging (MRI) clearly showed the spinal cord compressed at the same level by a low-intensity-signal lesion. Laminectomy and removal of the ossfied ligamenta flava improved her neurological dysfunction. The usefulness of the MRI in the neuroradiological diagnosis was confirmed.

    Notizen: Summary Among 110 patients (45 men, 65 women), aged 15 to 66, with clinical and/or biological diagnosis of multiple sclerosis (MS), severe to moderate corpus callosum (CC) atrophy was observed in 67 (60%) patients. Correlation between CC atrophy, brain atrophy, duration and severity of clinical symptoms, and high signal white matter areas, was carried out in 90 patients. Mean age was 46 years for those with severe CC atrophy, and 33 years for those without atrophy. Mean duration of the disease was 14 years in patients with severe atrophy, and 5 years in patients without atrophy. Severity of clinical symptoms is more pronounced in patients with severe CC atrophy. Numerous or large white matter high signal areas are observed in patients with severe CC atrophy on T2-weighted images. CC atrophy appears earlier than brain atrophy in the course of MS.

Two histologically confirmed cases were studied by computerized tomography (CT) and magnetic resonance (MR) imaging. T2 weighted, sagittal MR images were superior to CT in delineating the tumor from surrounding grey matter. The lesion was isointense to grey matter on T1 weighted images allowing exclusion of other hypothalamic tumors. MR will undoubtedly become the imaging modality of choice in the detection of hypothalamic hamartoma.

Metrizamide-enhanced spinal CT scan showed an intramedullary lesion at the level of T1-T2; this had expanded the cord in fusiform fashion but showed no evidence of a cystic component. Surgical resection was performed and the pathological diagnosis was cavernous hemangioma. Two and one-half years later, her left hemisensory deficit was worsening and a spinal MRI showed high signal intensity mass in the region of the previous surgery consistent with chronic hematoma which was re-evacuated with some improvement in the patient's neurological condition.

    Notizen: Summary Magnetic resonance (MR) images of 29 consecutive patients with intraspinal neoplasms (9 intramedullary tumors, 20 extramedullary tumors) were reviewed to evaluated the utility of MR imaging in distinguishing the intraspinal compartmental localisation and signal characteristics of each lesion. Compartment and histology of all neoplasms were surgically proven. MR correctly assigned one of three compartments to all lesions, 9 intramedullary, 14 intradural extramedullary (6 schwannomas, 3 neurofibromas, 5 meningiomas), and 6 extradural (3 schwannomas, 1 meningioma, 1 cavernous hemangioma, 1 metastatic renal cell carcinoma). All intramedullary tumors showed swelling of the spinal cord itself. In all five extradural tumors a low intensity band was visualized between the spinal cord and tumor. On the other hand, a low intensity band was demonstrated in no cases with intradural tumors. Visualization of this low intensity band is important in differentiating extradural from intradural-extramedullary lesions. We call this low intensity band, “the extradural sign”. Signal intensity of intradural tumors varied with histology. In extramedullary tumors, signal intensity of schwannomas was similar to that of the cerebrospinal fluid (CSF) both on T1 weighted (inversion recovery) and T2 weighted spin echo (SE) images. On the other hand, meningiomas tended to be isointense to the spinal cord on both T1 and T2 weighted SE images. We found relatively reliable signal characteristics to discriminate meningioma from schwannoma.

    Notizen: Summary Magnetic resonance scans of 74 patients with multiple sclerosis participating in a controlled trial were compared 6 months before and at the end of a 24–32 months-treatment period with either Cyclosporin A (n=31) or Azathioprine (n=43). Both qualitative rating and computation of lesion volume showed deterioration in more than 40% of the patients, while by clinical criteria only 10–30% were worse. No significant difference was noted when the two treatment groups were compared. If careful repositioning and standardized image parameters are used, MRI is an indispensable tool for the objective determination of disease progression in MS although it cannot replace clinical examination.

    Notizen: Summary Twelve patients with different diseases of the brain were examined with sodium and proton MRI at 1.5 Tesla and the results of both studies compared. Due to the 1500-fold lower concentration in the body, the decreased sensitivity and the shorter relaxation times of sodium compared with hydrogen a sequence with gradient reversal and a volume imaging method was applied to achieve a short echo time (4.5 ms). As a result of the mainly extracellular distribution of sodium the CSF spaces are depicted by their high signal intensity while the normal parenchyma is not visible. Changes induced by encephalitis, ischemic infarction and tumors can also be detected by their increased sodium content. Different tumors provided different signal intensities, not always permitting a distinction between tumor, surrounding edema and CSF. The diagnostic impact of these findings is discussed.

    Notizen: Summary Results of MRI at 0.15T in twelve successive patients with intracerebral haematoma are reviewed. Using T2 weighted spin echo (SE) and partial saturation (PS without a refocussing 180° pulse) sequences, low intensity areas were seen in eleven of the twelve cases. These included central regions (three cases), a peripheral rim (seven cases) and more diffuse patterns involving the brainstem and cerebral hemispheres (two cases). One case initially displayed a peripheral rim and later a central low intensity region. Central low intensity regions were seen in acute, subacute, and chronic cases. Follow up in five cases displayed an increase in signal within the haematoma in three cases and a decrease in signal intensity in two cases. Low signal intensity areas can be seen within and around intracerebral haematomas imaged with T2 weighted sequences at low field strength.

All patients showed a highly significant reduction in CSF volume in accordance with reexpansion of the brain after alcohol abstinence. T2 values for white matter, estimated by linear regression from 16 echoes of a CPGM sequence, however, showed no significant increase such as occurs in rehydration. This indicates, that alcohol-induced reversible brain atrophy cannot be attributed to fluctuation of free water in the brain only.

    Notizen: Abstract Acquired cystic kidney disease (ACKD) occurs in adult patients undergoing long-term dialysis. Early detection is important because clinically significant hematuria and malignancies are associated with ACKD. We evaluated by magnetic resonance imaging (MRI) and ultrasonography (US) the incidence of ACKD in 15 patients aged 7.3–21.6 years (mean 15.9 years) with non-cystic primary renal disease. Nine patients had been treated with peritoneal dialysis only, and 6 with both hemodialysis and peritoneal dialysis for 24–73 months (mean 37 months). Three patients (20%) had no cysts. In 5 patients (33%) with bilateral multiple cysts, the diagnosis of ACKD was made by MRI and US. In another 5 patients, solitary cysts were localized to one kidney by MRI, and in 2 patients solitary cysts were seen in both kidneys. This study documents that ACKD is not limited to older patients with end-stage renal disease. Early detection of these cysts can be accomplished by MRI and is warranted since 1 patient developed neoplastic tubular changes which can precede tumor formation.

In a patient with known or suspected aggressive fibromatosis, every other soft tissue nodule or mass in the same limb has to be regarded as an additional tumor of the same histology.

    Notizen: Abstract Diagnosis of chondromalacia of the patellofemoral joint using three-dimensional gradient-echo sequences was investigated in 41 patients, with arthroscopic verification in 25 patients. In vitro examinations in human caderveric patellae were performed in order to determine optimal imaging parameters. FLASH (TR=40 ms, TE=10 ms, flip angle=30°) and FISP (TR=40 ms, TE=10 ms, flip angle=40°) were used in clinical studies. Advanced stages of chrondromalacia could be visualized with high sensitivity. The therapeutically relevant differentiation of major and minor degrees of chondromalacia seems to be possible. 30° FLASH-images in the axial plane proved to be the most efficacious technique for the diagnosis of chondromalacia.

The tumour size, shape and mobility in the ventricular cavities were more precisely determined by two-dimensional echocardiography than electrocardiogramgated MRI, while the extent of tumour mass at the apex was more clearly delineated by MRI. As two-dimensional echocardiography provides real-time imaging of cardiac anatomy without sedation, it is useful for initial evaluation of cardiac masses in neonates with genetic predisposition to tuberous sclerosis.

    Notizen: Summary Progressive decline of anterograde memory functions has been increasingly recognized as a frequent symptom in chronic multiple sclerosis. In order to investigate the brain structures involved, magnetic resonance imaging was performed in 20 patients. Neuropsychological assessment included the WAIS and WMS subtests information, picture completion, similarities, digit span, logical memory, and paired associate learning. All patients with severely impaired memory functions (n=5) showed bilateral lesions in the medial temporal lobe, whereas in those patients with moderate (n=10) or no measurable impairment of memory testing (n=5) either no lesions were seen in the medial temporal lobes or these lesions were restricted to one side. A post hoc cluster analysis strikingly confirmed these results. The differences could not be related to the age of the patients, the disease duration, or the level of education. Extensive lesions in the white matter of the frontal lobes, thinning and lining of the corpus callosum, and bilateral involvement of the anterior cingulate gyrus had no bearing on the neuropsychological results. These findings indicate that bilateral demyelination in the hippocampal regions is the most likely explanation for the impairment of anterograde memory in such patients.

    Notizen: Abstract Chronic patellar tendinitis can be a frustrating diagnostic and therapeutic problem. This report evaluates seven tendons in five patients with chronic patellar tendinitis. The etiologies included “jumper's knee” and Osgood-Schlatter disease. In all cases magnetic resonance images (MRI) showed thickening of the tendon. Some of the tendons had focal areas of thickening which helped establish the etiology. All cases had intratendinous areas of increased signal which, in four cases, proved to be chronic tendon tears. MRI is useful in evaluating chronic patellar tendinitis because it establishes the diagnosis, detects associated chronic tears, and may help determine appropriate rehabilitation.

    Notizen: Abstract From 1984 to 1987 magnetic resonance (MR) imaging was performed on 100 patients suffering acute spinal trauma. MR demonstrated one or more injuries to the cervicothoracic region in 31 patients. It displayed a spectrum of spinal cord injury ranging from mild compression and swelling to complete transection. MR was also useful in evaluating alignment at the cervicothoracic junction, in depicting ligamentous injury, in establishing the presence of disc herniation, and in identifying unsuspected levels of injury. We present a diagnostic algorithm that incorporates the role of MR in evaluating acute cervicothoracic spinal trauma and emphasizes the replacement of myelography by MR in the initial assessment of neurologic deficit.

He became comatose, mydriatic, and was admitted to hospital in an apneic state. He woke up after 7 h and developed Parkinsonism in the following weeks. This regressed slowly during the 2 months after the poisoning apart from dysarthria, bradykinesia of the upper limbs, and very brisk monosynaptic reflexes. At 3 weeks after the intoxication, computerized tomography was largely normal, and there was CSF-dense hypodensity in both putamina after 5 months. Sharply delimited signal elevation in T2 corresponding to the two putamina was detected by magnetic resonance imaging 8 weeks and 5 months after ingestion of the poison.

    Notizen: Summary There is still a need to prove that even static magnetic fields up to 1.5 T used in magnetic resonance imaging (MRI) are biologically safe and harmless for humans. Recordings of median and ulnar nerves and brain-stem auditory evoked potentials in 20 patients were completed prior to and after MRI investigation of the central nervous system. Neither the somatosensory nor the auditory evoked potentials exhibited any significant change of latencies, interpeak latencies or amplitudes. Since these electrophysiological parameters are highly dependent on the quality of nerve conduction and integrity of information processing in various nuclei, it may be assumed that MRI causes no lasting changes in either respect.

    Notizen: Summary The efficacy of magnetic resonance imaging (MRI) in the diagnosis of diseases of the central nervous system is reviewed. MRI, computed tomography (CT) and certain radionuclide studies are compared in the evaluation of intracranial tumours, cerebral vascular disease, multiple sclerosis and other white matter diseases, dementia, head injury, infection, epilepsy, spinal lesions and in paediatric central nervous system disorders. The measurement of cerebrospinal fluid volumes and dynamics by MRI is discussed. MRI most clearly has advantages where CT is degraded by bone hardening and streak artefacts (spine, skull base, posterior and temporal fossa, sella and parasellar regions) and in diseases in which the X-ray attenuation of the suspected lesion differs little from normal parenchyma (paediatric brain disorders, demyelination and dysmyelination, early oedema associated with infarction, infection or low-grade infiltrating neoplasm, subacute and chronic haemorrhage and lesions in the spinal subarachnoid space and cord). Elsewhere MRI and CT should be seen as complementary rather than competitive methods of imaging. In spite of an absence of information about the contribution of MRI to management decisions and a lack of rigorous, prospective controlled trials, MRI will play an increasing role in the diagnosis of diseases of the central nervous system.

    Notizen: Abstract Magnetic resonance imaging (MRI) studies were obtained in 49 cases of various diseases in the maxillo-facial region and the results compared with X-ray computed tomography (CT). Bot MRI and CT scans were performed in 44 cases. In 31 cases (70%), MRI provided similar or superior information to that of CT in the evaluation of various diseases. In 10 cases, MRI was superior to CT in defining the images of diseases, 7 cases of these were carcinomas. CT was superior to MRI in 5 cases, especially in demonstrating calcifications and bony changes. Artifacts caused by prosthetic metals and patient motion disturbed MR images. Our experience with MRI lead us the conclusion that it holds great potential and that it already had some advantages over CT in the maxillo-facial region.

    Notizen: Abstract Gradient-field-echo technique was employed to evaluate the movement of the temporomandibular joint meniscus and the related structures combined with the opening of the mouth for asymptomatic TM joints in the use of a 0.5 T super-conductive MR system. The examination for the mouth opening phase took less than one minute. The movement of the meniscus was clearly presented especially by using a cine-mode display. The signal intensity in the retro-meniscus and the lateral pterygoid muscle was increased by the opening. These findings indicate that the MR dynamic imaging may be promising in diagnosing not only the shape of the meniscus but also the function of the TM joint.

L'utilisation de coupes en double obliquité permet l'obtention d'incidences similaires à celle de l'échographie. Ces mêmes incidences peuvent être obtenues in vivo. L'étude morphologique du cœur est ainsi extrêmement précise, de même que l'étude des volumes cardiaques.
    Notizen: Summary The authors present a correlation study between the anatomy of the heart and its appearance with magnetic resonance imaging (MRI) and ultrasonography (US). Six hearts were studied by MR, then sliced along the same planes. The results are presented and compared with the data obtained in volunteers by MRI and ultrasonography. The correlation between the MRI of isolated hearts and their anatomic slices is excellent, the thiniest anatomic details are reproduced. The use of double oblique slices provides incidences similar to those of ultrasonography. The same incidences can be obtained in vivo. The morphologic study of the heart by these imaging techniques is thus very precise, as well as the study of cardiac volumes.

    Notizen: Abstract Dysphagia due to CNS pathology usually stems from one of two patterns of disease: (1) bilateral corticobulbar tract dysfunction (“pseudobulbar palsy”) or (2) pontomedullary dysfunction (“bulbar palsy”). Computed tomography (CT) has proved to be useful for evaluating the brainstem in patients with neurogenic dysphagia. Nonetheless, artifacts are common in CT imaging of the posterior fossa. Also, direct sagittal imaging is not usually obtainable by CT in adult patients. Magnetic resonance imaging (MRI), in contrast to CT, simultaneously gathers sequential images in the same plane and can obtain direct reconstructions in any plane of interest. MRI has proven to be more sensitive than CT in demonstrating lesions of the brain, such as demyelinating (e.g., multiple sclerosis) and ischemic diseases, (Brant-Zawadzki et al. 1984, Bradley et al. 1984, Bydder et al. 1982, Sheldon et al. 1985) as well as neoplastic masses that may produce neurogenic dysphagia (Lee et al. 1985, Zimmerman et al. 1986). Five patients with dysphagia are reported for whom MRI was valuable in detecting and characterizing their lesions of the brainstem and the cerebral hemispheres.

    Beschreibung / Inhaltsverzeichnis: Résumé Habituellement, les coupes frontales du foie en résonance magnétique sont moins utilisées que les coupes passant par les autres plans de l'espace. Des coupes frontales du tronc en résonance magnétique intéressant le foie, réalisées chez plus de 80 patients pour des indications diverses, ont été confrontées avec des coupes frontales du tronc faites sur 10 cadavres. Un schéma général de l'anatomie du foie étudiée dans le plan frontal a été établi. Les coupes frontales en résonance magnétique permettent de faire une très bonne estimation de la morphologie et du volume du foie, et d'en reconnaître les variations individuelles. Elles montrent bien certains rapports inférieurs du foie. Surtout, les coupes frontales en résonance magnétique permettent d'identifier la plupart des principales veines du foie, veines sus-hépatiques et branches de la veine porte et de bien étudier toute la portion rétro-hépatique de la veine cave inférieure. Certaines images vasculaires sont retrouvées presque constamment d'un sujet à l'autre sur les coupes. Le foie droit se prête mieux à cette étude que le foie gauche, en raison de sa morphologie et de la disposition de ses veines. Les coupes frontales du foie en résonance magnétique représentent un moyen privilégié pour étudier l'anatomie du foie. Associées à des coupes transversales, elles permettent de préciser le siège et les rapports veineux à l'intérieur du foie d'un processus pathologique, en vue d'une hépatectomie.
    Notizen: Summary In general, frontal sections of the liver in magnetic resonance imaging are used less than sections passing through other planes of space. Frontal sections of the trunk in magnetic resonance imaging involving the liver, performed in over 80 patients for various reasons, were compared with frontal sections of the trunk made in 10 cadavers. A general schema was established of the anatomy of the liver studied in the frontal plane. Frontal sections in magnetic resonance imaging make it possible to form a very good estimate of the structure and size of the liver, and to recognize individual variations. They clearly show certain inferior relations of the liver. In particular, frontal sections in magnetic resonance imaging make it possible to identify most of the main veins of the liver, the main lobar veins and branches of the portal vein, and to properly study the entire retrohepatic portion of the inferior vena cava. Some of the vascular images were found almost constantly in the sections of the various subjects. The right lobe of the liver is more accessible to such study than the left by reason of its structure and its venous arrangements. Frontal sections of the liver in magnetic resonance imaging constitute a preferential method for studying the anatomy of the liver. Together with transverse sections, they make it possible to specify the site and venous relations of a pathologic process within the liver, with a view to hepatectomy.

The MR findings were different in the two cases for reasons not yet fully explained, full biochemical and biophysical analyses of the cyst contents not being available. Neuropathologically, the only significant difference was the abundant presence of cholesterin crystals in the colloid of case 1 and their absence in case 2. One of our cases is also peculiar from a CT point of view, since it was primarily hypodense and did not enhance on intravenous contrast administration.

In the majority of cases (n=11) MR is superior to CT because it allows earlier detection of the disease, a more exact definition of the spread and a more detailed representation of complex inflammatory processes. MRI should rank first among all examination methods, especially in the determination of herpes simplex-encephalitis as it allows earlier detection and treatment of this disease.

MRI proved to be a sensitive method for detecting brain lesions (clinically silent or manifest) in these patients. The most frequent abnormalities were periventricular lesions seen in 12 cases. Such changes are not specific for vascular disease, and are often seen in multiple sclerosis. However, additional changes were commonly seen which suggested the correct diagnosis.

    Notizen: Summary Five patients (1 female and 4 males) with cerebral infarction of 4 h to 27 months duration were studied 9 times with magnetic resonance (MR) using Gd-DTPA. Spin-echo (SE) MR images (MRI) were obtained before and after the administration of Gd-DTPA, and correlative CT scans were performed on the same day. In 2 cases, 4 h and 27 months after the ictus, there was no enhancement with Gd-DTPA. There was faint enhancement in 2 cases with cerebral infarction of about 24h duration and obvious enhancement in all cases in the subacute stage. Compared with enhanced CT, MR using Gd-DTPA demonstrated more obvious enhancement of infarcted areas. MR enhancement using Gd-DTPA showed a gradual increase and the accumulated Gd-DTPA in infarcted areas slowly diffused to the periphery. MR enhancement with Gd-DTPA is similar to that of enhanced CT, but may be more sensitive in the detection of blood brain barrier breakdown.

The most common abnormalities were ventricular enlargement (four patients) and diffuse periventricular white matter changes (three patients). Infarcts were seen in three patients and mass lesion in one. The lesions were seen in both T1 and T2 weighted images. The results are compared with CT findings. MRI seems to be more sensitive than CT in detecting white matter changes and infarcts. Ventricular enlargement and granulomas were equally well seen with both modalities.

    Notizen: Summary In seven patients with temporal bone fractures examined by both CT and MRI, thin section CT proved superior to MRI in demonstrating the full extent of the fractures and the status of the ossicular chain. MR studies were able to demonstrate fractures, when these fractures contained blood or CSF, and the presence of ossicular dislocation in one case where the middle ear was completely filled with CSF or blood. Admixture of air in the middle ear gave a false impression of ossicular dislocation, while air in the fracture obscured portions of it. MR proved superior to CT in the evaluation of intracranial contents by showing 5 additional subdural hematomas, 2 epidural hematomas and 2 hemorrhagic contusions.

    Notizen: Summary Three intracranial catecholamine-secreting paragangliomas are described. They involved a glomus jugulare, a sphenopalatine ganglion and the clivus and upper cervical spine respectively. The extent of the tumours was shown by CT and MRI. They were all highly vascular with a substantial blood supply from systemic arteries which was subjected to particulate embolisation, followed in two cases by surgery. The importance of studying any tumour which could possibly be a paraganglioma for hormonal and, especially, for catecholamine secretion prior to any invasive procedure, including angiography and embolisation, is emphasised: all such procedures should be covered with catecholamine blocking agents and, in addition, carefully monitored with resuscitation facilities immediately available.

The results of cavernous sinography, dynamic contrast enhanced Computed Tomography (CT) and Magnetic Resonance Imaging (MRI) showed good correlation with each other and with the appearances found at operation.     Notizen: Summary Disseminated lesions in the white matter of the cerebral hemispheres and confluent lesions at the borders of the lateral ventricles as seen on MRI are both considered acceptable paraclinical evidence for the diagnosis of multiple sclerosis. Similar changes are, however, also found in vascular diseases of the brain. We therefore aimed at identifying those additional traits in the infratentorial region, which in our experience are not frequently found in cerebrovascular pathology. We evaluated MR brain scans of 68 patients and found pontine lesions in 71% of cases with a clinically definite diagnosis (17 out of 24) and in 33% of cases with a probable diagnosis (14 out of 43). Lesions in the medulla oblongata were present in 50% and 16%, respectively, and in the midbrain in 25% and 7%, respectively. With rare exceptions all brainstem lesions were contiguous with the cisternal or ventricular cerebrospinal fluid spaces. In keeping with post-mortem reports the morphological spectrum ranged from large confluent patches to solitary, well delineated paramedian lesions or discrete linings of the cerebrospinal fluid border zones and were most clearly depicted from horizontal and sagittal T2 weighted SE-sequences. If there is a predilection for the outer or inner surfaces of the brainstem, such lesions can be considered an additional typical feature of multiple sclerosis and can be more reliably weighted as paraclinical evidence for a definite diagnosis.

    Notizen: Summary Combined Positron Emission Tomography (PET) and Proton Magnetic Resonance Imaging (MRI) study were performed in six patients with chronic supratentorial stroke to investigate whether remote hypometabolic regions revealed by PET showed any abnormality on MRI. Either regional oxygen consumption (n=4) or glucose utilization (n=2) were measured using PET and the 15O steady state 18FGD technique, respectively. Four patients, with deeply located brain lesions, showed a significant metabolic reduction in the overlying cerebral cortex. In the remaining two patients, affected by a large cortical infarct, there was a significant crossed cerebellar hypometabolism. The MRI weighted by the parameters spin density (ϱ), spin lattice (T1) and spin-spin (T2) relaxation times were obtained employing various sequences in the same subjects. In no patient did the MRI show any contrast modification in these hypometabolic remote regions, suggesting that subtle loss of tissue and/or biochemical change do not underlie the reduction in metabolic rate.

    Notizen: Summary Pelizaeus-Merzbacher's disease is a progressive encephalopathy with demyelination of the cerebral white matter. The diagnosis can not be made on clinical or biological grounds: pathological investigation is necessary to confirm tigroid demyelination. CT scanning failed to visualize this type of anomaly but detection is now possible with the advent of magnetic resonance imaging (MRI). The authors studied the case of a boy who, at the age of 8 presented with symptoms characteristic of the disease: rotatory nystagmus, progressive encephalopathy, and inherited X-linked recessive traits. Magnetic resonance imaging revealed a high signal in the supra-tentorial white matter and the usual contrast was inverted. The authors believe that MRI can make an important contribution to the diagnosis of the disease.

MRI was superior to CT and plain films in demonstrating areas of tissue inhomogeneity within the tumor as well as soft tissue extension. CT was superior in demonstrating cortical thinning. Multiplanar imaging capability and visualization of articular cartilage may demonstrate intra-articular tumor spread. The characteristic MRI findings with short TR/TE (T1-weighting) and long TR/TE (T2-weighting) are described. We also describe one case where serial MR scans were used to assess response to therapy.

    Notizen: Abstract Three cases of lytic, calcified epiphyseal lesions with plain film and computed tomography features suggestive of chondroblastoma were imaged by magnetic resonance imaging. Histopathologic correlation was obtained in each case. Two cases of chondroblastoma showed low signal intensity on both short (TR600/TE20ms) and long (TR2500/TE80ms) spin echo (SE) images. The third case, a clear cell chondrosarcoma, demonstrated increased signal intensity on moderately T2 weighted (TR2500/TE40ms) images. These findings suggest that magnetic resonance imaging may be helpful in distinguishing these lesions.

    Notizen: Abstract Early magnetic resonance (MR) experience in the evaluation of 14 consecutive long bone intramedullary osteosarcomas demonstrates the need for T1 and T2 weighted pulsing sequences in the staging of this disease. Intramedullary disease is best depicted by coronal T1 weighted pulsing sequences and subtle extra-compartmental discase by T2 weighted axial imaging. Both high intensity and low intensity intra-medullary signals were noted on T2 weighting, while all T1 weighted pulsing sequences showed intra-medullary disease to have low signal intensity. Extraosseous tumor on T2 weighting usually had a high signal, and disease extent was therefore sharply demarcated from uninvolved muscle and its relationship to vessels confidently assessed. MR appears optimally suited for local staging of osteosarcoma, further enhancing the role of radiology in planning limbsalvage surgical techniques.

    Notizen: Abstract We report the time-dependent magnetic resonance imaging (MRI) changes that resulted from an intramuscular injection of a commonly-used pediatric sedation regimen (“DPT”). These changes at the site of injection consist of a focal abnormality characterized by a slight increase in signal intensity on T1 weighted images and markedly increased signal intensity on T2 weighted images. Alterations in signal are detectable almost immediately after the injection and progress over the first 31 hours. This abnormality, which could be mistaken for real disease, persists up to 36 days following injection.
